Abstract:
OBJECTIVE: To discuss the approach for specialized clinical pharmacists to carry out clinical pharmaceutical care.METHODS: The experience of providing pharmaceutical care for one patient with unstable angina pectoris was introduced.RESULTS CONCLUSION: The suggestions put forward by clinical pharmacist on rational drug use achieved satisfactory results.On the one hand,clinical work calls for the help from specialized clinical pharmacists,on the other hand,clinical pharmacists are not independent of clinical work.Keywords:

Objective To explore approaches for clinical pharmacists to provide clinical pharmaceutica1 care in department of cardiology. Methods The clinical cases were summarized by analyzing the clinical pharmacists' experience of clinical pharmaceutical care. Results The work of clinical pharmacists were multifaceted,including clinical case analysis,drug interactions,rational use of antibiotics for special populations,drug therapy selection,report of adverse drug reactions,and patient education in order to promote rational drug use and ensure patient medication safety.Conclusion Clinical pharmacists' involvement in the cardiovascular internal medicine wards can effectively assist doctors to provide high quality of pharmaceutical care for patients.

OBJECTIVE: To explore the process and approaches and role of clinical pharmacist in carryinng out pharmaceutical care for diabetic patient following undergoing renal transplantation. METHODS: During ward round,the clinical pharmacist detected the problems in the treatment process of one typical diabetic case following undergoing renal transplantation, then he carried out pharmacy intervention and pharmaceutical care from aspects of drug selection,usage dosage,drug interaction,adverse drug reactions,and carried out patient education to promote safe and effective medication. RESULTS CONCLUSIONS: Reviewing medical orders and providing medication education for patients are clinical pharmacists' basic job. It is important for pharmacists to have a good command of professional knowledge,drug package inserts,clinical guidelines and expert consensus,besides specialization,they also need to grasp the clinical features and drug use characteristics of the other common diseases,communicate and collaborate with physicians and nurses for guarantee of rational use of drugs.

Objective To discuss the role of clinical pharmacists in clinical practice through the pharmaceutical care for 1 patient with chronic glomerulonephritis.Methods The clinical pharmacists actively assisted physicians in finding out the etiology,putting forward therapeutic schemes,and carrying out pharmaceutical care and education for the patient.Results Pharmaceutical care from clinical pharmacists for patients with chronic glomerulonephritis was helpful for clarifying the etiology,improving patient adherence and healing.Conclusion Clinical pharmacy service can provide effective help in rational drug use.

Non-adherence to prescribed treatment by the patient is one of the main pharmacotherapeutic problems. Pharmacists with their knowledge of drugs are in an ideal position to identify and solve problems related to non – compliance and can have a beneficial effect on the outcome of various therapies by providing quality pharmaceutical care. Aim: To study and analyze the opinion of pharmacists about their role, in the context of pharmaceutical care, for the patient’s adherence to the prescribed drug therapy. Methods and material : Documentary method, sociological method, statistical method. Results: Nearly 85% of pharmacists believe that only half of patients adhere to their prescribed therapy. The data from the study shows that 76.6% of patients often seek the opinion of the pharmacist, as the most accessible medical professional and source of the health information on the treatment of the disease. The majority of the surveyed pharmacists (83.1%) indicate that the role of pharmaceutical care in adhering patients to drug therapy is to improve the quality of life of patients and ensure rational drug use (78.2%) and increase satisfaction with patients (66.1%). Conclusion: By providing quality pharmaceutical care, the pharmacists can increase adherence to therapy, leading to better control of chronic diseases, ensuring rational drug use, reducing unwanted drug – related problems and improving public health.

A new model of pharmaceutical practice on chronic disease management under the macropolicy was introduced.We described the content and process of the pharmaceutical care with a case,and compared ours with those reported by literatures at home and abroad.The clinical pharmacists provided drug information to the clinicians and personal consultation to the patients,established patient profile to follow up and evaluated the outcomes of the treatment in the medical care team.The clinical pharmacists improved the medication use accuracy and compliance of the patients,and finally optimized the outcomes of the treatment.The pharmacy consulting fee was charged by pharmacist for the pharmaceutical care service.The practice brought both economic and social benefits for the pharmacists.It is meaningful for clinical pharmacist to participate in chronic disease management and provide pharmaceutical care,which is worth further expanding.

Objective: To investigate the points how clinical pharmacists do pharmaceutical care.Methods: The clinical pharmacists participated in the treatment of one patient with unstable angina,assessed the drug regimen in ward-round,and evaluated the medical order.They also suggested pertinence medical care on the use of antiplatelet agents and their interaction with proton pump inhibitors,nitrate resistance,and the crossover allergic reaction of sulfamethizole.Pharmaceutical care carried out during the drug treatment,and played a role in consideration and optimization of the drug regimen.Results: The pharmaceutical care avoided possible adverse events,ensured both the safety and effectiveness of drug uses in the patient,and provided some references for clinical pharmacists' work.Conclusion: The pharmaceutical care by clinical pharmacists can avoid the interactions,resistance and crossover allergic reactions of drugs.
