Abstract:
Objective To observe the efficacy and safety of milrinone for refractory heart failure. Methods Forty-eight patients with refratory heart failure received milrinone 10mg in 5% glucose 250 ml 6h/d , for 7 days. Results The total clinical effective rate was 91.7%, milrinone significantly improved left ventricular systolic and diastolic function. Conclusion Milinone is an effective and safe drug, and can improve the left ventricular function in patients with refractory heart failure.Keywords:

Objective: To explore the clinical efficacy of Amiodarone in the treatment of heart failure with ventricular arrhythmia.Methods: Conventional treatment of heart failure in 32 patients with Amiodarone,patients were observed on the changes of cardiac function,ventricular arrhythmias and untoward effect before and after treatment.Results: There were 27 patients with a positive response to the treatment,the total effective rate was 84.38%,left ventricular ejection fraction(LVEF) were compared before and after treatment in order to be statistically significant(P0.05),and there was no adverse reactions after treatment.Conclusion: Heart failure with ventricular arrhythmia treatment of heart failure while in the active Amiodarone treatment,which can effectively control the patient's condition,safe and effective.

To assess the hemodynamic effects of milrinone in a 48h intravenous infusion in patients with severe congestive heart failure (CHF) (NYHA class III or IV).Forty patients with CHF were sequentially evaluated. Right heart catheterization was performed in order to measure, before and after administration of milrinone, several hemodynamic parameters (cardiac index, pulmonary wedge pressure, systemic and pulmonary vascular resistance).There was a significant improvement in hemodynamic parameters (cardiac index, cardiac output), and a decrease in systemic vascular resistance and pulmonary vascular resistance. Serious side effects were not observed in these patients.These findings indicate that milrinone is effective in the treatment of deteriorating phases of CHF and suggest that milrinone should be used in these select patients.

Objective: To study clinical application of intravenous Milrinone in patients with congestive heart failure with special reference to hemodynamic effects and hormonal changes of domestic Milrinone. Method: In this self contrast trial, adopting balloon tipped flotation catheter both determined before and after intravenous Milrinone to 20 patients with congestive heart failure. Right atrial pressure(RAP), pulmonary arterial pressure(PAP), pulmonary capillary wedge pressure(PEWP) and cardiac output(CO) were measured. AngiotensionⅡ(AngⅡ),artrial natriuretic peptide(ANP) and endothelim(ET) were determined with radioimmunoassay. Result: The results showed that intravenous Milrinone decreased PAP, PCWP rapidly, increased CO, CI, but no affect on heart rate and blood pressure. Milrinone had both inotropic and vasodilative effect on patients with chronic congestive heart failure. Milrinone decreased AngⅡ, ANP, ET, it indicates that the mechanism of action of Milrinone to a certain extent may be associated with the neurohumoral factors. Conclusion: Because Milrinone has unique function of pharmacology, short term intravenous Milrinone is an effective means to improve hemodynamics rapidly when routine treatment has no effect.

Objective:To explore the clinical effect and the use of security of amiodarone on congestive heart failure and ventricular arrhythmia.Methods:76patients with congestive heart failure with ventricular arrhythmias hospitalized in our hospital during January 2010to December 2012were divided into treatment group(plus amiodarone therapy on the basis of the conventional anti-heart failure therapy)and control group(conventional anti-heart failure treatment).Antiarrhythmic and improving cardiac function in patients were observed after treatment in two groups,comparing heart rate,blood pressure,left ventricular ejection fraction(LVEF),the QT interval and adverse drug reactions.Results: Effectiveness of anti ventricular arrhythmias and improving cardiac function and LVEF in treatment group was significantly higher than in control group,the difference was statistically significant(P0.05).Heart rate,QT interval of the treatment group improved significantly compared with the control group,the difference was statistically significant too(P0.05).Adverse reactions of the two groups showed no significant difference(P0.05).Conclusion:Amiodarone in the treatment of congestive heart failure with ventricular arrhythmias is effective and safe.

Objective: To investigate the clinical study of Acertil(Perindopril) in treating congestive heart failure.Methods: 118 patients,who had unsatisfactory results under conventional Digitoras,Diuretics,Vasodilators therapy,were given Acertil 2 ~4 mg orally once a day for 4 ~6 weeks.The changes of heart rate(HR),blood pressure(BP),cardiothoracic ratio,left ventricular and diastolic diameter(LVDd),left ventricular ejection fraction(LVEF) and cardiac function(NYHA) were observed.Results: After 4 ~6 weeks by the therapy,HR BP cardiothoracic ratio and LVDd all decreased significantly(P0.05);LVEF increased(P0.05);Cardiac function(NYHA) improved by 1 ~2 grade.Acertil had few adverse effects and patients tolerance was perfect.Conclusion: The effect of Acertil in treating congestive heart failure is satisfactory.Acertil has little adverse effect and is effective for congesitive heart failure.

Objective:To evaluate the effect of milrinone in treating refractory heart failure. Methods:5mg of milrinone in 250ml of 5% Glucose or 0.9% NaCl Solution were given daily by intravenous drip to 56 cases of refractory heart failure for a period of consecutive 7 days. The symptoms and signs were observed before and after the treatment, heart rate, blood pressure and heart function examined by Doppler echocardiography. Results:a. The total effective rate was 85.7%. b. The left ventricular ejection fraction (LVEF) and fraction shortening (FS) were increased significantly ( P 0.01) after treatment. The rate of filling velocity in early diastole (E) over that in late diastole (A) was increased significantly ( P 0.05). Conclusion:Milrinone has good effect for treating refractory heart failure. It improves not only the cardiac systolic function but also the diastolic function.

Objective:Carvedilol in chronic heart failure.Methods:Patients with chronic heart failure patients,in accordance with New York Heart Association(NYHA) heart function classification standards group,randomly divided into treatment group(40 cases) and carvedilol treatment group(40 cases),the former application of vasodilators,diuretics,ACEI,and cardiac drugs to correct heart failure,correct the failure of conventional treatment of the latter based on the combination of carvedilol,groups 6 months after treatment by echocardiography left ventricular ejection fraction(LVEF),left ventricular end systolic diameter(LVESD),left ventricular end diastolic diameter(LVEDD),and 6min walking test measured(6-MWT).Results:In the conventional treatment of heart failure based on the correct application of carvedilol,LVEF increased significantly,LVESD,LVEDD significantly decreased,6-MWT significantly improved heart function improved significantly better than the conventional treatment group(P0.05).Conclusion:During routine clinical treatment of chronic heart failure carvedilol in combination,Hemodynamic abnormality can be corrected,Improve exercise tolerance,Improve heart function.

Objective To investigate the therapy for aged patients with severe acute left heart failure and to evaluate the effect value of brain natriuretic peptide (BNP).Methods Aged heart failure in NYHA class Ⅳ(n=72) needed mechanical ventilation by small tidal volume small positive end-expiratory pressure(PEEP) and received Milrinone infusion 10mg in 5% glucose 50 ml continued for 6h,qd,for 14 days.BNP levels were obtained.Results Mechanical ventilation therapy used (8.2±3.7)d.The total clinical response rate was 62.5%(45/72),the mortality rate was 37.5%(27/72).Those patients with diuretic response and BNP was significantly reduced after treatment mostly with good prognosis,otherwise with poor prognosis mostly combined with acute respiratory failure type Ⅱand cardiogenic shock.Conclusion Protecting mechanical ventilation and Milrinone therapy could improve the prognosis for aged heart failure,BNP was significantly reduced if treatment effectively.
