Abstract:
Objective To observe the long-term clinical effect of molars with chronic apical periodontitis treated by empty root canal medication.Methods Two hundred thirty-eight molars with chronic apical periodontitis were treated by empty root canal medication.Results Five years after operation,213 molars were successfully treated and the success rate was 89.49%.Conclusion Empty root canal medication is a feasible and effective method for the treatment of molars with chronic apical periodontitis.Cite
AIM:To evaluate the therapeutic efficiency of Vitapex in single visit on chronic apical periodontitis combined with sinus.METHODS: 98 teeth affected with chronic apical periodontitis and combined with sinus were rinsed with hydrogen peroxide solution,followed by root canal filling permanently with Vitapex and gutta percha.RESULTS: After one week,95 cases showed no spontaneous pain and disappearance of sinus(96.93%).After six months,90 teeth showed success(91.83%).CONCLUSION: Chronic apical periodontitis with sinus treated by single-visit Vitapex and gutta percha is an acceptable method.

The aim of this study was to investigate the technical quality of root fillings in root filled teeth, their association with periapical status and prevalence of apical periodontitis.The sample consisted of 83 subjects, presenting consecutively as new patients seeking dental care (prosthetic, endodontic and cariologic treatment) in the years 2005/2006. Clinical and radiographic examination on each patient was performed using the scoring system (Periapical index (PAI)) proposed by Ørstavik et al. From the periapical radiographs status of endodontically treated teeth was recorded. For each tooth the following items were surveyed: the presence of a root filling, its quality (lateral seal and length in the root canal) and the periapical status.Of the 2186 functional teeth, 283 had undergone root canal treatment (13%). Amongst 283 root filled teeth, 122 teeth (43.1%) had radiological signs of a periapical lesion (PAI>2). Only 28.6% of the root filled teeth fulfilled the criteria of an acceptable root canal filling. Inadeguate lateral seal of root filling was observed in 165 (58.3%) of 283 endodontically treated teeth. Inadequate length of endodontic treatment was discovered in 183 (64.7%) out of 283 teeth. Root filled teeth without voids had apical periodontitis in 25 (21.0%) out of 118 of cases, whereas if voids were detected, disease was present in 97 (58.8%) out of 165 teeth (p<0.001). Apical periodontitis was found in 23 (23.0%) out of 100 teeth with adequate length of root filling, whereas if the filling was too short or long, periapical lesions were present in 99 (54.0%) out of 183 teeth (p<0.001).Apical periodontitis was present in 43,1% of root filled teeth. Only 28.6% of the root filled teeth fulfilled the criteria of an acceptable root canal filling. The results of this study indicate that inadequate root fillings were more often associated with an increased prevalence of apical periodontitis. On the contrary, adequate root fillings significantly reduced the prevalence of disease. Many root canal treatments were technically unsatisfactory and substantial efforts must be made to improve the standard of endodontic treatment.

Objective To explore if one time root canal therapy has a better effectiveness than conventional root canal therapy in treating acute periapical periodontitis of anterior teeth.Methods A total of 140 patients with acute periapical periodontitis of anterior teeth were randomly divided into two groups.Each group had 70 patients in it.They were treated respectively by one time root canal therapy and conventional root canal therapy.The complications were observed in the patients after one day's and five day's treatment.Their therapeutic effects were compared according to clinical symptoms and X-ray examination one year after treatment.Results The incidence of complication of one time root canal therapy was higher than that of conventional root canal therapy after one day's treatment(P0.05).The therapeutic effect of one time root canal was better than that of conventional root canal therapy after five day's treatment,but the curative effects by both methods had no significant difference after one year(P0.05).Conclusion Both of conventional root canal therapy and one time root canal therapy can be adopted in treating acute periapical periodontitis of anterior teeth.One time root canal therapy must be carefully employed.

