    Anti-tumor immunity and vitiligo: from immune-related adverse reactions to protection from cancer

    Vitiligo is one of many diseases that can be found with diabetes mellitus. Diabetic patients have a high incidence of vitiligo compared to the general population. This study assesses prevalence of vitiligo in type 2 diabetic patients in Iran.One thousand one hundred type 2 diabetic patients and 1100 healthy adults were recruited in this cross-sectional study. Patients were evaluated for vitiligo by a dermatologist both clinically and by Wood's lamp. Data were analyzed using SPSS. P value ≤0.05 was considered statistically significant.There was a significant difference between groups with respect to presence of vitiligo; 4.9% of diabetic patients had vitiligo versus 1.8% of control group (P = 0.001). In the female group with diabetes, 5.8% had vitiligo, and in male diabetic patients, 3.9% had vitiligo (P = 0.057). In the control group, 1.8% of females and 1.8% of males had vitiligo that was equal.Vitiligo and diabetes may have a causal relationship. Vitiligo may coexist with type 2 diabetes. Therefore it is reasonable to investigate each patient periodically.

    SUMMARY. —A study has been made on the incidence of vitiligo in mature‐onset diabetics attending hospital. An increased incidence of vitiligo was found in the whole group of diabetics, and in the female diabetics, when compared with controls. In more than half the patients with both vitiligo and diabetes, the vitiligo preceded the diabetes. Vitiligo begins before the age of 40 in over 80% of all cases but did so in only 24% of 25 diabetics with vitiligo.

    A 71-year-old woman with vitiligo developed multiple actinic keratoses only on the vitiligo skin of sun-exposed areas. She had worked outdoors as a farmer for a long period. Although actinic skin cancers arising on vitiligo skin have rarely been reported, the present case demonstrates that vitiligo skin is more susceptible to actinic damage than normal skin. Patients with vitiligo should be well informed about the importance of sun protection.

    Vitiligo is an acquired pigmentary disorder, clinically characterized by depigmented macules caused by destruction of melanocytes in the affected skin. Half of all patients develop the disease in childhood and adolescence before the age of 20 years, making vitiligo an important skin disease of childhood. There are numerous studies in the literature that suggest the efficacy of topical tacrolimus in vitiligo, without serious adverse effects. We describe a case of vitiligo in a pediatric patient who developed hyperpigmentation in the periorbital lesions of vitiligo with the use of topical tacrolimus. To the best of our knowledge, this is only the second such reported occurrence in a patient with vitiligo.

    Six patients with spreading vitiligo treated with applications of monobenzone developed a vesicular dermatitis. The eruption was restricted exclusively to the pigmented areas of the skin. Patch tests applied to pigmented and depigmented skin produced an inflammatory response only within the pigmented areas. The explanation for this puzzling phenomenon is not known.

    Abstract Vitiligo is characterized by the selective destruction of melanocytes resulting in patches of skin depigmentation. Vitiligo is a therapeutic challenge. Eyebrows, eyelids and genital vitiligo are a therapeutic dilemma, especially in children. The possible side effects of topical corticosteroids and the difficulties for choosing any other adequate treatment option are a major concern. We present two children, one with vitiligo of the eyelids and the other with genital vitiligo, both treated with pimecrolimus 1% cream with almost full repigmentation of the lesions, showing pimecrolimus could be an adequate option for the treatment of vitiligo for these special vitiligo areas in children.
