Características clínicas de adolescentes con alteraciones citológicas

Caracterizar las adolescentes con alteracion citologica que asistieron a confirmacion diagnostica en una institucion de salud. Estudio observacional analitico. La poblacion fueron todas las mujeres menores de 19 anos. La informacion se obtuvo de la base de datos de la institucion. Se analizaron variables relacionadas con el resultado de la citologia y de la biopsia. Se realizo analisis uni y bivariado, se calculo el OR con su intervalo de confianza (IC 95%) y nivel de significancia. Se realizaron calculos exploratorios de la concordancia. La lesion intraepitelial de bajo grado fue la mas frecuente con un 43.8%, la oportunidad en el acceso para la colposcopia tuvo una mediana de 47 dias. Los factores asociados con neoplasia intracervical de alto grado fueron: el regimen de afiliacion a salud p: 0.003 (OR 3.1 IC 1.39-7.0), la lesion intraepitelial de alto grado en la citologia p: 0.0004 (OR 7.66 IC 2.08-28.18) y el resultado de la colposcopia grado I con p: 0.02 (OR 6.33 IC 1.10-36.16) y grado II p: <0.0001 (OR 31.9 IC 5.8-173). Las adolescentes son un grupo especial de mujeres vistas bajo la optica de la patologia cervical, pueden presentar frecuencias altas de alteraciones citologicas de bajo grado, bajas tasas de neoplasias intracervicales de alto grado y ningun cancer de cervix. To characterize the adolescents with cytological alteration who attended a diagnostic confirmation in a health institution. Analytical observational study. The population was all women under 19 years. The information was obtained from the institution’s database. Variables related to the result of cytology and biopsy were analyzed. Univariate and bivariate analysis was performed; the OR was calculated with its confidence interval (95% CI) and level of significance. Exploratory concordance calculations were performed. Low-grade intraepithelial lesion was the most frequent with 43.8%, the opportunity for colposcopy access had a median of 47 days. The factors associated with high-grade intracervical neoplasia were: health affiliation regimen p: 0.003 (OR 3.1 CI 1.39-7.0), high-grade intraepithelial lesion in cytology p: 0.0004 (OR 7.66 CI 2.08-28.18) and the result of grade I in colposcopy with p: 0.02 (OR 6.33 IC 1.10-36.16) and grade II p: <0.0001 (OR 31.9 IC 5.8-173). Adolescent girls are a special group of women seen from the perspective of cervical pathology; they can present high frequencies of low-grade cytological abnormalities, low rates of high-grade intracervical malignancies and no cervical cancer.
