Abstract:
Surgical experience of a rare case of malignant retroperitoneal cyst is reported. A 41-year-old female was admitted on Feb. 26, 1986, complaining of left lower abdominal tumor and mild abdominal pain. She underwent complete removal of an abdominal tumor located at the left flank lateral to the sigmoid colon on March 5. The tumor was well encapsulated, cystic and oval, 12 X 10 X 9 cm in size. Histologic feature of the tumor is classified as mucinous cystadenoma of low grade malignancy by WHO classification.Keywords:

Objective:To evaluate the diagnostic value of CT and MRI in intrahepatic biliary cystadenomas by summarizing CT and MRI characteristics.Methods:Clinical data and CT,MRI materials from 5 cases of intrahepatic biliary cystadenomas,which proved by surgery and pathology,were retrospectively analyzed.Results:There were 2 males and 3 females.There were 4 cases in the right lobe and 1 case in the left lobe.5 cases were all single with 2 cases of unilocular and 3 cases of multilocular cyst.There were 3 cases of cystic nodules or wall papillary projections,1 case of dot calcifications in the shells and 2 cases of cystic dilatation of common bile duct.The wall,septations and mural nodules were enhanced in CT and MR enhancement scans.Conclusion:The CT and MRI appearance of intrahepatic biliary are characteristics.Cystic lesions associated with separation,mural nodules or papillary projections may prompt diagnosis of cystadenoma.Significant expansion of common bile duct is an important indirect sign to diagnose mucinous cystadenoma connected with the bile duct.

A case of primary retroperitoneal mucinous cystic tumor of borderline malignancy is reported. A 36-year-old, nulliparous woman complained of abdominal fullness. Physical examination revealed a cystic mass 12-cm x 8-cm in size. At laparotomy, a cystic tumor was observed in the right retroperitoneal space. Both ovaries appeared normal and the uterus was almost normal except for small myomatous nodules. Removal of the retroperitoneal tumor, an appendectomy, and a myomectomy were performed. The histologic diagnosis was a primary retroperitoneal mucinous cystic tumor of borderline malignancy, similar to findings for ovarian tumor. There was no evidence of disease 6 months after the surgery.

Objective. Differentiation of benign and malignant cystic adnexal masses by analysis of cyst fluid was investigated. Study design. Eighty‐one cystic adnexal lesions of 80 patients were punctured intraoperatively or after surgical removal. Color of the cyst fluid, cytology, concentrations of steroid hormones (estradiol. progesterone, testosterone), and tumor markers (CEA, CA 125, CA 15‐3) were correlated to histology. Results. Color or cyst fluid was not reliable in the differential diagnosis of cystic adnexal masses. Cytology missed four out of seven malignant tumors. One mucinous cystadenoma was cytologically suspicious for malignancy. The combination of negative cytology, low tumor marker levels. and estradiol levels higher than 1000 pg/ml may support the diagnosis of a ‘functional’ cyst. Elevated CEA levels may be suspicious for all mucinous and malignant lesions. Conclusions: Analysis of cyst fluid could not accurately discriminate between benign or malignant tumors.

A case of a primary retroperitoneal mucinous cystadenoma of borderline malignancy is presented. A 37-year-old female was found to have a retroperitoneal tumor associated with calcification by computed tomography. On laparotomy a cystic tumor in the retroperitoneum near the the left flank was found while the two normal ovaries were confirmed. The tumor was well encapsulated and 6.5×4.5×3.8cm in size. Its histological feature was mucinous cystadenoma of borderline malignancy by WHO classification. Retroperitoneal mucinous cystadenoma of borderline malignancy is so rare that only 12 cases including our case have been reported in the world. Clinical features and the prognosis are analized and appropriate surgical management of this disease is discussed.

Diagnosis of cystic tumor of the pancreas is based on the World Health Organization criteria that classify pancreatic cystadenomas into four types: intra-ductal papillary mucinous neoplasms, mucinous cystic neoplasms (MCNs), serous cystic neoplasms, and solid pseudo-papillary neoplasms depending on their secretion and presence of ovarian-like stroma. Recently, Albores-Saavedra identified non-mucinous cystadenomas of the pancreas with pancreato-biliary phenotype and ovarian-like stroma. This precipitated examination of the proportions of these rare tumors in patients treated at Tokushima University Hospital. Case 1 was a 40-year-old woman with a cystic tumor in the tail of the pancreas. Computed tomography (CT) revealed a diffuse and non-enhanced cystic tumor in the tail of the pancreas. This tumor was diagnosed as a simple cyst at this point. However, 2 years later, the tumor had increased in size by 3 cm. Thus, laparoscopic distal pancreatectomy was performed. The content of the cyst was serous. The epithelial cells were lined with a single layer of cuboidal cells and the tumor had ovarian-like stroma pathologically. The final pathological diagnosis was non-mucinous cystadenoma of the pancreas with ovarian-like stroma. In Case 2, a cystic tumor in the pancreas was found by medical examination in a woman in her sixties who presented without symptoms. CT showed a 1.5-cm cystic tumor in the tail and body of the pancreas and a septum in the cyst. Nine years later, the tumor had grown to 2.4 cm in diameter and had a clear septum in the cyst. This tumor was diagnosed preoperatively as MCN. Thus, laparoscopic distal pancreatectomy was performed. The cyst contained serous fluid. Microscopic examination showed no ovarian-like stroma and the epithelial cells were lined by a single layer of cuboidal cells. The final pathological diagnosis was non-mucinous cystadenoma of the pancreas with ovarian-like stroma. Accurate preoperative diagnosis of this type of pancreatic cystic tumor may be difficult, although it occurs more often than expected. Non-mucinous cystadenomas of the pancreas with ovarian-like stroma need to be considered as a differential diagnosis.

Primary retroperitoneal mucinous cystadenoma of borderline malignancy represents a rare tumor, with unclear histogenesis, concerning almost exclusively women. Only two cases concerning male patients have been reported. We herein report a case of a 37 year old man undergone laparotomy for a sizable retroperitoneal tumor resulting after the histological examination to a primary retroperitoneal mucinous cystadenoma of borderline malignancy. This is the third case of primary retroperitoneal mucinous cystadenoma of borderline malignancy in a male patient reported in the literature. The preoperative diagnosis is impossible. Laparotomy constitutes the only diagnostic and curative approach.
