    OBJECTIVE:To approach the points of clinical pharmacists participating in pharmaceutical care for severe aged patients.METHODS:Clinical pharmacists participated in treatment for a severe aged patient and put forward suggestions on nutritional support,analgesia treatment,drugs by nasal feeding and anti-infection treatment,etc.Clinical pharmacists also developed individual dosage regimen and provided medication guidance for nurse and family members of patients.RESULTS:Through pharmaceutical care,the reasonable drug therapy was provided by the clinical pharmacist and the patient got satisfactory results.CONCLUSION:Clinical pharmacists,physicians and nurses form treatment group,which is benefit to improve clinical treatment and rational drug use.

    OBJECTIVE: To analyze the emergency hospitalizations trend for ambulatory care sensitive conditions between 2011 and 2015 in a health insureance company of the Colombian Social Security General System. METHODS: A log-linear analysis based on age-adjusted hospitalization rates for ambulatory care sensitive conditions in the Entidad Promotora de Salud Sanitas was used to estimate the annual percentage change in these rates and to identify joinponts of the rates. Data was collected from administrative sources. RESULTS: There were 38,530 hospitalizations for ambulatory care sensitive conditions in 26,501 Entidad Promotora de Salud Sanitas enrollees, with a significant decrease in hospitalization rates. The annual percentage change estimated for the period was -9.5% with no significant joinpoints throughout the time interval. CONCLUSIONS: A significant reduction in hospital admissions due to ambulatory care sensitive conditions in Entidad Promotora de Salud Sanitas enrollees were reported for the last five years in this study.
    The development of clinical pharmacy in 1960-70s and pharmaceutical care in 1990 has represented an interesting innovation in pharmacy field. However clinical pharmacy and Pharmaceutical care showed some practice critical limits: (a) clinical pharmacy approach is more oriented to clinical population needs or pathology but is not strictly request to manage at the level of every single clinical case. (b) In hospital setting Pharmaceutical care can not to be applied to the entire patient for economic reason (limited number of pharmacist applied). In order to give more rationale priority of actions and to select the patient to be seen in priority way we think a good solution to apply the principle of pharmaceutical care with the instrument of clinical pharmacy into a new management systems.

    Objective: To investigate the clinical thinking and pharmaceutical care of pharmacists in drug therapy of cardiology department.Methods: Clinical pharmacist took part in in-depth treatment of patients including complex programs,and summarized the methods and experiences of pharmaceutical care in department of cardiology.Results and conclusion: Clinical pharmacists can provide various kinds of clinical pharmaceutical services for the safe and effectiveness of drug therapies,which can also improve pharmaceutical thinking.

    OBJECTIVE:To discuss the clinical pharmacists how to carry out pharmaceutical care for epilepsy in a patient with acute exacerbations of chronic obstructive pulmonary disease(AECOPD) during the treatment.METHODS:Taking clinical case as example,it was necessary to select the appropriate pharmaceutical care entry point,combined with the COPD guidelines.RESULTS:Clinical pharmacist regarded choice of antimicrobial drug,drug interaction and adverse drug reactions as pharmaceutical care entry point according to clinical situation.CONCLUSION:Clinical pharmacist should be concerned about the patient's actual situation and give full play to one's advantages,then optimize medication therapy by participating in the clinical treatment.

    OBJECTIVE: To construct informational pharmaceutical care system(IPCS) for clinical pharmacists and to improve the level of pharmaceutical care.METHODS: By means of the latest ideas on pharmaceutical care and the support of local area network technology of Hospital Information System,the construction and running of IPCS for clinical pharmacists were illustrated using clinical cases.RESULTS: Based on the establishment of pharmaceutical record database,we could realize the integration of IPCS which included the automatic monitoring of important drugs and bacteria,clinical pharmacist consultation,information collection,information publication,information inquiry,medication counseling,establishment of specialized pharmaceutical staff and online further education,etc.CONCLUSION: The pharmaceutical care based on information platform is a good practice model for clinical pharmacy.It is the only way for clinical pharmacists to carry out further development of pharmaceutical care.

    OBJECTIVE: To summarize the approaches and methods for clinical pharmacists to provide clinical pharmaceutical care.METHODS: By reviewing pertinent literature combining the working experience of the clinical pharmacists,the approaches and methods for clinical pharmacists to provide pharmaceutical care were explored.RESULTS: Pharmacists could provide clinical pharmaceutical care by participating in clinical rounds,consultation and pharmaceutical monitoring,intervening medical order,offering drug-counseling service,handling the adverse clinical pharmacy events and providing medication education etc.CONCLUSIONS: Clinical pharmacists can provide various kinds of clinical pharmaceutical services for the safe,effective and economical use of drugs.

    OBJECTIVE: To discuss the approach for specialized clinical pharmacists to carry out clinical pharmaceutical care.METHODS: The experience of providing pharmaceutical care for one patient with unstable angina pectoris was introduced.RESULTS CONCLUSION: The suggestions put forward by clinical pharmacist on rational drug use achieved satisfactory results.On the one hand,clinical work calls for the help from specialized clinical pharmacists,on the other hand,clinical pharmacists are not independent of clinical work.

    Objective: To explore the patterns of the hospital pharmacists in providing the pharmaceutical care. Methods: The contents and working procedures of the full-time clinical pharmacists in providing pharmaceutical care in the designated department or section with clinical cases in Bethune International Peace Hospital of the PLA were summarized. Results: It is a feasible working pattern under the present condition that the pharmacists go to the designated clinical ward regularly to provide the patients with pharmaceutical care directly and to tram the clinical pharmacists according to the hospital conditions. Conclusion: This working pattern of the clinical pharmacists is advantageous for bringing the pharmacists' professional skills into full play.
