Abstract:
We conducted an analysis of indirect costs alongside the LY.12 randomized trial in patients with relapsed or refractory (R/R) aggressive non-Hodgkin lymphoma (NHL). Lost productivity data for Canadian patients and caregivers in the trial were collected at baseline and with each chemotherapy cycle pre-transplant, using an adapted Lost Productivity questionnaire. Mean per patient indirect costs were CAD 2999 for patients in the GDP arm and CAD 3400 in the DHAP arm. A substantial majority was not working or had to reduce their workload during this treatment time. Salvage chemotherapy for R/R aggressive NHL is associated with significant indirect costs to patients and their caregivers.Keywords:

Background: Optimal pharmaceutical regimen for advanced thymic epithelial tumors (TETs) remains controversial when first-line chemotherapy fails. This retrospective study aims to evaluate the efficacy and safety of anlotinib treatment for patients with relapsed and refractory TETs. Methods: Patients with progression disease after failure of platinum-based chemotherapy were enrolled in this study. Anlotinib was orally taken once a day at an initial dose of 12 mg (10 mg when body weight <60 kg). The cycle was repeated every 3 weeks (2 weeks of treatment followed by 1 week rest). There are 3 dose levels (12, 10 and 8 mg), and dose may be reduced to a lower level when grade 3 toxicity occurred. Objective response rate (ORR) and progression-free survival (PFS) were recorded as primary end points, and they were analyzed separately in thymoma (THY) and thymic carcinoma (TC) cohorts. Meanwhile, toxicities were assessed according to CTCAE (version 5.0). Results: There were 50 patients enrolled in this study from October 2018 to June 2021 at a median age of 50 (range, 23–79) years old. Patients with THY and TC were 33 (66%) and 17 (34%) respectively. The ORR in THY and TC patients were 33% (11/33) and 41% (7/17), respectively. The median PFS (mPFS) were 7 (95% CI: 5.9–10.2) months in THY patients and 6 (95% CI: 4.6–9.3) months in TC group. Eleven patients experienced dose reduction due to toxicities, among whom, 8 patients discontinued treatment even after dose reduction. Six patients with THY showed myasthenia gravis (MG) deterioration during treatment, and 2 of them died of MG crisis. Conclusions: Anlotinib is active in patients with advanced TETs refractory to routine chemotherapy. Prescription of Anlotinib to patients with MG should be made cautiously.

The aim of this study was to compare the workload between tasks undertaken manually and those with the assistance of mechanical devices. The results indicate that the workload during the tasks assisted by mechanisation was usually very low or relatively low. Statistical analysis showed the impact of the way of the work performance on the workload. However, the results also indicate that the workload is not evenly distributed: the effort relating to the working tasks is larger in relation to the work of women than men. The need for mechanisation of the tasks performed by women is higher than that for the mechanisation of tasks performed by men. The type of work performance (manual, or technically assisted) did not clearly show significant differences in the workload. This surprising result indicates that the technical devices used on many farms are still far from perfect. It is concluded that assisting work with mechanical devices reduces the workload of farmers, but the way of work performance (manual or mechanical) was not statistically significant.

Forty one patients with refractory malignant lymphoma were treated with a combination of VP-16, ACM, BH-AC, MTX and PDN as a salvage chemotherapy. These patients were either resistant to frontline therapy or refractory in their relapses. Three patients (7%) achieved a complete remission and 14 patients (34%) attained a partial remission. An overall response rate was 41%. Major toxicities were myelosuppression, nausea, and vomiting. However, they were well tolerated. This regimen has been effective in the treatment for the patients with refractory malignant lymphoma.

Four patients with refractory or recurrent germinal testicular cancers were treated with the combination of VM-26 and cis-platinum, and two of them also in combination with radiation therapy. Two complete responses and two partial responses were observed. One partial response was rendered no evidence of disease by sequential radiation therapy. Three of the patients remained to have no evidence of disease from 16, 21 and 22 months. Toxicity was tolerable. VM-26 is an active drug in germinal testicular cancers and plays an important role in salvage therapy, for refractory or recurrent cases.

Driver mental workload is an often studied concept, however less attention is given to the question of transitions in driver workload. Fourteen adult drivers completed a simulated driving task following a navigation system which would fail at certain intervals. Subjective measures of driver workload were taken and demonstrate that the recovery from a driving stressor is asymmetrical and time-delayed. Drivers' subjective ratings of workload remained high after the stressors were removed. Findings and implications are discussed.

Abstract In a real work situation the operative strategy choosen by subjects depends on three variables, the characteristics of the operator (training, motivation, age, health, etc.); the characteristics of the task, i.e. the level of task requirements; and the level of workload, which itself results from the operative strategy. Ergonomista try often to measure the workload, but do not pay enough attention to the regulating effects exerted by the workload, through feed-back, on the strategy used by operators. This paper summarizes an experimental study carried out in a ATCC, showing how air traffic controllers modify their operative methods when workload increases.

This paper reviews selected literature on workload, noting critical issues and efforts directed at a more equitable estimation, assignment and evaluation of faculty workload. It underlines the need to consider all factors and aspects of faculty work in assigning and evaluating workload. A flexible approach to evaluation is advocated since faculty have different strengths and interests and departments have different demands and requirements. It is also suggested that flexible formulas may be applied in evaluating faculty at the College of The Bahamas and that they need not be measured in an identical or uniform manner.
