Abstract:
Objective: To explore the patterns of the hospital pharmacists in providing the pharmaceutical care. Methods: The contents and working procedures of the full-time clinical pharmacists in providing pharmaceutical care in the designated department or section with clinical cases in Bethune International Peace Hospital of the PLA were summarized. Results: It is a feasible working pattern under the present condition that the pharmacists go to the designated clinical ward regularly to provide the patients with pharmaceutical care directly and to tram the clinical pharmacists according to the hospital conditions. Conclusion: This working pattern of the clinical pharmacists is advantageous for bringing the pharmacists' professional skills into full play.Keywords:

OBJECTIVE: To summarize the content and experience of clinical pharmacists developing pharmaceutical care in department of gastroenterology. METHODS: The contents of pharmaceutical care were described in details based on case review. RESULTS: Clinical pharmacists could play a professional role according the clinical condition. Pharmaceutical care carried out in gastroenterology department had obtained preliminary success. CONCLUSIONS: Clinical pharmacists should continue to summarize and explore in the clinical work so as to provide strong guarantee for safe and rational drug use.

Objective To provide a meaningful reference for the improvement of clinical pharmacists′ ability through thinking the problems and improving measures in pharmaceutical care.Methods Medical treatments in which clinical pharmacists assisted doctors from Jan.2011 to July 2011 in the respiratory department of our hospital were introduced,the lackness and improving measures of clinical pharmacists in pharmaceutical care were analyzed and summerized.Results Conclusion Pharmaceutical care in the hospital is surely to be developed better as long as clinical pharmacists pay attention to improving themselves′ general competence and obtain corresponding policy support.

OBJECTIVE:To explore the working model of clinical pharmacists in developing pharmaceutical care. METHODS:The medical records of patients undergoing pharmaceutical care were analyzed retrospectively.RESULTS CONCLUSION:Developing pharmaceutical care is an indispensable part of clinical pharmacists' work,at the present stage, the effective working model of developing pharmaceutical care should be the one operated in 3 or 4 steps based on patients' different situation and adverse drug reactions.

OBJECTIVE:To explore the cultivation of clinical pharmacists and development of pharmaceutical care in respiratory department.METHODS:Relative works of pharmaceutical care were summarized through analyzing 2 cases of clinical pharmacy practice in which clinical pharmacist participated.RESULTS:Clinical pharmacists could fulfill their specialty and perform important responsibilities in the clinical work of the safety maintenance of drug use and ADR monitoring,etc.CONCLUSION:Clinical pharmacists should keep raising the level of the basic knowledge of medicine and pharmacy,assist doctor to use drugs rationally and avoid the occurrence of ADR,so that they can provide high-quality and brand-new pharmaceutical care for patients.

Objective: To analyse the role of clinical pharmacists engaged in pharmaceutical care in endocrinology.Methods: The clinical pharmacists participated in selecting the optimal therapeutic regimen in endocrine department and provided pharmaceutical care to medical staff and patients through case analysis. Results: Clinical pharmacists can promote the rational use of drug and reduce the occurrence of adverse drug reaction through participating in the clinical therapy actively with their professional knowledge of pharmacy. Conclusion: The new treatment team of doctor-nurse-pharmacist has been built,and the drug will be used more safely, effectively and rationally through pharmacists participating in the clinical practice.

Objective To explore approaches for clinical pharmacists to provide clinical pharmaceutical care in neurology department. Methods Combined with the experience of clinical pharmacists about drug treatment,administration step,medication education for patients and focus on the patient's choice clinical experience in neurology department,the working contents and working pattern of clinical pharmacists were summarized. Results clinical pharmacists choose the need to monitor the patient,to providing pharmaceutical care in medical consultation, pharmaceutical ward round and group discussions,helped to improve the efficiency of clinical treatment. METHODS:Four typical drug treatment cases in which the clinical pharmacists had participated were reported and analyzed. OBJECTIVE:To summarize the experiences of pharmacists in clinical pharmaceutical care for pharmacists' references.METHODS:By reviewing pertinent literature and data,we summarized pharmacists' daily practices such as carrying out medication education among patients,ADR monitoring in endemic area,involving in formulation of pharmacotherapy,providing medication guidance for patients on discharge,providing pharmaceutical information for nursing staff etc.RESULTS:Through this practice,clinical pharmacists' role was brought to full play practically by offering help for physicians,nursing staff and patients,which served as guidance for pharmacists' work.CONCLUSION:By carrying out the above work,the pharmaceutical care provided by clinical pharmacists gained positive feedback clinically,meanwhile,pharmacists' quality were also improved.This pharmaceutical care is suitable to be carried out in clinical practice.

Objective To summarize the experiences of pharmaceutical care of clinical pharmacists in the department of medical oncology, and provide references for the future pharmaceutical care by pharmacists. Methods By analyzing the cases of pharmaceutical care practice, we summarized some breakthrough points for clinical pharmacists participating in the work of medical oncology department. Results Clinical pharmacists could carry out pharmaceutical care from such aspects as check of the examination index before chemotherapy, chemotherapy scheme selection and dosage adjustment, prescribed chemotherapy drugs examination and enforcement, education on drug use, monitoring and performing treatment on adverse drug reaction, rational use of analgesic drugs and so on. Conclusion Clinical pharmacists should constantly sum up experiences through the learning of theoretical knowledge and clinical practice. By enhancing the communication and collaboration skills and playing the pharmacy specialty, clinical pharmacists could give positive and effective pharmaceutical care for patients and promote the rational use of drugs.
