    Abstract:
    Background: Psoriasis can present different phenotypes and could affect diverse body areas. In contrast to the high effectiveness of biological drugs in the treatment of trunk and extremities plaque psoriasis, in palmoplantar phenotypes and in plaque scalp psoriasis, these same drugs usually have reduced efficacy. Anti-TNF drugs could induce the appearance of palmoplantar pustulosis (PPP) in patients with other inflammatory diseases. The objective of this study is to identify if there are DNA Copy Number Variations (CNVs) associated with these different clinical phenotypes, which could justify the differences found in clinical practice. Moreover, we intend to elucidate if anti-TNF-induced PPP has a similar genetic background to idiopathic PPP. Methods: Skin samples were collected from 39 patients with different patterns of psoriasis and six patients with anti-TNF-induced PPP. The CNVs were obtained from methylation array data (Illumina Infinium Human Methylation) using the conumee R package. Results: No significant CNVs were found between the different phenotypes and the locations of psoriasis compared. Nevertheless, we found two significant bins harboring five different genes associated with anti-TNF-induced PPP in patients with a different background other than psoriasis. Conclusions: Our results may help to predict which patients could develop anti-TNF-induced PPP.

    Seperti pada dewasa, teknik regional anestesi pada pediatrik kini makin popular digunakan oleh ahli anestesikarena keuntungannya. Namun demikian selalu ada risiko dan kemungkinan timbulnya komplikasi dari setiap tindakan yang dilakukan, termasuk tindakan anestesi regional pada pediatrik. Insidensi komplikasi anestesi regional pada pediatrik tidak banyak, dan kalaupun terjadi komplikasi adalah minor. Komplikasi bisa diakibatkan dari identifikasi ruang saraf, alat, obat, teknis tindakan anestesi regionalnya dan komplikasi lainnya.Walaupun tidak banyak kejadian komplikasi regional anestesi yang dilaporkan pada pediatrik, dan bukanlah komplikasi yang fatal, teknik regional anestesi pada pediatrik harus dilakukan dengan lebih hatihati, pertimbangan risiko dan keuntungannya untuk menghindari terjadinya komplikasi, terlebih karena kebanyakan komplikasi dapat dihindari dengan mempelajari teknik yang benar, menggunakan peralatan yang sesuai, dan sangat menerapkan prinsip keamanan pada pasien dengan baik.

    SUMMARY There is a wide range of morphological variation of psoriasis which is due to structural and functional regional differences of the skin surface; to the frequent combination of psoriasis with other dermatoses; to the action of various triggering factors and to the changes produced by treatment of the dermatosis. For the understanding and management of the resulting complex psoriasiform dermatoses, an appreciation of the existence of these factors is essential. The patch of psoriasis offers resistance to impetiginization and to super‐infection by dermatophytes. The coexistence of eczema and psoriasis in the same lesion is common and may pass unsuspected. The combination of atopic dermatitis and psoriasis in the same individual is rarely seen. There is good reason to believe that patients affected by keratotic (Norwegian) scabies are psoriatics. Pityriasis rubra pilairs is not rarely a complex dermatosis in a psoriatic patient. Some forms of palmo‐plantar and linear keratoses are of hidden psoriatic nature. Due to modern methods of therapy (corticosteroids and antimetabolites) the number of the complex, atypical forms of psoriasis is on the increase.
    Pityriasis rubra pilaris
    Topical methoxsalen photochemotherapy has been assessed in 22 patients suffering from recalcitrant palmoplantar pustulosis or psoriasis predominantly involving the hands and feet. Although 20 out of 22 patients improved and good results were obtained in exactly half of those treated, only 4 patients were classified as being "clear" or "minimally involved" at the end of 12 weeks. Two patients have shown no improvement at all. Minor local side effects were relatively common and included symptomatic erythema, blistering and local pigmentation. The histological findings in post PUVA treated skin are discussed and the pros and cons of topical photochemotherapy in this group of diseases is reviewed.

    Pyoderma gangrenosum (PG) is a rare autoinflammatory skin disorder, which is characterised by rapidly developing and tender cutaneous ulcers. The treatment of PG is challenging. Palmoplantar pustulosis (PPP) is also an autoinflammatory dermatosis with sterile pustules on the palms and/or the soles. We demonstrated a 68-year-old patient with coexisting autoinflammatory diseases including PG, 1-year history of plaque psoriasis and PPP, recovered after treatment with adalimumab. We also reviewed published reports of PG-associated autoinflammatory syndromes with adalimumab.

    Irritant and allergic contact dermatitis occuring in a patient with psoriasis may clinically appear as a dermatitis, psoriasis (eg, pustular psoriasis) or as an ill-defined subgroup sometimes designated eczematous psoriasis. This subgroup of psoriasis has a clinically heterogenic picture, but retains the chronicity of classical psoriasis. Coexistence of psoriasis and contact dermatitis has prognostic and therapeutic implications for the patient. Allergic contact dermatitis in psoriatics may present itself clinically as psoriasis, and the diagnosis can therefore be missed unless appropriate patch testing is performed. The present study examines the clinical presentation of patients with recognized eczematous psoriasis. It is concluded that the definition of this entity primarily rests on the patient history and response to treatment. The morphology and distribution of the skin lesions are of lesser importance. A missed contact allergy in a psoriasis patient may be one of several reasons why the psoriasis spreads or resists adequate treatment.
