    Abstract:
    Objective :To discuss the outcoming of treating with mitomycin C(MMC) after TUVBt as a treatment of superficial bladder carcinoma.Method:Retrospectively study of 46 cases from 1999 to 2005,underwent TUVBT.Result:Postoperation all the patients received mitomycin C(MMC) intravesical instillation.no one was recurrence in 33 cases followup duration 36 months,7 case recurrence.Conclusion :TUVBT is less invasive,with clear visual field,definitive resection depth,and less recurrence in treatment of superficial bladder carcinoma.

    Objective: To investigate the efficiency of bladder instillation of pirarubicin(THP) combined with mitomycin C(MMC) for prevention of TURBt postoperative superficial bladder urothelial carcinoma recurrence.Methods: A total of 78 patients underwent carcinoma of the urinary bladder surgery were randomly divided into two groups,one group(40 patients) was treated with bladder instillation of pirarubicin(THP) for a course of 12 months and the other(38 patients) was dealt with bladder instillation of mitomycin C(MMC) for 2 years.Recurrence and side effects were observed and compared.Results: In the THP group,2-year recurrence rate and adverse reaction rates were 15.0% and 27.5%,respectively,and the corresponding data of the MMC group were 28.9% and 31.6% of MMC group.Significant difference in recurrence rate was observed(P0.01).Conclusions: THP bladder perfusion shows better effeciency for prevention of postoperative recurrence of bladder cancer than MMC.MMC is also commonly used because it is cheaper.

    (Purpose) We studied prophylactic intravesical instillation of mitomycin C (MMC) and pirarubicin (THP) following transurethral resection of bladder tumor (TUR-Bt) for superficial bladder cancer.(Materials and Methods) Forty-six evaluable patients were administered intravesically 20mg of MMC dissolved in 20ml saline on day 1 and 20mg of THP dissolved 20ml 5% dextrose on day 2. The patients were followed up by cystscopy and urinary cytology. Intravesical instillations were performed once a month and continued for 5 years.(Results) The non-recurrence rates at 1, 3 and 5 years were 88.8%, 79.5%and 67.0%, respectively. No significant differences were observed between grade 1-2 and 3, male and female, and solitary and multiple tumors. Although the side effects were relatively mild, 6 patients were stopped intravesical instillation.(Conclusions) Because non-recurrence rates of our report is not better than previous reports with shorter treatment periods, intravesical MMC and THP instillation for 5 years is not beneficial to the patients with superficial bladder cancer.

    AIM To evaluate intravesical instilliation of epirubicin (EPI) for the prevention of postoperative recurrence of bladder cancer. METHOD Transurethral resection or partial cystectomy was performed on 63 patients with superficial bladder cancer, followed by EPI intravesical instillation once a week for 8 weeks and then once a month until 1 year after surgery. RESULTS All patients have been followed-up for 7 to 28 months. It showed that tumor recurrence occurred only in 4 patients(6.3%). No systemic adverse reaction has been observed, and only 5 patients have transitory bladder irritation. CONCLUSION Postoperative intravesical instillation of EPI for superficial bladder cancer has good prophylactic effects on the recurrence with least side effects.

    Objective To evaluate the clinical effect of instantly intravesical instillation of pirarubicin for preventing recurrence of superficial bladder cancer after transurethral bladder tumor resection.Methods One hundred and two cases of superficial bladder cancer were randomly divided into instantly instillation group (group A,n =57) and routine instillation group (group B,n =45).Group A accepted intravesical instillation of pirarubicin 30 mg within 6 h after transurethral bladder tumor resection.Then two groups started routine treatment of pirarubicin after 1 week of operation.The tumor recurrence rates and the complications associated with therapy were observed between the two groups.Results After operation,the tumor recurrence rates of group A (8 cases) and g,roup B (15 cases) were 14.03% and 33.33%,the average recurrent time of group A and group B were 15 months and 10 months.There were significant differences between the two groups in recurrence rates and average recurrent time (P < 0.05).After intravesical instillation,27 cases of patients were complicated by cystitis or hematuria.There were no significant differences between the two groups(P > 0.05).Conclusions Instantly intravesical instillation of pirarubicin is a effective and safe method for superficial bladder cancer after TURBT.It can reduce the tumor recurrence rate and prolong the average recurrent time without increasing the incidence of complications.     Objective To evaluate intravesical epirubicin instillation for preventing the recurrence of superficial bladder tumors after surgical management. Methods From Aug.1998 to Aug.1999, 32 cases of superficial bladder tumors after TURBt or partial cystectomy, received early single intravesical epirubicin instillation and the patients have been followed up for 12~24 months with cystoscopy. Results The patients followed up from 12~24 months after surgery(average time was 18 months).In 4 patients the tumor recurred 3, 6, 7 and 8 months after surgery.No side effect and complication was observed either during or after the instillation. Conclusions Early single intravesical epirubicin instillation is markedly effective for preventing the recurrence of superficial bladder tumors after surgical management. Its toxic side effects are mild. This method is of high clinical value.

