Reperfusion of the human myocardium by saphenous vein bypass grafts. Biochemical considerations.

Abstract:
The adenine nucleotide content of the human myocardium in the distribution of the left anterior descending coronary artery (LAD) was measured before and after saphenous vein bypass grafting. The purpose of the study were twofold: (1) to relate the level of adenosine triphosphate (ATP) before bypass grafting to the percent stenoses of the LAD and (2) to determine the benefit or lack of benefit of bypass grafting on ATP content. Eighteen patients with angiographically determined LAD lesions of 40% to 100% underwent bypass grafting with standard cardiopulmonary bypass and cardioplegia. Transmural needle biopsy specimens were obtained from the center of the area perfused by the LAD immediately before cross-clamping of the aorta and 30 minutes after reperfusion of the myocardium via the native LAD and the graft. The tissue was divided into thirds: The endocardial and epicardial thirds were analyzed for ATP by high-pressure liquid chromatography and the middle third was viewed by light microscopy. The percent narrowing of the LAD correlated well (r = -0.71) with the ratio of ATP to total adenine nucleotides (TAN) in the endocardium. Epicardial ATP did not correlate with the percent stenoses of the LAD. The endocardial ATP/TAN ratio increased in the group as a whole from 0.51 +/- 0.27 (mean +/- SD) to 0.64 +/- 0.26 (p less than 0.01) after bypass grafting, and this was most impressive in those eight patients with LAD lesions greater than 90% (0.32 +/- 0.20 before grafting to 0.60 +/- 0.29 after grafting, p less than 0.005). However, the epicardial ATP/TAN ratio decreased from 0.75 +/- 0.15 before grafting to 0.64 +/- 0.17 after grafting (p less than 0.05), and this decrease occurred regardless of the percent narrowing of the LAD. There was no difference in vacuolization between the pre-grafting and post-grafting biopsy specimens, and intramyocardial hemorrhage was not observed. This study has demonstrated a close relationship between the degree of LAD stenosis and endocardial ATP content. Also, the endocardium supplied by arteries with greater than 90% lesions had significantly increased ATP while the epicardium had decreased ATP content after bypass grafting.Keywords:

O uso de enxerto composto de artéria torácica interna esquerda (ATIE) com segmentos arteriais ou segmentos de veia safena magna (VSM) pode permitir a revascularização completa do sistema coronariano esquerdo (SCE) sem circulação extracorpórea (CEC) e sem manuseio da aorta ascendente (MAA), como forma de tentar reduzir alguns riscos e complicações no pós-operatório imediato. Neste trabalho, relatamos os resultados angiográficos, após oito anos, de dois pacientes submetidos à cirurgia de revascularização do SCE com enxerto composto de ATIE e VSM, sem CEC e sem MAA.

The median sternotomy is the preferred incision for coronary artery bypass grafting (CABG) but its usage was initially associated with wound infection rates of up to 5%. The advent of antibiotic prophylaxis and improved surgical technique has improved these rates to 1% to 2% in most series. During a 5-year period from January 1988 to December 1992, 2193 patients underwent CABG in the Singapore General Hospital. Of these, 22 (1%) suffered postoperative wound infection. Thirteen (0.59%) of these had deep-seated mediastinitis which involved the osteocartilagenous tissues and retrosternal space and 9 (0.41%) had superficial wound infections. The superficial infections were successfully treated with dressings and delayed closure in all cases. The deep-seated infections, however, required in addition, antibiotic therapy, povidone iodine irrigation, repeated debridement and flap closure. In total, 6 muscle and myocutaneous flaps were used with good result. This paper reviews the Plastic Surgery unit's experience in treating this difficult postoperative complication.

The purpose of this article is to describe our technique for performing coronary artery bypass grafting without cardiopulmonary bypass and to demonstrate that this operation is safe and useful to the case of LV dysfunction. Between April 1988 and March 1994, 2 men and 1 woman aged 61 to 69 years (mean 65 years) with LV dysfunction (EF < or = 30%) underwent coronary artery bypass grafting without cardiopulmonary bypass. Saphenous vein grafts were placed to the right coronary artery (n = 2), left anterior descending artery (n = 2), and internal thoracic artery grafts were placed to the right coronary artery (n = 1), left anterior descending artery (n = 1). The post operative course were uneventful in all patients and discharged with no complications. All patients underwent coronary angiography, and 5 of 6 (83.3%) grafts placed without cardiopulmonary bypass were patent. In selected patients, coronary artery bypass grafting to the case of LV dysfunction without cardiopulmonary bypass can be performed safe and satisfactory graft patency rates.

Ascending aortic aneurysms are usually treated with graft replacement under cardiopulmonary bypass. However, if a candidate for off-pump coronary artery bypass grafting has an enlarged ascending aorta, surgeons may consider wrapping it without cardiopulmonary bypass. Here, we report a 78-year-old female who underwent successful wrapping of the ascending aorta concomitant with off-pump coronary artery bypass grafting, using a new wrapping technique that involves multiple bootstraps.
