An Unmatched Case‐Control Study of Nearly Lethal Suicide Attempts in Houston, Texas: Research Methods and Measurements

This article details the research methods and measurements used in conducting a population-based, case-control study of nearly lethal suicide attempts among persons aged 13-34 years, residing in Houston, Texas. From November 1992 to July 1995, we interviewed 153 case subjects presenting at one of three participating hospital emergency departments and used random digit dialing to identify 513 control subjects residing in the same catchment area in which cases were enlisted. Unlike most research in this area, this study was designed to extend our understanding of suicidal behavior and prevention activities beyond identification and treatment of depression and other mental illnesses. We discuss the overall strengths and weaknesses of our study design and conclude that this methodology is well suited for studying rare outcomes such as nearly lethal suicide.

The article by Wintemute et al.1 in this issue of the Journal adds to the growing literature that connects firearms with increased risks of suicide and homicide.26 Some of these studies have examined the risks associated with the possession of firearms, and some, the risks associated with ownership; now, this article looks at the risks in relation to firearm purchase. Abstract In case‐control studies, determination of alcohol consumption by cases immediately prior to the injury event is often conceptually straightforward. However, determination of consumption status by controls is difficult because they lack a reference point, especially when cases and controls are not individually matched. We describe a method of assigning alcohol consumption status to controls using a 24‐hour drinking history, the distribution in time of case events, and the random assignment of a specific time period to each control subject. This methodology offers a practical approach for determining alcohol consumption status among control subjects immediately prior to a case event, when controls lack a reference point and have not been individually matched to cases. Characteristics of Impulsive Suicide Attempts and Attempters

S uicide attempts often are impulsive, yet little is known about the characteristics of impulsive suicide. We examined impulsive suicide attempts within a population‐based, case‐control study of nearly lethal suicide attempts among people 13–34 years of age. Attempts were considered impulsive if the respondent reported spending less than 5 minutes between the decision to attempt suicide and the actual attempt. Among the 153 case‐subjects, 24% attempted impulsively. Impulsive attempts were more likely among those who had been in a physical fight and less likely among those who were depressed. Relative to control subjects, male sex, fighting, and hopelessness distinguished impulsive cases but depression did not. Our findings suggest that inadequate control of aggressive impulses might be a greater indicator of risk for impulsive suicide attempts than depression.

Demographic Characteristics of Rural Residents in Financial Distress and Social and Community Impacts of the Farm Crisis

This chapter examines the demographic characteristics of those being impacted by the crisis and the social and community-level impacts of the crisis. It explores the characteristics of two sets of persons being impacted by the crisis—producers and residents in agriculturally dependent rural communities, and also examines that groups that have been directly or indirectly impacted by the crisis. The chapter shows that those being most directly impacted by the crisis are producers and business operators in early career stages who entered agriculture or business in the 1970s. Data comparing the perceptions of producers and other rural residents and providing comparisons among persons who have been impacted to different degrees by the farm crisis have seldom been available. In describing the characteristics of those being affected by financial stress, the chapter also examines data from several different data sets. In like manner, those experiencing higher levels of financial stress were likely to have begun farming during the 1970s.

Young motherhood: levels and socioeconomic determinants of adolescent fertility in Ghana

Abstract Adolescent fertility levels have shown considerable improvements globally over the past decades. However, adolescent childbearing remains high in developing countries, particularly in sub-Saharan Africa. This study, thus, examines the levels and socioeconomic factors associated with adolescent fertility in Ghana. The study drew on data from the 2003, 2008, to the 2014 Ghana Demographic and Health Surveys to perform a logistic regression analysis of socioeconomic factors associated with adolescent fertility. The results show that adolescent childbearing levels have not shown any considerable improvements over the study periods (10%, 10%, and 11% for 2003, 2008, and 2014, respectively). Socioeconomic factors such as household wealth status, working status, employer status, and employment period were associated with adolescent fertility. Female adolescents from poor households, employed and self-employed adolescents, as well as regular workers, were linked to higher adolescent fertility risks. Older adolescents, and ever married adolescents also show significantly higher childbearing risks while the risk levels steadily increased over time. Promoting economic empowerment among female adolescents and targeting employed female adolescents in fertility control measures may have considerable positive implications for adolescent fertility levels in Ghana.

Levels and socioeconomic correlates of nonmarital fertility in Ghana

Childbirth outside marriage has several negative implications for the well-being of children, women, and families globally. In sub-Saharan Africa, however, the phenomenon appears to be under-studied. In this study, we examine the levels and socioeconomic correlates of nonmarital fertility in Ghana. Using pooled data from the 2003, 2008, and the 2014 Ghana Demographic and Health Surveys, logistic regression models were used in determining significantly predictive factors of nonmarital fertility. The results show that nonmarital fertility levels have been on the rise over time without any sign of reduction (24.0%, 33.0%, and 40.0% for 2003, 2008, and 2014, respectively). Some socioeconomic characteristics are linked to nonmarital fertility levels with women without formal education, women from poor households, and self-employed women having significantly higher nonmarital fertility risks. Also, older unmarried women, women who have an early sexual debut, cohabiters, women with unmet need for family planning are all associated with considerably higher risks of nonmarital childbearing. A few significant regional disparities also exist, with the Central Region having higher whereas the Upper West Region has lower risks of nonmarital fertility compared to the Greater Accra Region. Childbirth outside marriage is a social concern among women in Ghana. The findings have possible implications for bridging socioeconomic disparities among unmarried women.

Effect of socioeconomic inequalities and contextual factors on induced abortion in Ghana: A Bayesian multilevel analysis

There is a dearth of information on induced abortion in Ghana, possibly owing to the sensitive nature of the subject. In this study, we examine the effect of socioeconomic and contextual factors on induced abortion in Ghana. This study draws on data from the 2017 Ghana Maternal Health Survey. The study used a Bayesian multilevel logistic regression analysis to estimate both individual- and contextual-level factors affecting induced abortion levels in Ghana. The results show a total induced abortion prevalence of 19.6% coupled with considerable district-level disparities. Induced abortion is significantly associated with socioeconomic factors such as educational attainment, wealth status, and marital status at the individual-level. The risk of induced abortion is considerably higher among the educated, wealthy, and cohabiting women. The current age of women, age at first sex, religious affiliation, parity, and type of residence are the demographic factors having an association with induced abortion levels. At the contextual-level, district health insurance coverage and poverty rate have a significant association with induced abortion. Induced abortion appears to be prevalent in Ghana and is underpinned by both individual-level socioeconomic and aggregate-level factors. Addressing induced abortion levels in Ghana may require policies that take a multilevel approach by focusing on the socioeconomic status of women and district-level contextual factors.
