    Abstract:
    To evaluate the short-term and long-term effects on treatment of neck pain caused by cervical spondylosis with the combination of acupuncture and moxibustion with seed-size moxa cone.One hundred and forty-five patients of neck pain were randomly divided into an acupuncture-moxibustion group (49 cases), an acupuncture group (48 cases) and a moxibustion group (48 cases). Acupoints of Bailao (Extra), Dazhui (GV 14), Jianzhongshu (SI15) and Zhongzhu (TE 3) were adopted for all the 3 groups. Acupuncture was applied at all the acupoints with 20 min needling retention for the acupuncture group. Moxibustion with seed-size moxa cone was used with 5 cones on each point for the moxibustion group. And both acupuncture and moxibustion with seed-size moxa cone were adopted for the acupuncture-moxibustion group. The treatment was applied once every 3 days, and 10 treatments should be finished within 4 weeks. Follow-up should be carried out for 3 months. The short-term and long-term effects were evaluated with the scores of Northwick Park Pain Questionnaire (NPQ) and McGill Pain Questionnaire (MPQ) as the indices of therapeutic effect.The NPQ score and MPQ score of all the 3 groups after the treating course and the 3-month follow-up were both decreased when compared with those before the treatment (all P<0. 05). The scores of NPQ and MPQ the acupuncture-moxibustion group were lower than that of the other two groups. And the difference had obvious significance (P<0. 05). High efficiency of pain relieving for cervical spondylosis could be found in all the 3 groups, which showed that short-term and long-term effects were good for all the 3 groups. And the highest curative effect could be found in acupuncture-moxibustion group.Combination of acupuncture and moxibustion with seed-size moxa cone has reached a superior effect in short-term and long-term for neck pain caused by cervical spondylosis.

    To observe the therapeutic effect of acupuncture of Ashi points in combination with moxibustion of heat-sensitive points for neck-back myofascial pain syndrome, so as to find a better combined therapy.A total of 62 eligible patients were randomly divided into treatment group (acupuncture of Ashi-points plus moxibustion of heat-sensitive points, n=32) and control group (acupuncture of Ashi-points plus TDP irradiation, n=30) by using single-blind method. Ashi-points were the tenderpoints or subcutaneous induration spots determined by digital pressure in the focus region, and the heat-sensitive points were the acupoints around the subcutaneous induration spots in the neck-back regions determined by patients' feeling (heat from the ignited moxa transmitting toward the deep muscle layer, extending toward the surrounding region of the Ashi-points, etc. and the distal part of the body) during moxibustion. Ashi-points were punctured with filiform needles and stimulated with reducing method by lifting, thrusting and twirling the acupuncture needle repeatedly till "Deqi", followed by retaining the needle for 30 min. Moxibustion was given to the patients for 10-90 min (when the patient began to feel heat penetrating into the deeper muscle layer to the termination of the heat transmission). TDP irradiation was given to the Ashi-points for 30 min in every session of treatment. The treatment was conducted once daily, 5 times a week, two weeks altogether. McGill pain questionnaire containing pain rating index (PRI), visual analogue scale (VAS) and present pain intensity (PPI) and "the criteria for assessing the therapeutic effect of back-myofascitis" recorded in "Standards for Diagnosis and Efficacy Evaluation of Clinical Conditions in Chinese Medicine" (published in 1994 in China) were used to evaluate the analgesic effect.Before the treatment, no significant differences were found between the treatment and control groups in PRI, VAS and PPI. After the treatment, PRI, VAS and PPI were all significantly lower in the treatment group than in the control group (P < 0.01). Of the 32 and 30 myofascitis patients in the treatment and control groups, 11 (34.4%) and 3 (10.0%) were cured, 20 (62.5%) and 25 (83.3%) experienced improvement in their symptoms, 1 (3.1%) and 2 (6.7%) had no apparent changes. The cure rate of the treatment group was significant bigger than that of the control group (P < 0.05).Acupuncture combined with moxibustion of Ashi-points can effectively relieve pain reaction in neck-back myofascial pain syndrome patients, which is significantly superior to that of acupuncture plus TDP irradiation therapy.

    Objective To explore the applicability of patient-reported outcomes(PRO) for the evaluation of therapeutic effect of acupuncture and moxibustion for neck pain induced by cervical spondylosis.Methods A multiple randomized control clinical trial was carried out in 896 qualified patients.With Nape Pain Questionnaire(NPN) items as the main indexes of therapeutic effect,PRO mode was used to evaluate the therapeutic effect of acupuncture and moxibustion for the patients with neck pain induced by cervical spondylosis.With McGill Pain Questionnaire(MPQ) and SF-36 life quality questionnaire as reference,we evaluated the reliability,validity and responsiveness of NPQ scores of 896 patients before treatment,after treatment and during follow-up.Results The results of reliability analysis showed that the Cronbach's α coefficient of NPQ scores was 0.769(95% CI:0.761-0.804).The results of content validity analysis showed that NPQ scores were moderately correlated with each dimensions of MPQ,and were moderately or strongly correlated with the somatic pain in SF-36.NPQ scores were mildly correlated with 6 dimensions of SF-36,and were weakly correlated with mental health.The results of structure validity analysis showed that 3 factors selected from NPQ had an accumulative variance contribution rate of 59.3%.The results of the analysis of responsiveness showed that NPQ scores at the end of treatment and 3 months after the follow-up differed from those before treatment(P0.001).Conclusion PRO shows good reliability,validity and responsiveness for the evaluation of therapeutic effect of acupuncture and moxibustion for neck pain induced by cervical spondylosis,which is expected to be extensively used for the clinical research of acupuncture and moxibustion.

