Therapeutic effect of Bi-directional shunt on complex congenital heart disease

Objective To summarize the clinical experiences of Bi-directional shunt (GLENN operation) in treatment of complicated congenital heart diseases(CHD). Methods The data of 44 patients with CHD were retrospectively analyzed in whom 39 patients underwent the procedure without cardiopulmonary bypass(CPB). The superior vena cara (SVC) and right atrium (RA) were connected,and 5 patients underwent the operation under the CPB whose aortas were not clamped.6 patients who had left SVC underwent the bi-directional GLENN operation on both sides. Results 2 patients died after operation,1 patient had circulatory failure and 1 patient happened renal failure. The other 42 patients recovered well after operation,and patients' life qualities were improved obviously,and they could be competent enough for common physical activities smoothly,whose cyanoses were relived,and transcutaneous oxygen saturation was increased from (70.2±5.1)% before operation to (92.5±1.5)% after operation. Conclusion GLENN operation is a feasible method to treat complex congenital heart disease,with correct operative indications,surgical skill and postoperative management.