Objectives:To investigate an effective treatment for the rapy-resistant apical periodontitis and evaluate clinical effect of mycostatin,as root canal medicament,in treating therapy-resistant apical periodontitis.Methods:Mycostatin was sealed in root canals in 76 cases with therapy-resistant apical periodontitis,and post-operation response was evaluated.Results:The mycostatin was significantly effective in 53 cases, effective in 17 cases and invalid in 6 cases.Conclusions:In some apical periodontitis, the pain wouldn't be relieved after conventional medication treatment, but better clinical results can be obtained after sealing mycostatin in root canals.

Effectiveness of singleversus multiple-visit endodontic treatment of teeth with apical periodontitis
Objectives To investigate the effectiveness of one-appointment versus multiple-appointment endodontic therapy for asymptomatic teeth with apical periodontitis.Methods 160 asymptomatic teeth with apical periodontitis were assigned into two groups with stratified randomization for root canal treatment.One group was treated with one-appointment method;the other group was treated with multiple-appointment method.The incidence and degree of postoperative pain after root canal treatment in the two groups were recorded.The effectiveness of endodontic therapy was evaluated according to the radiographs before and after operation.Results There were not statistically significant differences in the incidence of postoperative pain and the effectiveness of endodontic therapy between two groups.Conclusions One-appointment endodontic therapy is feasible in selected cases of teeth with apical periodontitis.

Objective:To investigate the clinical efficacy of disposable root canal treatment of chronic periapical periodontitis.Methods:80 cases of patients with chronic periapical periodontitis were divided into observation group and the control group,with 40 cases in each group.The observation group with disposable root canal treatment.The control group used the multiple root canal treatment,Then comparison of two groups' clinical effect.It is worthy of clinical application.Results:The incidence of pain is higher in control group than the observation group after treatment 1 day(p 0.05).There is no significant difference of treatment success rate in the two groups after 6-month follow-up(p 0.05).Conclusion:Disposable root canal treatment have similar long-term effects to the multiple root canal treatment,it can reduce the apical tissues around the physical and chemical and physical stimulation,it is worthy of clinical application.

Objective
To investigate clinical effect of file hand stainless steel and nickel-titanium instrument combined with sonic endodontic on chronic periapical periodontitis for C-shaped root canals in mandibular second molars.
Methods
140 patients with chronic periapical periodontitis for C-shaped root canals in mandibular second molar were chosen and randomly divided into two groups, including A group (70 patients) with file hand stainless steel and B group (70patients) with sonic endodontic combined with nickel-titanium instrument.The following-up success rate of root canal therapy, filling quality of root canal, incidence of pain reaction and complication of root canal preparation between the two groups were compared.
Results
The following-up success rate of root canal therapy in 6 months after treatment of A group and B group were 32.86%(23/70), 67.14%(47/70). The following-up success rate of root canal therapy in 12 months after treatment of A group and B group were 64.29%(45/70), 72.86%(51/70). The following-up success rate of root canal therapy in 6 months after treatment of B group was significantly higher than A group(χ2=10.27, P 0.05). The good filling rate of root canal of B group was significantly higher than A group(χ2=9.70, P 0.05).
Conclusion
Nickel-titanium instrument combined with sonic endodontic on chronic periapical periodontitis for C-shaped root canals in mandibular second molars can efficiently improve rational symptoms, enhance filling quality of root canals and reduce pain degree and complication risk, the effect is better than file hand stainless steel for canal preparation.

Objective To compare the clinical effect of empty root canal drug therapy and root canal filling therapy for deciduous teeth endodontitis.Methods From March 2008 to December 2009,160 child patients with deciduous teeth endodontitis were treated by empty root canal drug threapy and root canal filling threapy respectively.These 160 cases were randomly divided into two groups,treated respectively by empty root canal drug therapy and root canal filling therapy.Compare clinical efficacy of two kinds of treatment.Results The treatment success rate of empty root canal drug therapy was 97.5%.It is slightly higher than that of root canal filling therapy,which was 93.8%.There was no significant difference between the two groups(P 0.05).Conclusion Empty root canal drug therapy was higher success rate.It is very simple and can reduce the number of children referral.It is a better treatment suitable for children.
