Abstract:
Antiphospholipid syndrome (APS) is an autoimmune disease characterised by arterial and venous thrombosis. Recurrent thrombosis and pregnancy losses are important in its diagnosis. Due to the risk of recurrence of thrombosis, long-term anticoagulant treatment may be required. Here, we present the case of a patient who suffered from intracardiac thrombosis due to APS. At present, treatment for significant thrombotic events in patients with APS, such as the aforementioned intracardiac thrombosis, is generally lifelong.To design a device for direct vision intracardiac operation without cardiopulmonary bypass, and assess its applicability preliminarily.The device was designed according to the clinical needs of intracardiac operation and used in operations for repairing atrial septal defect in 5 ex vivo porcine heart models. The practical applicability of this device was thoroughly tested and the results of the operations were evaluated.Direct vision operation for repairing atrial septal defect was successfully performed using this device, which can be a well applicable in some intracardiac operations, but its clinical effects need further evaluation.

The paper reviews air travel and venous thrombosis. The risk of venous thrombosis increases with distance flown. The mechanisms that cause thrombosis are unknown, but may be related to hypobaric hypoxia, i.e. reduced air pressure inside the aeroplane, stasis of the lower limbs, often referred to as economy class syndrome (ECS), and dehydration.Symptomless deep venous thrombosis may occur in 10% of healthy long-haul airline travellers over 50 years of age, and up to 4.5% of passengers under 50 at high risk of thrombosis. Wearing of elastic compression stockings during the flight is associated with reduced risk of deep venous thrombosis.Passengers should be encouraged to use the muscle-vein pump regularly, ensure adequate hydration, and in some cases wear elastic compression stockings. A majority of passengers will not need any thromboprophylaxis. Aspirin may have a protective role, but its efficacy in preventing venous thrombosis is much smaller than that of low molecular weight heparins. A high prophylactic dose of a low molecular weight heparin should be considered in passengers at high risk of thrombosis.

OBJECTIVE To explore the effects of beating heart and arrested heart intracardiac procedure on the expression of tumor necrosis factor alpha (TNF-α) mRNA in myocardium.METHODS 30 congenital VSD patients aged from 5 to 10 years were randomly divided into two groups equally. Group 1 was undergone in traditional arrested heart intracardiac procedures; Group 2 was undergone in beating heart procedures. Specimens of myocardium were obtained while the right atrium was opened (baseline) and closed for the determination of TNF-αmRNA. Concentration of TNF-αwas respectively measured after anesthetic induction (T1), 20 minutes after the beginning of CPB (T2), at the end of CPB (T3) and 6, 12, 24 hour after CPB (T4-6) in all patients. RESULTS Compared with baseline, the expression of TNF-αmRNA was significantly increased in both groups (P0.05). TNF-αmRNA level of group1 was significantly higher than that of group 2 while the right atrium was being closed (P0.05). The plasma concentration of TNF-α in group1 was significantly higher than that of group 2 at T4-6(P0.05). CONCLUSION Compared with traditional arrested CPB, beating heart intracardiac procedure can effectively reduce the expression and release of TNF-α; it will benefit the protection of pediatric myocardium during CPB.
