The therapeutic effect of aminophylline in children having severe asthmatic attacks with a special emphasis on an appropriate dose, adequate administration and measurement of serum aminophylline concentrations is presented. The study group consisted of 24 patients with severe asthmatic attacks. The patients were assigned to three groups according to the dosage of aminophylline administered. Patients in Group 1 received a dose of 7 mg/kg body weight, those in Group 2 5 mg/kg body weight and those in Group 3 (previously treated with theophylline) 4 mg/kg body weight. For each patient, clinical and laboratory parameters were followed. The results demonstrated that adequate therapeutic response and optimal serum theophylline concentrations were achieved only in Group 1. Patients in which lower doses of aminophylline were employed (Group 2 and Group 3) showed neither the adequate therapeutic response not did achieve the adequate serum theophylline concentrations, and administration of aminophylline had to be repeated. In the majority of patients, mild hyperglicemia was noticed. The mechanism and the sequence of events how it occurred was not determined. In conclusion, this data show that aminophylline, if administered in an adequate dose, is a drug of choice in the treatment of children with severe asthmatic attacks.

Arrhythmogenic effects of aminophylline (theophylline ethylenediamine) during halothane anesthesia have been reported but have not been related to serum theophylline levels. This study was designed to determine the arrhythmogenicity of therapeutic and toxic serum theophylline levels during halothane anesthesia. The study consisted of three parts. In part 1 (induction) six dogs were anesthetized for 15 minutes with 1% halothane in air and then given intravenous aminophylline, 50 mg/kg. In part 2 (maintenance) eight dogs were anesthetized for 2 hours with 1% halothane and then given intravenous aminophylline, 10 mg/kg. In part 3, after four additional hours of steady-state 1% halothane anesthesia, additional intravenous aminophylline, 25 mg/kg, was given to these eight animals. Three of six dogs in part 1 had arrhythmias following aminophylline, with serum theophylline levels ranging from 48 to 66 mg/L. No dog in part 2 had arthythmias following the 10 mg/kg dose of aminophylline, with serum theophylline levels of 14 to 23 mg/L. Six of eight dogs in part 3 had arrhythmias shortly after aminophylline, 25 mg/kg, with serum theophylline levels of 36 to 72 mg/L. Aminophylline administration after prolonger 1% halothane anesthesia appears free from arrhythmogenic effects if serum theophylline levels remain near the therapeutic range (10 to 20 mg/L). Aminophylline administration resulting in high serum theophylline levels (above 36 mg/L) causes ventricular arrhythmias when aminophylline is given during induction or maintenance of 1% halothane anesthesia. Arrhythmias usually (89%) begin with 5 minutes of aminophylline administration, and these arrhythmias always resolve spontaneously within 2 minutes of onset.

Intravenous aminophylline is widely used for the treatment of acute asthma and anecdotes about its apparent effectiveness abound. Early studies demonstrated that aminophylline is an inefficient bronchodilator compared with adrenergic agonists given by injection or inhalation. This paper presents the results of a review of randomized controlled trials examining the effect of adding aminophylline to standard modern therapy with a nebulized β2 agonist and systemic corticosteroids in acute asthma in adults. Six of the seven trials, comprising 343 patients, failed to demonstrate any beneficial effect of aminophylline. Furthermore, minor toxicity was common. There is presently no evidence to support the use of aminophylline in addition to standard therapy for acute asthma in adults.

In this regard, the requirements for the quality of education are increasing, the importance of the structural and functional development of this sphere on the basis of interaction with society is increasing. Objective To evaluate the appliability of curing cough,other sort of asthma with aminophylline.Methods 35 patients were selected randomly to receive either aminophylline or non-aminophylline.We observed the degree of cough and used ridit analysis.Results In aminophylline group the degree of cough was less in treated than in non-treated.The degree of cough was less in aminophylline group than in the control.Conclusion Cough is another sort of asthma and can be cured by aminophylline.

Objective To monitor the blood concentration of aminophylline in order to use it reasonably.Methods According to the ADR reports of aminophylline,the relation of blood concentration and therapeutic effect of aminophylline were analyzed.Results Blood concentration of 9 patients using aminophylline combined with Quinolone antibacterial or H2 receptor blockers,anti-arrhythmic drugs was higher than 20 μg/mL(effective concentration),then the ADR occured.Conclusion Blood concentration of aminophylline should be monitored when clinic using.The individual medication should be guaranteed according to patients′ physiology,pathology,pharmaco-toxicology,interaction and therapeutic effect.

Objective: To observe the correlation between the serum concentration of aminophylline and the clinical effect on pediatric patients with asthma, and to study the factors that affect the serum concentration of aminophylline. Meth ods: The data of 43 pediatric patients with asthma whose serum concentration of aminophylline were determined were assessed retrospectively. Results: The averag e serum concentration of aminophylline was 12.89±5.19 mg/L. 86.0% of the cases had the effective serum concentration of aminophylline (5~20 mg/L). None exceede d 20 mg/L. Combined-usage of drugs always effect the serum concentration of amin ophylline. Conclusions: There is great individual difference in the serum concen tration of aminophylline. It is important to monitor the serum concentration of aminophylline in order to assure the clinical effect.

Objective To give some information for rational adminstration of aminophylline for elder patients in clinic. Methods We analyzed the therapeutic drug monitoring data of aminophylline in 47 elder patients. Results 16 patients kept their blood aminophylline concentration among 10 μg/mL~20 μg/mL, accounting for 34.0% of total patients observed. Other 16 patients whose blood drug concentration was below 10 μg/mL, accounting for 34.0 %. Blood drug concentration of 15 cases was more than 20 μg/mL, accounting for 31.9 %. Conclusions The administration of aminophylline for elder patients must be based on the data of drug concentration in blood, the other factors must be also considered.

Theophylline/aminophylline use for asthma and chronic obstructive pulmonary disease has declined over time, as new and safer therapies developed. However, theophylline/aminophylline overdose can occur. Hereby it is described an unusual case of severe aminophylline intoxication due to mesodermic injections treated with CVVHDF session.
