Abstract Purpose To develop two deep learning-based systems for diagnosing and localizing pneumothorax on portable supine chest X-rays (SCXRs). Methods For this retrospective study, images meeting the following inclusion criteria were included: (1) patient age ≥ 20 years; (2) portable SCXR; (3) imaging obtained in the emergency department or intensive care unit. Included images were temporally split into training (1571 images, between January 2015 and December 2019) and testing (1071 images, between January 2020 to December 2020) datasets. All images were annotated using pixel-level labels. Object detection and image segmentation were adopted to develop separate systems. For the detection-based system, EfficientNet-B2, DneseNet-121, and Inception-v3 were the architecture for the classification model; Deformable DETR, TOOD, and VFNet were the architecture for the localization model. Both classification and localization models of the segmentation-based system shared the UNet architecture. Results In diagnosing pneumothorax, performance was excellent for both detection-based (Area under receiver operating characteristics curve [AUC]: 0.940, 95% confidence interval [CI]: 0.907–0.967) and segmentation-based (AUC: 0.979, 95% CI: 0.963–0.991) systems. For images with both predicted and ground-truth pneumothorax, lesion localization was highly accurate (detection-based Dice coefficient: 0.758, 95% CI: 0.707–0.806; segmentation-based Dice coefficient: 0.681, 95% CI: 0.642–0.721). The performance of the two deep learning-based systems declined as pneumothorax size diminished. Nonetheless, both systems were similar or better than human readers in diagnosis or localization performance across all sizes of pneumothorax. Conclusions Both deep learning-based systems excelled when tested in a temporally different dataset with differing patient or image characteristics, showing favourable potential for external generalizability.
Synthesizing a CT image from an available MR image has recently emerged as a key goal in radiotherapy treatment planning for cancer patients. CycleGANs have achieved promising results on unsupervised MR-to-CT image synthesis; however, because they have no direct constraints between input and synthetic images, cycleGANs do not guarantee structural consistency between these two images. This means that anatomical geometry can be shifted in the synthetic CT images, clearly a highly undesirable outcome in the given application. In this paper, we propose a structure-constrained cycleGAN for unsupervised MR-to-CT synthesis by defining an extra structure-consistency loss based on the modality independent neighborhood descriptor. We also utilize a spectral normalization technique to stabilize the training process and a self-attention module to model the long-range spatial dependencies in the synthetic images. Results on unpaired brain and abdomen MR-to-CT image synthesis show that our method produces better synthetic CT images in both accuracy and visual quality as compared to other unsupervised synthesis methods. We also show that an approximate affine pre-registration for unpaired training data can improve synthesis results.
Recently, neural architecture search (NAS) has been applied to automatically search high-performance networks for medical image segmentation. The NAS search space usually contains a network topology level (controlling connections among cells with different spatial scales) and a cell level (operations within each cell). Existing methods either require long searching time for large-scale 3D image datasets, or are limited to pre-defined topologies (such as U-shaped or single-path). In this work, we focus on three important aspects of NAS in 3D medical image segmentation: flexible multi-path network topology, high search efficiency, and budgeted GPU memory usage. A novel differentiable search framework is proposed to support fast gradient-based search within a highly flexible network topology search space. The discretization of the searched optimal continuous model in differentiable scheme may produce a sub-optimal final discrete model (discretization gap). Therefore, we propose a topology loss to alleviate this problem. In addition, the GPU memory usage for the searched 3D model is limited with budget constraints during search. Our Differentiable Network Topology Search scheme (DiNTS) is evaluated on the Medical Segmentation Decathlon (MSD) challenge, which contains ten challenging segmentation tasks. Our method achieves the state-of-the-art performance and the top ranking on the MSD challenge leaderboard.
The performance of large-scale peer-to-peer (P2P) video-on-demand (VoD) streaming systems can be very challenging to analyze due to sparse connectivity and complex, random dynamics. Specifically, in practical P2P VoD systems, each peer only interacts with a small number of other peers/neighbors. Furthermore, its upload capacity, downloading position, and content availability change dynamically and randomly. In this paper, we rigorously study large-scale P2P VoD systems with sparse connectivity among peers and investigate simple and decentralized P2P control strategies that can provably achieve close-to-optimal streaming capacity. We first focus on a single streaming channel. Using a simple algorithm that assigns each peer a random set of Θ(logN) neighbors and allocates upload capacity uniformly, we show that a close-to-optimal streaming rate can be asymptotically achieved for all peers with high probability as the number of peers N increases. Furthermore, the tracker does not need to obtain detailed knowledge of which chunks each peer caches, and hence incurs low overhead. We then study multiple streaming channels where peers watching one channel may help peers in another channel with insufficient upload bandwidth. We propose a simple random cache-placement strategy and show that a close-to-optimal streaming capacity region for all channels can be attained with high probability, again with only Θ(logN) per-peer neighbors. These results provide important insights into the dynamics of large-scale P2P VoD systems, which will be useful for guiding the design of improved P2P control protocols.
