Article AbstractLetter to the EditorSir: I read with interest the recent article by Nafziger and colleagues1 (March 1999 issue) describing the incidence of sexual dysfunction in 20 healthy volunteers who were administered fluvoxamine, 150 mg/day, for 2 to 4 weeks as part of a clinical pharmacology trial. The authors conclude that healthy normal adults (presumably free of underlying psychopathology) had a 35% rate of reporting symptoms of sexual impairment during fluvoxamine therapy as based on data from a structured adverse event questionnaire. The implication is that this result would also be true in patients who are taking fluvoxamine for obsessive-compulsive disorder or depression, and that the rate for fluvoxamine would then be comparable to the rate reported for other agents in the selective serotonin reuptake inhibitor (SSRI) class.

Fluvoxamine, a selective serotonin reuptake inhibitor, was studied extensively in 34587 predominantly depressed patients in 66 studies conducted world-wide. These studies were largely uncontrolled trials representing the use of fluvoxamine by psychiatric and general practice physicians in everyday conditions. The safety data were analyzed according to standardized medical review and data management policies. Approximately 70% of the fluvoxamine population were female and 44% were aged 31–51 years. The modal total daily dose was 100 mg. Safety findings revealed a pharmacological adverse event profile similar to that seen with other serotonin reuptake inhibitors. Nausea was found to be the only common symptom, with an incidence rate of 16%. Approximately 2% of the fluvoxamine population reported at least one serious adverse event (per FDA criteria). Overall suicidality rates of fluvoxamine were found to be low (0.7%). No cases of zimelidine syndrome, bleeding syndrome or Guillain-Barre syndrome were identified. Overall, fluvoxamine was found to be safe and well tolerated suggesting a favorable alternative in the treatment of depression.

A placebo-controlled, double-blind study of 63 inpatients with major affective disorder was performed to compare the safety and efficacy of fluvoxamine and imipramine. Results indicate that fluvoxamine and imipramine are superior to placebo and demonstrate a trend toward superiority of fluvoxamine over imipramine. Fluvoxamine was generally well tolerated in most patients.

Abstract Interim directorships are an important, but little investigated, aspect of museum management. This study of 52 interim directorships in the United States provides baseline parameters of the experience: how long it lasts, who is selected to serve, and how the experience affects their careers. One-fifth of the interim appointees assume the directorship in their museums, including nearly one-half of those who actively sought the position and one-fifth leave the institution shortly after their interim experience. Interim directorships appear to be an indicator of future museum appointments, as one-third of the interim appointees went on to regular directorship appointments later in their careers. This paper argues that the appointment of internal candidates as interim directors significantly alters the social ecology of the museum. It also discusses the ethical issues represented by the appointment of interim directors and suggests areas for further research.
