    Objective: Laparoscopic surgery is cosmetically superior to laparotomy. Recently, single-site laparoscopic surgery (SSL) is being performed in clinical practice because it requires a small incision. However, owing to the technical difficulty in performing this procedure, it is performed in gynecology departments only at a few hospitals. Forceps interfere with the charge-coupled device camera used during SSL (single-site laparoscopic surgery) owing to the restricted surgical field available in such cases. The SSL operation is more difficult than multi-port laparoscopic surgery owing to technical obstacles. We report the efficacy of the scope holder system (ViKY EP system™, Endocontrol, La Tronche, France) used during SSL in such technically difficult cases.

    We modify traditional trans-umbilical single incision laparoscopic surgery to trans- right lower abdomen single incision laparoscopic surgery. This kind of method can improve the vision and operative plane in a straight line and the chopstick effect between instruments and reduce the operative difficulty and shorten the operative time and increase the operative quality. It is easier to conduct and promote the single incision laparoscopic surgery.     During laparoscopic surgery, as in open surgery, exposure is critical. We describe our surgical technique that provides intra-abdominal multiple organ retraction in order to obtain a stable surgical field and adequate exposure.The Flex Arm™ surgical holder (Mediflex Surgical Products, Islandia, NY) is an adjustable, spring-loaded articulating instrument holder. The device offers the flexibility of the attachment of multiple arms (up to three) on the same rod, which can rotate 360° to aid in positioning. This device permits multiple laparoscopic instruments to be held simultaneously. We used this device for multiple organ retraction during urological laparoscopic surgery (retroperitoneoscopic radical prostatectomy, n=5; laparoscopic radical nephrectomy, n=3; laparoscopic partial nephrectomy, n=2; laparoscopic pyeloplasty, n=2).We achieved significant improvements in the efficiency of retraction of the liver, kidney, colon, and prostate during urological laparoscopic surgery. Our method maintains a constant surgical field through the proper retractor position and the proper amount of isometric force for optimal exposure.With this device, instrument migration due to muscle fatigue and organ injury as a result of excessive force are all substantially reduced. We believe that this technique may be helpful not only in urological laparoscopic surgery, but also in laparoscopic approaches in other departments.

    A Laparoscopic surgery has some advantages such as minimum invasion and earlier recovery of patients. On the other hand, the laparoscopic surgery requires more skill of surgeons compared with the open abdominal surgery. Therefore, to train the skill of laparoscopic surgery, many simulators of the laparoscopic surgery have been developed. In those simulators, to perform effective training, the proper evaluation is necessary. To evaluate the skill of laparoscopic surgery, at first, it is necessary to discuss the difference between an experienced surgeon and a beginner in forceps manipulation. In this paper, to clarify the difference between an experienced surgeon and a beginner, experiments with a sensor forceps are carried out. In the experiments, the motion of suturation has been performed. The experimental results show that the orientation of the forceps, shaft forces and so on are different between an experienced surgeon and a beginner.

    We developed forceps manipulator for laparoscopic surgery. Some problems in laparoscopic surgery are mainly due to the poor D.O.F. (degree of freedom) of controlling forceps and laparoscope. The forceps manipulator we newly developed has 2 additional D.O.F. of bending. This manipulator can solve the problems in laparoscopic surgery, and establish new laparoscopic surgery with higher technology and safety.

    Abstract Hybrid total laparoscopic hysterectomy combines conventional laparoscopic surgery and robot-assisted devices. During hybrid total laparoscopic hysterectomy, the camera and assistant forceps are operated by a robotic device, whereas the surgeon performs human procedures using a laparoscope. This enables surgery with a completely fixed field of view, thus significantly reducing errors in forceps grasping and needle misalignment. We created and implemented a new use for the Hugo™ robot-assisted surgery system, which consists of independent arms. This new technique involving new equipment can improve surgeon training and performance. We will continue to develop new techniques that can further improve surgical performance.
