Effect of structured lipid emulsion on protein and lipid metabolism

Objective To study the influence of structured lipid emulsion on protein and lipid metabolism in gastrointestinal surgery patients.Methods Sixty patients were randomly allocated into structured lipid emulsion group(ST,n=30)and physical mixture of medium-chain/long-chain triglycerides emulsion group(MCT/LCT, n=30).They received isonitrogenous and isocaloric parentenal nutrition(PN)during post-opD1 to post-op D5. Liver and renal function tests,serum prealbumin,triglyceride and cholesterol were measured before operation, post-opD1 and post-opD6.Twenty patients of each group were performed the serum triglyceride,cholesterol and free fatty acids on post op D2 before and after PN.Results①No significant differences were observed in the liver and renal functions between the two groups.②After PN 5 d.comparing with the basal value,the prealbumin le- vels of each group was decreased still markedly.③The levels of serum triglyceride and cholesterol were significant- ly reduced on post-op D2 after PN,except the oleic acid.(C18:1),other free fatty acid levels showed no obvious differences before PN.The level of total free fatty acids in ST group was lower than that in the LCT/MCT group, but with no statistical significant difference.Conclusion It is safe to infuse structured lipid emulsion postoperative- ly to the patients undergone gastrointestinal surgery,and it is as efficacious as the physical mixture on the protein metabolism and with less influence on the lipid levels.(Shanghai Med J,2006,29:534-537)
