Intravitreal bevacizumab injection on idiopathic choroidal neovascularization

2011
Objective To evaluate the efficacy of intravitreal bevacizumab injection for idiopathic choroidal neovascularization. Methods An uncontrolled retrospective clinical study was taken.Thirty-two eyes from 32 patients with the idiopathic choroidal neovascularization were diagnosed by indirect or direct ophthalmoscope, fundus fluorescein angiography, fundus indocyanine green angiography and optic coherence tomography. The course of diseases were from 10 days to 1 years.Bevacizumab(25 g·L-1) was intravitreal injected from 1 time to 3 times with the interval 4 weeks. The visual acuity, fundus findings, fluorescein leakage in fundus fluorescein angiography, fundus indocyanine green angiography, and central foveal thickness were recorded and analyzed before injection and the last follow-up.The follow-up time was from 4 months to 18 months. Results The mean baseline logMAR visual acuity before injection and the last follow-up were 0.235 90±0.197 51 and 0.397 50±.0272 87,there was significant difference(t=-5.530,P0.001).The mean central foveal thickness before injection and the last follow-up were(317.72±75.01)μm and(206.28±31.56)μm,there was significant difference(t=8.857,P0.001).The fluorescein leakage was obviously disappeared in 18 eyes, relieved in 12 eyes and sustained in 2 eyes. The subconjunctival hemorrhage occurred in 5 eyes, the vitreous hemorrhage occurred in 1 eye and IOP obviously dropped in 3 eyes,no other complication was found. Two cases(6.25%) relapsed after injection, while re-injection was effective. Conclusion Intravitreal bevacizumab injection is safe and effective for idiopathic choroidal neovascularization, but the multi-center randomized controlled clinical study is needed to evaluate the long-time safety and efficacy.
