Dabigatran is a novel anticoagulant that works through direct thrombin inhibition and is predominantly eliminated through the kidneys. A reduced dosage is recommended for patients whose creatinine clearance (CLcr) is 15–30 mL/min, and it is not recommended in patients with a CLcr of <15 mL/min.[1