    Objective To observe the therapeutic efficacy of acupuncture plus heat-sensitive moxibustion in treating cervical spondylosis of nerve-root type.Method A total of 120 patients with cervical spondylosis of nerve-root type were randomized into a treatment group and a control group,60 in each.Acupuncture and heat-sensitive moxibustion were adopted in the treatment group,while acupuncture alone was adopted in the control group.Therapeutic efficacy was observed after 3 treatment courses.Result The total effective rate and recovery rate in the treatment group were markedly higher than those in the control group(P0.01).Conclusion Acupuncture plus heat-sensitive moxibustion has better therapeutic efficacy than single acupuncture treatment in managing cervical spondylosis of nerve-root type.

    Objective To investigate the clinical efficacy of Shi's warm needling moxibustion plus acupoint application in treating cervical spondylotic radiculopathy. Method Sixty patients satisfying the criteria were allocated to treatment and control groups, 30 cases each. The treatment group received Shi's warm needling moxibustion plus acupoint application and the control group, conventional acupuncture plus acupoint application. The changes of VAS, PRI, and PPI after treatment were observed in the two groups. Result After treatment, various observation indices were higher in the treatment group than in the control group(P 0.01). The cure and marked efficacy rate was higher in the treatment group than in the control group(P0.01). Conclusion Warm needling moxibustion plus acupoint application has a marked therapeutic effect on cervical spondylotic radiculopathy.

    Objective To observe the effect of deferent needling depths on the treatment efficacy during acupuncture at cervical Jiaji points(Ex-B 2) in treating cervical spondylosis.Method Sixty patients with cervical spondylosis were randomized into a treatment group and a control group,30 in each group.The two groups both received acupuncture at cervical Jiaji points(Ex-B 2), 0.3~0.6 of depth in the treatment group versus 1.0~1.3 cun in the control group.The two groups were compared with each other in treatment efficacy.Result After treatment,there were no significant differences between the two groups in comparing cervical function and total effective rate(P0.05).Conclusion Given that there is needling sensation,changes of acupuncture depth at cervical Jiaji points(Ex-B 2) do not make any obvious differences to the therapeutic efficacy in treating cervical spondylosis.

    To observe the differences in the clinical effects on cervical spondylosis with neck pain between the grain-moxibustion combined with acupuncture and simple acupuncture.Eighty-eight cervical spondylosis patients with neck pain who met the inclusion criteria were randomly divided into an observation group and a control group, 44 cases in each one. The patients in the control group were treated with conventional acupuncture, and grain-moxibustion was applied on the basis treatment of the control group in the observation group. The treatment was given 3 times a week, once every other day, 10 times as one course, and followed up after 6 months. Northwick Park neck pain inventory (NPQ) and the McGill pain questionnaire (MPQ) of the two groups before and after treatment and 6 months follow-up were observed. The clinical efficacy was compared.After one course of treatment in both groups, the effective rate was 84.1% (37/44) in the control group and 86.4% (38/44) in the observation group; after 6 months follow-up, the effective rate was 79.5% (35/44) in the control group, and 81.8% (36/44) in the observation group. There was no significant difference in overall efficacy between the two groups (both P>0.05). Compared before treatment, the NPQ scores and MPQ scores were decreased at the end of one course of treatment and after 6 months follow-up in the two groups (all P<0.05). There was no significant difference in NPQ scores and MPQ scores between the two groups after one course of treatment (both P>0.05), but the NPQ scores and MPQ scores in the observation group were significantly lower than those in the control group after 6 months follow-up (both P<0.05).The immediate efficacy of grain-moxibustion combined with acupuncture for cervical spondylosis with neck pain is similar to that of simple acupuncture, however, the long-term curative effect is better.

    Objective To investigate the efficacy of traditional proximal needling in treating cervical spondylosis of nerve root type. Methods Sixty-six patients were randomly allocated to a treatment group (proximal needling plus electroacupuncture) and a control group (conventional needling plus electroacupuncture), 33 cases each. The therapeutic effects were evaluated using the simplified McGill pain scale, the cervical spinal function assessment table and the conventional criteria of the therapeutic effect after 20 days' treatment. Results The pain and neck function improved to a certain degree in both the treatment and control groups (P0.05). But the neck symptoms and signs, activities of daily living and the pain index improved more in the treatment group; there was a statistically significant difference between the two groups (P0.05). Conclusion Traditional proximal needling plus electroacupuncture can obviously improve the pain and neck function in patients with cervical spondylosis of nerve root type.
