Objective To explore approaches for clinical pharmacists to provide clinical pharmaceutica1 care in department of cardiology. Methods The clinical cases were summarized by analyzing the clinical pharmacists' experience of clinical pharmaceutical care. Results The work of clinical pharmacists were multifaceted,including clinical case analysis,drug interactions,rational use of antibiotics for special populations,drug therapy selection,report of adverse drug reactions,and patient education in order to promote rational drug use and ensure patient medication safety.Conclusion Clinical pharmacists' involvement in the cardiovascular internal medicine wards can effectively assist doctors to provide high quality of pharmaceutical care for patients.

Objective Clinical pharmacists exert its action through participating in the treatment of 1 uremia patient with antibiotics encephalopathy.Methods According to the whole course pharmaceutical care for the patient,clinical pharmacists monitored relevant indicators and adverse reaction,gave adjustment advice to the doctors timely and evaluated the efficacy of the treatment.Results Through implementation of pharmaceutical care,clinical pharmacists can improve the safety and efficacy of drugs as well as the patient compliance.Conclusion Through participating in clinical practice,clinical pharmacist play an important role in the rational use of drugs,and help patients get excellent pharmacy services.

OBJECTIVE: To investigate the role of clinical pharmacists in clinical treatment through the implementation process of pharmaceutical care for AIDS patients with opportunistic infection. METHODS: Based on the specific case, clinical pharmacists assisted physicians to develop effective treatment programs, established medicine history of patients and put forward reasonable measures of pharmaceutical care and health education programs based on adverse drug reactions of patients during the treatment. RESULTS: Clinical pharmacists provided pharmaceutical care for patient to improve the level of drug treatment and safety and effectiveness of medication, and promoted the quality of life. CONCLUSION: Involvement of clinical pharmacists in therapeutic practice could improve the level of pharmacotherapy, and the use of drugs is more standardized.

Pharmaceutical care for a patient with infection complicating with epilepsy from clinical pharmacist
Objective To provide pharmaceutical care for patients with infection complicating with epilepsy from clinical pharmacists to improve rational use of drugs. Methods By supervising the whole process of treatment for a patient with fever as chief complaint complicating with epilepsy, clinical pharmacists provided pharmaceutical care in terms of antibiotics, hepatoprotective agents, acid-reducing drugs, glucocorticoid and antiepileptic agents, and put forward precautions and suggestion. Results Clinical pharmacists participated in the clinical treatment, provided pharmaceutical care for doctors, nurses and patients directly, and enhanced the therapeutic effi cacy. Conclusion By participating in clinical practice, clinical pharmacists play an important role in the rational use of drugs and provide patients with better pharmacy service.

OBJECTIVE:To explore approaches for clinical pharmacists to provide clinical pharmaceutical care and participate in making the therapy plan in neurology department.METHODS:By means of pharmaceutical care in neurology department,clinical pharmacists focused on patients who were exposed to broad-spectrum antibiotics and patients with liver and kidney dysfunction,carried out medication education among patients with high-risk drugs or low medication compliance,focused on special caution and adverse drug reactions,cooperated closely with doctors and nurses for medical treatment as a strong team.RESULTS:By pharmaceutical care,clinical pharmacists helped to improve rational use of drugs and the efficiency of clinical treatment and reduce the incidence of adverse drug reaction.CONCLUSION:Clinical pharmacists can provide various pharmaceutical care to achieve safe and effective drug use.

OBJECTIVE:To approach the points of clinical pharmacists participating in pharmaceutical care for severe aged patients.METHODS:Clinical pharmacists participated in treatment for a severe aged patient and put forward suggestions on nutritional support,analgesia treatment,drugs by nasal feeding and anti-infection treatment,etc.Clinical pharmacists also developed individual dosage regimen and provided medication guidance for nurse and family members of patients.RESULTS:Through pharmaceutical care,the reasonable drug therapy was provided by the clinical pharmacist and the patient got satisfactory results.CONCLUSION:Clinical pharmacists,physicians and nurses form treatment group,which is benefit to improve clinical treatment and rational drug use.

Objective: To investigate the points how clinical pharmacists do pharmaceutical care.Methods: The clinical pharmacists participated in the treatment of one patient with unstable angina,assessed the drug regimen in ward-round,and evaluated the medical order.They also suggested pertinence medical care on the use of antiplatelet agents and their interaction with proton pump inhibitors,nitrate resistance,and the crossover allergic reaction of sulfamethizole.Pharmaceutical care carried out during the drug treatment,and played a role in consideration and optimization of the drug regimen.Results: The pharmaceutical care avoided possible adverse events,ensured both the safety and effectiveness of drug uses in the patient,and provided some references for clinical pharmacists' work.Conclusion: The pharmaceutical care by clinical pharmacists can avoid the interactions,resistance and crossover allergic reactions of drugs.