    Objective To evaluate the efficacy of intravesical instillation of mitomycin C after transurethral plasmakinetic resection for the prophylaxis of superficial bladder cancer recurrence.Method A total of 73 cases of superficial bladder cancer underwent transurethral plasmakinetic resection.All the cases were treated with intravesical instillation of 40 mg mitomycin C immediately after the operation.Regular intravesical instillation of mitomycin C has been given postoperatively to all cases for 1 year.All cases were followed up for 12 to 36 months(the mean is 24.6 months).Result There are 3,11,15 cases recurred at 6,12,20 months respectively.No serious side effect and complication developed either during or after instillation in all the cases.Conclusion Intravesical instillation of mitomycin C has been effective against recurrence of superficial bladder cancer after transurethral plasmakinetic resection,with advantages of good compliance and less systemic side effects.

    Objective To evaluate the alternate intravesical instillation of MMC and IL 2 together with BCG for the prevention of bladder cancer recurrence. Methods The instillation was carried out for 41 cases of bladder cancer after surgical management.All the patients have been followed up for 38~66 months with an average of 50 months.TNFa levels were periodically detected after the instillation. Results No recurrence has been noted in all the 34 patients of Ta,T 1 and T 2 tumors except 1 with mixed squamous cancer. 3 of 7 T 3 cases underwent TURBT,recurrence was noted in 2 and the TNFa remained much higher after the instillation. Conclusions Alternate intravesical instillation of MMC and IL 2 together with BCG is an effective means of preventing bladder cancer recurrence.TNFa detection might serve as a marker for bladder cancer recurrence.

    Objective To evaluate the intravesical instillation of pirarubicin (THP) after resection of superficial bladder cancer. Methods In 45 patients after transurethral resection of the superficial bladder cancer or partial cystectomy,intravesical instillation of THP was given once a week for 8 weeks and then once a month until one year after surgery.The dose of THP was 40mg dissolved in 40ml of 5% glucose solution and retained in the bladder for 30 minutes.The patients have been followed up for 9 to 12 months. Results 44 of the 45 patients have been free from the tumor (97.8%).Tumor recurrence occurred only in 1 patient 6 months after the initial surgery.Transitory bladder irritation was noted only in 2 cases.No systemic side effect has been observed. Conclusions Intravesical instillation of THP has been effective against local recurrence of superficial bladder cancer with least side effects.

    Objective To investigate the effective method for preventing the postoperative recurrence of bladder cancer. Methods 62 cases of bladder cancer were treated by conservative bladder operations including TURBt and partial cystectomy,and followed by postoperative intravesical instillation of MMC adding IFN-α2b. 68 cases of bladder cancer as control group were treated by identical operations and instilled with MMC after operation only.The preventive effect of two groups was compared. Results The recurrence rate of MMC+IFN group was 8.5%,which was less than MMC group,and the side-effects occurred in MMC+IFN group were very mild. Conclusions Intravesical instillation of MMC adding IFN-α2b is an effecfive method for preventing recurrence of bladder cancer.

    Objective:To evaluate the effect of transurethral resection of bladder tumor(TUR-Bt) combined with Mitomycin C(MMC)of intravesical instillation in the treatment of bladder cancer.Method:Twenty-nine cases with bladder cancer after TUR-Bt were given MMC of intravesical instillation for 2 years.Results:All the patients were operated successfully.There were 4 cases to recur(the recurrence rate was 17.6%),followed up from 6 to 50 months.Conclusion:TUR-Bt combined with MMC of intravesical instillation is the safe and effective way in the treatment of bladder cancer.
