Recent developments in vitreoretinal surgery have increased the need for suitable vitreous substitutes. A successful substitute should maintain all the physical and biochemical properties of the original vitreous, be easy to manipulate, and be long lasting. Substitutes can be gaseous or liquid, both of which have associated advantages and disadvantages related to their physical properties and use. Furthermore, new surgical techniques with smaller vitreoretinal instruments have driven the use of more viscous substitutes. In this review, we analyze and discuss the most frequently used vitreous substitutes and look ahead to future alternatives. We classify these compounds based on their composition and structure, discuss their clinical use with respect to their associated advantages and disadvantages, and analyze how new vitreoretinal surgical techniques have modified their use.

Vitreoretinal surgery has been radically changed over the past 10 years by the development of new techniques, smaller gauge instrumentation, and improvements in vitrectomy machines. The indications for vitrectomy have expanded dramatically, and inoperable conditions have become amenable to surgical treatment. In addition to improvements in intraocular instruments, various dyes become available and enable better visualization and a more complete removal of vitreous and membranes. In this chapter, we issued latest developments in the surgical field of retina that enable improved surgical outcomes and less complications.

Pars plana vitrectomy is a well developed and widespread used surgical technique with several instruments and special tools for selected vitreoretinal cases. Knowledge of complications during the various steps of the surgical procedure make easier to avoid them. Most important is first to collect experiences with simple cases and to get accustomed to the otherwise special surgical technique that needs good cooperation of the surgeons two hands, two feet and needs steady attention on the tips of the intraocular instruments. However, after certain experience complications will occur because of the complicated cases of proliferative diabetic retinopathy or other vitreoretinal diseases.

Advances in surgical techniques, instrumentation, and vitreous substitutes over the past few decades have greatly improved the anatomical and functional success of vitreoretinal surgery. Intraocular gases are the most common type of tamponade used in vitreoretinal surgery and they are indispensable for that purpose. A thorough understanding of the properties, indications, and potential complications of intraocular gases is therefore essential to optimizing the outcomes. This article reviews the properties, indications, and complications associated with commonly used intraocular gases in vitreoretinal surgery.

In this paper, we propose new evaluation measures for scene segmentation results, which are based on computing the difference between a region extracted from a segmentation map and the corresponding one on an ideal segmentation. The proposed measures take into account separately both under and over detected pixels. It also associates in its computation the compactness of the region under investigation.

Advances in vitreoretinal surgery included the development and characterization of suitable substitutes for the vitreous.Air,expansile gases,silicone oil,heavy silicone oil (HSO),semifluorinated alkanes (SFA) and perfluorocarbons serve(PFCL) integral roles in modem vitreoretinal surgery.Here we review the currently available vitreous substitutes as well those in the experimental phase in their properties,the clinical use,advantages,and shortcomings.
