Current status of laparoscopic surgery usage in Türkiye: A middle-income country

Abstract:
This study aimed to determine the usage status of laparoscopic procedures in general surgical practice in Türkiye, which is a sample of middle-income countries.The questionnaire was sent to general surgeons, gastrointestinal surgeons, and surgical oncologists who have completed their residency training and are actively working in university, public or private hospitals. Demographic data, laparoscopy training and the period of education, the rate of laparoscopy use, the type and volume of laparoscopic surgical procedures, their views on the advantages and disadvantages of laparoscopic surgery, and the reasons for preferring laparoscopy were determined with a 30-item questionnaire.Two hundred and forty-four questionnaires from 55 different cities of Türkiye were evaluated. The responders were mainly males, younger surgeons (F/M= 11.1/88.9 % and 30-39 y/o), and graduated from the university hospital residence program (56.6%). Laparoscopic training was frequently taken during residency (77.5%) in the younger age group, while the elderly participants mostly received additional training after specialization (91.7%). Laparoscopic surgery was mostly not available in public hospitals for advanced procedures (p <0.0001) but was available for cholecystectomy and appendectomy operations (p= NS). However, participants working in university hospitals mostly stated that the laparoscopic approach was the first choice for advanced procedures.The results of this study showed that the surgeons working in MICs spent strong effort to use laparoscopy in daily practice, especially in university and high-volume hospitals. However, inappropriate education, cost of laparoscopic equipment, healthcare policies, and some cultural and social barriers might have negatively impacted the widespread use of laparoscopic surgery and its usage in daily practice in MICs such as Türkiye.The surveillance of residence as compulsory measure confronts with lots of difficulties.The academic community has criticized for a long time,some scholars give suggestions to abrogate the surveillance of residence even.Considering of the immanent value of the system,the legislation should reserve the system and divide the surveillance of residence into segments such as the restriction of residence and a new form of the restrict,which should be set conditions newly.Simultaneously,the mechanism of the execution should be ameliorated for improving the applicability of the surveillance of residencec as ompulsory measure.

Objective To assess the practical value of laparoscopic cholecystectomy combined with other laparoscopic surgery in clinic. Methods 397 laparoscopic cholecystectomy combined with other laparoscopic surgery performed from 2000 to now were analyzed. Results All laparoscopic procedures were successfully completed.No one needed to be convened to laparotomy and had postoperative complications. Postoperative stay was similar to that of patients under laparoscopic cholecystectomy in the same time. Conclusion laparoscopic cholecystectomy combined with other laparoscopic surgery can be performed conveniently.It offers significant benefits to patients such as decreased postoperative pain,reduced trauma to the abdominal wall and is worthy of wide usage.

This paper deals with the vicissitudes of the Samurai Edo Residence in the early modern era by Hosokawa Archives. The contents are as follows. 1. Tatsunoguchi main-residence. 2. Hosokawa Sansai's residence. 3. Shiba secondary-residence. And in short, Hosokawa Edo residence was only Tatsunoguchi residence in 1622, and three residence existed in the site of this residence. So residence for the formal Shogunate visit could not be made. The residence for the formal Shogunate visit can be traced to the time of Kanei period. The center of Edo residences changed secondary-residence which was broad site from main residence which was narrow site in Kanei period. Also Sukiya residence can be traced time of Genna period.

Background: Laparoscopic cholecystectomy is getting popularity in developing countries like India.Conversion from laparoscopic cholecystectomy to open is also becoming common.This study evaluates the causes and rate of conversion and establishes the efficacy and safety of the procedure.Methods: This is a retrospective study, conducted in department of General Surgery, at NRI medical college and general hospital from January 2016 to December 2016.Patients of more than 20 years with symptomatic gall stones were included in the study.Patients with dilated CBD, choledocholithiasis, carcinoma gall bladder were excluded.Results: A total of 85 patients with 33 males, 52 females, 5 patients with conversion were recorded.Most common cause of conversion was adhesions followed by obscured anatomy at CALOT'S triangle.Conclusion: Most common cause of conversion was dense adhesions followed by obscured anatomy at CALOT'S TRIANGLE.Learning curve also contributes to conversion rates.

Domicile and habitual residence,two of the most important constituents of Chinese Residence system,have some defects in theory and practice. First,The General Principles of Civil Law defines the domicile as a registered permanent residence and the change of the residence must be based on the registration of household,resulting in the fact those who leave their registered permanent residence for a long time without registration or who come from foreign countries are unable to have a legal domicile. Second,how to identify habitual residence,which used to be connected with one's temporary residence permit,with the development of residence permit,should definitely be modified. Therefore,in order to improve Chinese residence system,we should start from the correct definitions of domicile and habitual residence and their constitutive requirements,and with there form of household registration system and the spread of residence permit,we should also explore what roles they will play in the recognition of subject and object constitutive requirements.

The usefulness of the U.S. decennial census depends critically on the accuracy with which individual people are counted in specific housing units, at precise geographic locations. The 2000 and other recent censuses have relied on a set of residence rules to craft instructions on the census questionnaire in order to guide respondents to identify their correct usual residence. Determining the proper place to count such groups as college students, prisoners, and military personnel has always been complicated and controversial; major societal trends such as placement of children in shared custody arrangements and the prevalence of snowbird and sunbird populations who regularly move to favorable climates further make it difficult to specify ties to one household and one place.
