Objective To explore approaches for clinical pharmacists to provide clinical pharmaceutica1 care in department of cardiology. Methods The clinical cases were summarized by analyzing the clinical pharmacists' experience of clinical pharmaceutical care. Results The work of clinical pharmacists were multifaceted,including clinical case analysis,drug interactions,rational use of antibiotics for special populations,drug therapy selection,report of adverse drug reactions,and patient education in order to promote rational drug use and ensure patient medication safety.Conclusion Clinical pharmacists' involvement in the cardiovascular internal medicine wards can effectively assist doctors to provide high quality of pharmaceutical care for patients.

OBJECTIVE: To investigate the role of clinical pharmacists in clinical treatment through the implementation process of pharmaceutical care for AIDS patients with opportunistic infection. METHODS: Based on the specific case, clinical pharmacists assisted physicians to develop effective treatment programs, established medicine history of patients and put forward reasonable measures of pharmaceutical care and health education programs based on adverse drug reactions of patients during the treatment. RESULTS: Clinical pharmacists provided pharmaceutical care for patient to improve the level of drug treatment and safety and effectiveness of medication, and promoted the quality of life. CONCLUSION: Involvement of clinical pharmacists in therapeutic practice could improve the level of pharmacotherapy, and the use of drugs is more standardized.

OBJECTIVE:To explore approaches for clinical pharmacists to provide clinical pharmaceutical care and participate in making the therapy plan in neurology department.METHODS:By means of pharmaceutical care in neurology department,clinical pharmacists focused on patients who were exposed to broad-spectrum antibiotics and patients with liver and kidney dysfunction,carried out medication education among patients with high-risk drugs or low medication compliance,focused on special caution and adverse drug reactions,cooperated closely with doctors and nurses for medical treatment as a strong team.RESULTS:By pharmaceutical care,clinical pharmacists helped to improve rational use of drugs and the efficiency of clinical treatment and reduce the incidence of adverse drug reaction.CONCLUSION:Clinical pharmacists can provide various pharmaceutical care to achieve safe and effective drug use.

OBJECTIVE:To discuss the approach for clinical pharmacists to participate in the drug treatment.METHODS:The contents and importance for clinical pharmacist to participate in ward rounds and discuss with doctors about the optimal dosage regimen were introduced.RESULTS CONCLUSION:A satisfactory outcome was able to be achieved through efforts of both clinical pharmacists and physicians to modify and work out a new individualized dosage regimen.Clinical pharmacists can offer direct,responsible drug-related pharmaceutical care for patients by making fully use of the related pharmaceutical knowledge they have mastered.

OBJECTIVE:To approach the points of clinical pharmacists participating in pharmaceutical care for severe aged patients.METHODS:Clinical pharmacists participated in treatment for a severe aged patient and put forward suggestions on nutritional support,analgesia treatment,drugs by nasal feeding and anti-infection treatment,etc.Clinical pharmacists also developed individual dosage regimen and provided medication guidance for nurse and family members of patients.RESULTS:Through pharmaceutical care,the reasonable drug therapy was provided by the clinical pharmacist and the patient got satisfactory results.CONCLUSION:Clinical pharmacists,physicians and nurses form treatment group,which is benefit to improve clinical treatment and rational drug use.

A new model of pharmaceutical practice on chronic disease management under the macropolicy was introduced.We described the content and process of the pharmaceutical care with a case,and compared ours with those reported by literatures at home and abroad.The clinical pharmacists provided drug information to the clinicians and personal consultation to the patients,established patient profile to follow up and evaluated the outcomes of the treatment in the medical care team.The clinical pharmacists improved the medication use accuracy and compliance of the patients,and finally optimized the outcomes of the treatment.The pharmacy consulting fee was charged by pharmacist for the pharmaceutical care service.The practice brought both economic and social benefits for the pharmacists.It is meaningful for clinical pharmacist to participate in chronic disease management and provide pharmaceutical care,which is worth further expanding.

Objective: To ensure the safety and efficacy in Parkinson's patients taking drugs by analyzing the mode and content of pharmaceutical care to Parkinson's disease.Methods: According to medication history and the changes in patient conditions,clinical pharmacists provided medication education and treatment recommendation,discussed and designed individual drug therapy together with clinicians.Results: Clinical pharmacists optimized the treatment program and solved clinical problems by taking pharmaceutical care.Conclusion: Through actively participating clinical practice and educing pharmacological expertise,clinical pharmacists can improve the effectiveness and safety and compliance of drug treatment.

OBJECTIVE: To explore an approach and method for the pharmaceutical care for COPD patients. METHODS: According to characteristics of clinical pharmacy in China, the approaches and methods of pharmaceutical care for COPD patients were established by referring to approaches for pharmaceutical care issued by Joint Accreditation Commission of Health Care Organizations. RESULTSCONCLUSION:Pharmaceutical care could apparently improve drug use more safely, effectively and economically as well as prognosis for patients.
