Abstract:
Objective To observe the long-term clinical effect of molars with chronic apical periodontitis treated by empty root canal medication.Methods Two hundred thirty-eight molars with chronic apical periodontitis were treated by empty root canal medication.Results Five years after operation,213 molars were successfully treated and the success rate was 89.49%.Conclusion Empty root canal medication is a feasible and effective method for the treatment of molars with chronic apical periodontitis.Cite
Objective To evaluate the effect of one-visit endodontic treatment on chronic periapical periodontitis.Methods Eighty teeth with chronic periapical periodontitis were randomly divided into one-visit root canal treatment group and two-visit root canal treatment group.The root canals were prepared with modified double-flared technique and balanced force technique.Teeth in one-visit root canal treatment group were filled with gutta-percha and AHPLUS sealer by using lateral condensation technique.Teeth in two-visit root canal treatment group were medicated with calcium hydroxide.Patients were reviewed for pain 1,7 and 30 days after treatment.Clinical and radiographic examination was performed 1 year after treatment.The success rate of root canal treatment was compared between the two groups.Results The incidence of pain on postoperative day 1 in one-visit root canal treatment group was significantly higher than that in two-visit root canal treatment group(P0.05).There were no statistical differences in long-term follow-up evaluation between the two groups(90.0% vs 92.5%,P0.05).Conclusion One-visit endodontic treatment can shorten the duration of pain in chronic periapical periodontitis.

AIM:To evaluate the short period clinical responses after one-visit root canal filling and pulpectomy for infected permanent teeth.METHODS:Treatment records of 1100 teeth with acute or chronic pulpitis or chronic apical periodontitis were compared. 514 teeth were treated by debridement and instrumentation, followed by obturation at a second visit as control group. 586 teeth were treated by one-visit root canal filling as experimental group. The effects were evaluated through clinical observation.RESULTS:The ratio(%) of severe acute reaction in 1 week following root canal filling in control group and experimental group was 5.2%,5.1%,in anterior and 9.1%,10.0%, in postevior respectively ( P 0.05).CONCLUSION:One-visit root canal filling causes no more severe acute reaction compared with traditional method.

Objective To explore the clinical curative effect of root canal therapy(RCT) to pulpitis and apical periodontitis.Methods Nine hundred ninety eight patients(1 008 teeth)with pulpitis and apical periodontitis were treated with RCT.Root canal filling quality and treatment period of these teeth were evaluated.Teeth of the total were followed up 2 years and 2-year success rate were evaluated.And to observe the failure factors in RCT.Results ①In 1 008 teeth,the number of adequate filling,underfilling,and overfilling were 783,177 and 48.The ratios of adequate filling,underfilling,and overfilling were 77.68%,17.56% and 4.76%,receptively.②Before the root canal filling was significantly less than molar(P0.01).③Following up 2 years,the failure rate is 12.28% in 1-year,the failure rate is 15.29% until 2-year.Conclusion The clinical curative effect of RCT to pulpitis and apical periodontitis is excellent.The numbers and morphological difference type of root canal,root curvature and calcification have significantly influenced outcome of RCT.

Objective:To observe the curative effect of one-off root canal treatment for chronic front teeth apical periodontitis with fistula.Methods:131 front teeth from 115 cases with chronic apical periodontitis with fistula were randomly divided into two groups.Group A was the one-off root canal therapeutic that involved 72 teeth in 63 cases.Group B was the traditional many-time canal therapeutic as control group the involved 59 teeth in 52 cases.Two years later,these lesion teeth were performed examination and the cure effects between the two groups were compared.All data were managed statistically.Results:The long-term rates of cure were 93.06%(67/72 teeth) in Group A and 94.92%(56/59)in Group B(p0.05).There were no significant difference in the cure effects between the two groups.Conclusions:One-off root canal therapeutic method not only has the same effect as the traditional method,but also has the advantage of saving time and expenses.Therefore,this method should be superior to the traditional method for treatment of chronic front teeth apical periodontitis with fistula.

REVASCULARIZATION IN MATURE PERMANENT TEETH WITH NECROTIC PULP AND APICAL PERIODONTITIS: CASE SERIES
INTRODUCTION: Revascularization procedures have been limited to immature teeth but application to mature teeth should be clinically investigated as an alternative to conventional root canal treatment. The translation of regenerative endodontic procedures into treating mature teeth is challenging and depends on efficient root canal disinfection, proper size of the apical foramen, the availability and delivery of stem cells into the root canal system. OBJECTIVES: Evaluation of revascularization in mature teeth after enlargement of the apical foramen to size 35 K-file. MATERIALS AND METHODS: Three necrotic incisors with mature roots and apical periodontitis in three patients ranging from 20–30 years old were treated by revascularization procedures. Access opening was performed. The canal was mechanically instrumented to the radiographic apex. The apical foramen was enlarged with a size 35 K-file. Triple antibiotic paste (0.1 mg/ml) was applied in the canal using a syringe. After three weeks, the medication was removed with 20 ml of 1.5% sodium hypochlorite followed by final irrigation with 20 ml of 17% EDTA. Bleeding was induced in the root canal and mineral trioxide aggregate (MTA) was placed approximately 3-4 mm below the CEJ. The tooth was restored with a layer of glass ionomer followed by composite resin. The patient was scheduled for follow up and evaluation of healing after 3 months, 6 months and 9 months. Resolution of apical periodontitis and regression of clinical signs and symptoms were observed during the follow up periods. RESULTS: After a follow-up period of 9 months, the three teeth demonstrated radiographic evidence of periapical healing with absence of clinical signs and symptoms. CONCLUSIONS: The present cases demonstrated a favorable outcome of the revascularization procedure in mature necrotic incisors with chronic apical periodontitis

Objective To investigate the clinical effect of root canal filling paste Vitapex on intractable apical periodontitis.Methods 58 cases with intractable apical periodontitis were treated by expanding tooth root canal and filling with paste Vitapex and gutta\|percha point.Results All cases were cured of through 6 months to 3 years clinical follow\|up survey. All sinuses in 19 cases complicated with sinus disappeared. Apical shadows in 38 cases disappeared and those in 20 cases reduced significantly. 16 patients felt the affected tooth floated, extended and chewing pain in the very day when their affected teeth were treated, and their symptoms spontaneously alleviated in 2 to 3 days. Conclusion Root canal filling paste Vitapex is a perfect filling material which was used to treat intractable apical periodontitis.

Root canal treatment of teeth with necrotic pulps and apical periodontitis may be complicated by limited access to the root canals due to restorations and dystrophic calcifications. The objective of this study was to evaluate retrograde root canal as a primary treatment using a surgical approach as an alternative to conventional orthograde treatment.Patients with apical periodontitis in the anterior region of the maxilla were consecutively recruited to the study over a period of 4 years. Fifty-seven patients met the inclusion criteria and received retrograde root canal treatment. A clinical and radiographic evaluation was made after 1 and 2 years postoperatively.Clinical and radiographically evaluation after 2 years revealed a successful outcome (as defined in this study) in 90% of the cases.Retrograde root canal as a primary treatment was a reliable alternative to treat apical periodontitis on single- and two-rooted teeth with limited orthograde access to the root canals in the maxilla.

Objective:To evaluate the thereutic efficiency of Vitapex for canal filling on chronic apical periodontitis with sinus of primary teeth.Methods:76 primary teeth with chronic apical periodontitis by root canal filling with vitapex.Results:After two week,71 cases showed on no pain and disappearance of sinus(93.4%).Conclusion:Vitapex canal filling is a good methods in treating chronic apical periodontits with sinus of primary teeth.
