Abstract:
Objective:Carvedilol in chronic heart failure. Methods:Patients with chronic heart failure patients,in accordance with New York Heart Association(NYHA) heart function classification standards group, randomly divided into treatment group(40 cases) and carvedilol treatment group(40 cases),the former application of vasodilators, diuretics,ACEI,and cardiac drugs to correct heart failure, correct the failure of conventional treatment of the latter based on the combination of carvedilol, groups 6 months after treatment by echocardiography left ventricular ejection fraction(LVEF),left ventricular end systolic diameter(LVESD),left ventricular end diastolic diameter(LVEDD),and 6min walking test measured(6-MWT).Results:In the conventional treatment of heart failure based on the correct application of carvedilol,LVEF increased significantly,LVESD,LVEDD significantly decreased,6-MWT significantly improved heart function improved significantly better than the conventional treatment group(P0.05).Conclusion:During routine clinical treatment of chronic heart failure carvedilol in combination,Hemodynamic abnormality can be corrected,Improve exercise tolerance,Improve heart function. Keywords:

[Objective] To observe the effect of Carvedilol on ventricular remodeling and cardiac function of chronicity congested heart failure (CHF). [Methods] 115 patients with CHF were divided into group A (61patients) and group B (64 patients). Both groups were treated with normal therapy. Carvedilol was given to Group B. NYHA classification, left ventricular end diastolic (LVEDd),left ventricular end systolic diameter (LVESd),left ventricular eject fraction (LVEF) were recorded before and after treatment. [Results] The LVEDd,LVESd and LVEF of group B showed significant difference when comparing with those before treatment (P﹤0.05). Compared with group A,group B had great significant difference (P﹤0.05). [Conclusion] Normal therapy adds Carvedilol can obviously improve the ventricular remodeling and cardiac function in the CHF.

Objective:To evaluate the clinical efficacy of carvedilol in treatment of dilated cardiomyopathy heart failure.Methods:112 cases of dilated cardiomyopathy patients with heart failure were randomly divided into two groups, control group with conventional treatment, the treatment group with conventional therapy plus carvedilol intervention.Comparison of two treatment methods and the total effective rate after treatment by echocardiography changes in the indicators. Results:The total effective rate of the control group was significantly lower than the treatment group(P0.05) ;the treatment group in the improvement of echocardiographic indices are better than the control group, the difference was statistically significant(P0.05) .Conclusion:The conventional anti-failure treatment based on the use of carvedilol can significantly improve the dilated cardiomyopathy patients's heart function, inhibiting left ventricular remodeling, improveing the quality of life. The carvedilol is worthy to be popularized.

Objective To evaluate the efficacy and safety of carvedilol in patients with chronic heart failure. Methods 72 cases with heart failure induced by hypertensive heart disease were randomly divided to a treatment group with carvidilol and a control group with basic treatment. Before and after therapy the indexes such as heart rate, blood pressure, left ventricular function and pathogenetic condition change were compared. Results The ejection fraction increased in the two groups, the left ventricular end diastolic volume and the ventricular end-systolic volume decreased at the end of the experiment, but the change of ejection fraction, the left ventricular end diastolic volume and the ventricular end-systolic volume in treatment group were better than those in control group. Conclusion It is safe and effective to treat heart failure induced by hypertensive heart disease with carvedilol on the basis of controlling blood pressure and hear failure.

Objective To investigate the effects of Carvedilol on heart function and neorohormones in patients with congestive heart failure(CHF) by double-blind randomized clinical trial.Method40 congestive heart failure patients were double-blind randomly divided into two groups,20 of them randomized to Carvedidilol with routine therapy group and 20 patients to Placebo with routine therapy group. The change of heart rate, blood pressure,NYHA classification,6 min.walk distance, left ventricular ejection fraction(LVEF) and left ventricular diastolic dimension(LVDd) by 2-D Echocardiography were observed before and 1、3 months after treatment, meanwhile the plasma concentration of norepinephrine, renin and angiotensin II in patients who were supine position were measured in the morning before and 6 weeks after treatment.ResultsThe study showed the patients' heart rate, blood pressure,NYHA classification,6 min.walk distance,LVEDd,LVEF significantly improved. The plasma concetration of norepinephrine, renin, angiotensin II in patients with CHF were higher than those of healthy control group(P0.01),and signicantly reduced(P0.01) after treatment with Carvedilol compared with routine treatment. No patient died and had heart failure worsened in carvedilol group, but 6 patients were rehospitalized because of worsening heart failure in Placebo group.ConclusionIt was indicated that Carvedilol could signicantly improve heart function and reduce the plasma concentration of neurohormones in patients with congestive heart failure.

Objective To explore the effects and safety of Carvedilol on cardiac contractility in ischemic heart disease with the chronic congestive heart failure(CHF).Methods Sixty-four patients with CHF were randomly divided into 2 groups:the Betaloc group(A group,32cases)and the Carvedilol group(B group,32 cases). The A group was treated by Betaloc, while B group by Carvedilol base on the basic treatment.Follow-up for 3 mongths and detect the index. Results Both the two groups could reduce heart rate, blood pressure, left ventricle's LVEDD and LVESD,but the B group had the lower LVESD and higher Dd value than the A group(P0.05).CO and CI value of group A was reduced, there was significant difference between two groups(P0.05).Group B had the higher SV(P0.05)and LVEF(P0.01)than that in group A.Conclusion Carvedilol is more effective than Betaloc. It is a effective and safty treatment to chronic congestive heart failure.

Objective: To evaluate the clinical effects and the mechanisms of carvedilol in patients with congestive heart failure (CHF, New York Heart Association functional class Ⅲ-Ⅳ ). Methods: Fifty-five CHF patients were randomly divided into two groups. One is conventional treatment group (n=26) treated with digitalis, diuretics, and vasodilatation including ACEI; and another is carvedilol treatment group (n=30) treated with carvedilol and conventional therapy. Echocardiogram, blood routine examination and blood biochemical examination were done bofore and after treatment. Result: After treated for three months, the cardiac function improved markedly in both groups, but that in carvedilol treatment group improved more significantly than that in conventional treatment group. Conclusion: Carvedilol has a cardiac protective effect in patients with congestive heart failure, in improving cardiac function and decreasing ventricular remodeling progress.

Objective:To investigate the effects of carvedilol on the left ventricular function and plasma level of B-type natriuretic peptides(BNP)in patients with chronic systolic heart failure(CHF).Methods:Fifty-four patients with CHF were equally randomized into conventional therapy and carvedilol group. The former received medication of digoxin, diuretics and ACEI or ARB,and the latter were given carvedilol in addition to conventional therapy for 16 weeks.Determination of the heart function was performed before and after the treatment concerning plasma BNP,left ventricular ejection fraction(LVEF) and left ventricular internal dimension at end-diastole(LVEDd).Results:Improved heart function and decreased BNP level were seen in both groups, though, significance was found in carvedilol group(P0.05),and yet,no significant difference was seen in the subjects with carvedilol intervention regarding LVEF and LVEDd,despite of increase or fall to a certain degree as compared with the conventional therapy group. Conclusion:Additional carvedilol on conventional medication basis can be more effective in treatment of patients with CHF by bettering the left ventricular function but decreasing plasma BNP.
