Healthcare is costly and complex, and medical errors can lead to poor outcomes. Recent advances in pediatric hematology, oncology, and stem cell transplantation have improved outcomes with increased quantity and quality of survival for many. The therapies are complex and present challenges to maintaining quality and safety in practice. As providers, it is our responsibility to ensure our patients receive the right care at the right time, in as safe a manner as possible.
