We report a unique case of a minute, occult synovial sarcoma of the lung detected intraoperatively during a pneumothorax repair in a 17-year-old boy. No alternative primary site could be detected upon complete body imaging studies and physical examinations. The diagnosis was confirmed by demonstration of the characteristic SYT/SSX gene fusion by reverse transcriptase polymerase chain reaction (RT-PCR) performed upon RNA extracted from the paraffin block of the biopsy. This case demonstrates the utility of this technique in diagnostic pathology.

Objective To investigate the feasibility and effectivity of polymerase chain reaction associated with direct gene sequencing technique, and to find a new method for rapid diagnosis of infective endophthalmitis. Methods 47 cases diagnosed as infective endophthalmitis were included. The specimen taken in operations were detected by polymerase chain reaction associated with direct gene sequencing technique. Meanwhile nacterial smear and bacteria, fungus, anaerobic culture were detected at the same time. Results It only took 8~10 hours by polymerase chain reaction associated with direct gene sequencing technique. In 47 cases, 25 PCR detections are positive. 19 pathogens had been detected in 25 cases with direct gene sequencing technique. Conclusions The polymerase chain reaction combined with direct gene sequencing technique is a rapid and effective method for the diagnosis of infective endophthalmitis comparing with traditional pathogen detection.
