Abstract:
Ethnopharmacological relevancePolygonum capitatum Buch.-Ham. ex D. Don (Pc, DB52/YC141-2003), a traditional herbal medicine of the Miao nationality, has been recognized for its therapeutic potential and efficacy in treating various urologic disorders, notably chronic bacterial prostatitis(CBP).Experimental and clinical evidence implicates Pc combination Ciprofloxacin(CIP) can effectively improve the clinical symptoms of CBP patients with urinary symptoms.Combination therapies are superior to monotherapy for CBP.However, the underlying mechanisms and constituents responsible for this synergistic effect remain elusive, limiting its implementation in clinical settings.Aim of the studyThis study aims to elucidate the potential synergistic mechanism of Pc and CIP in ameliorating CBP, and to find out which the major active ingredient of Pc contributed the most to the therapeutic efficacy combined with CIP.Materials and methodsCBP was induced by prostate bilateral injections of Escherichia coli (E. coli.) and treated with Pc combined with CIP.The therapeutic effect of Pc combined with CIP was assessed, prostate index, and bacterial colonization were quantified.The morphological changes of the prostate were observed by HE staining.Expressions of inflammatory mediators were measured by Western blotting (WB), qRT-PCR, and immunohistochemical staining.Transcriptome data were analyzed using Ingenuity Pathway Analysis (IPA) to elucidate the molecular mechanisms underlying the Pc combined with CIP, and key targets were verified by qRT-PCR and WB.Furthermore, tissue-thermal proteome profiling (Tissue-TPP) was conducted to identify potential targets of Pc combined with CIP, and the binding of Pc combined with CIP to these targets was validated using thermal shift assays.Molecular docking was conducted to predict the binding interactions between active compounds and key target proteins.Finally, the therapeutic effect of active ingredients in Pc combined with CIP was assessed.ResultsThe results indicated that Pc combined with CIP significantly reduced the prostatic index and bacterial concentration, restored the prostate gland structure, and inhibited the mRNA and protein expression of pro-inflammatory factors (TNF-α and IL-1β) in CBP rats.RNA-Seq combined with IPA analysis showed that Pc combined with CIP significantly inhibited inflammatory signaling pathway in CBP rats, especially the NF-κB/IL-6/JAK2/STAT3 pathway.Moreover, Tissue-TPP and thermal shift assays revealed that Pik3cb is a direct target of Pc combined with CIP, and molecular docking found that gallic acid (GA) can directly bind to Pik3cb, a predominant component of Pc.Finally, GA combined with CIP was also found to significantly improve CBP.ConclusionThe results suggest that Pc combined with CIP can mitigate CBP by targeting Pik3cb to inhibit the NF-κB/IL-6/JAK2/STAT3 signaling pathway, and GA is the key active component in Pc.Chronic prostatitis is possibly the most common infectious disease in middle-aged men. Clinical investigation was made on 24 outpatients with chronic prostatitis at our department, during the period of October, 1983 to August, 1984. The diagnostic criterion of the prostatitis was detection of more than ten leukocytes per high power field on microscopic examination of the expressed prostatic secretion and urine after prostatic massage. Twenty-four patients with chronic prostatitis were treated with 2 tablets of Dolcol (Pipemidic acid, PPA) three times daily for 7-30 days. The effectiveness was evaluated by improvement of subjective symptoms and objective findings. The results of the therapy with PPA on chronic prostatitis were excellent in 8 patients (33.3%), good in 10 (41.7%), fair in 2 (8.3%), and poor in 4 (16.7%). The effective rate was 83.3%, while no side effects were noted. Therefore, PPA was suggested to be effective against chronic prostatitis.

Chronic prostatitis is a common andrologic disease among adult men.The etiology and pathogenesis of the disease are not yet completely clear.Chronic prostatitis clinical practice guideline has advantage to modify the diagnosis and treatment of chronic prostatitis.The effects of Chinese medicine on chronic prostatitis is confirmed,and the Chinese medicine clinical practice guideline in chronic prostatitis should be established as soon as possible.This article reviews and discuses the diagnosis and treatment of NIH and Chinese medicine and their role in the prostatitis,especially there are good effects on treating chronic prostatitis by integrated Chinese and western medicine.

Objective To investigate the impact of psychological factors on pathogenic mechanism of prostatitis among the young soldiers. Methods This study was designed as across sectional survey of 2 500 young soldiers aged 18-30 years, all the men completed questionnaires on prostatitis. Questionaires also collected information about the chronic prostatitis-like symptoms, related factors and the psychological status. The final response rate was 85%(2 125men). The NIH-CPSI was used to determine the chronic prostatitis-like symptoms; 100 men with prostatitis-like symptoms and 100 healthy men were collected to investigate the psychological features of the young men with chronic prostatitis-like symptoms by using SCL-90. Results The psychological disturbance was common among the young soldiers with prostatitis-like symptoms.Conclusion Chronic prostatitis-like symptoms may be common among the young soldiers, and the psychological factor plays an important role in aggravating symptoms in chronic prostatitis patients.

NAG was measured before and after treatment and compared in 11 acute prostatitis patients, 13 chronic prostatitis patients and 10 healthy volunteers. Urinary NAG before treatment was 34.1 +/- 18.4 U/l in the acute prostatitis patients and was significantly higher than that in the healthy volunteers, which was 5.1 +/- 1.2 U/l. It was 8.5 +/- 3.2 U/l in the chronic prostatitis patients. Urinary NAG after treatment decreased and was 7.6 +/- 4.4 U/l and 6.7 +/- 1.7 U/l in the acute and chronic prostatitis groups, respectively. These results suggest that urinary NAG is useful in evaluating the changes in the condition of prostatitis and in diagnosing prostatitis, especially acute prostatitis.

Objective:To investigate the clinical efficacy of the QIANLIEHUICHUN capsule on the treatment of type Ⅲ chronic prostatitis.Methods:54 cases of chronic nonbacterial prostatitis/chronic pelvic pain syndrome(CPPS) were divided into type ⅢA 28 cases,ⅢB 26 cases.All cases were given to the QIANLIEHUICHUN capsule treatment(1.8 g/time,3 times a day,lasted for 3 months).The changes of the U.S.National Institutes of Health chronic prostatitis symptom index(NIH-CPSI) score were recorded before and after treatment.Also,examination of EPS also used as an index to compare the response of patients with type ⅢA and ⅢB chronic prostatitis.Results:The overall effective rate was 68.0%(17/25) in patients with type ⅢA,38.5%(10/26) in patients with type ⅢB.The statistical analysis showed that changes of the NIH-CPSI was much different between the two types of patients with chronic prostatitis(P0.05).Conclusion:QIANLIEHUICHUN capsule has different therapeutical effect on different type of chronic prostatitis.It is important to identify the type of chronic prostatitis for improving its therapeutical effect.

So far the etiology and pathogenesis of chronic prostatitis, particularly chronic pelvic pain syndrome (CPPS), remain to be elucidated and there is no unified recognition in the treatment of this disease. This article discusses the thoughts and methods for the diagnosis and treatment of chronic prostatitis by combination of TCM and Western medicine systematically.
