Abstract:
Sarcoidosis is an inflammatory disorder of unknown etiology. The main characteristics of sarcoidosis are non-caseating granulomas which can be formed in affected organs. Eye and its adnexal tissues can also be involved. In the early stages of sarcoidosis, the clinical picture may be mild and not always specific. Although the early signs of sarcoidosis are lung associated, we present a clinical case with diffuse enlargement of the lacrimal glands as a first sign of systemic sarcoidosis. A 21-years-old woman presented to the ophthalmological center with a 2-weeks history of having swollen, painful upper eyelids. Primary eye examination revealed palpable tender masses in the locations of the lacrimal glands. Characteristic for sarcoidosis X-ray findings and blood tests demanded lacrimal gland and lung biopsy for diagnosis verification. In spite sarcoidosis of lacrimal glands is a rare condition, it has to be taken into consideration in the differential diagnosis of lacrimal gland masses.Keywords:

Sarcoidosis is an idiopathic granulomatous disease with many cutaneous manifestations and a known predilection for scars and areas of previous trauma. We report the first case of systemic sarcoidosis presenting in only the black dye of a tattoo. The skin manifestations of sarcoidosis and the histologic differential diagnosis of granulomas are reviewed.

Three hundred and thirty-two patients with pulmonary sarcoidosis were examined in 2005 to 2008. Atypical disposition of chest X-ray changes was found in 32 patients. In these cases physicians encounter the greatest difficulties in the differential diagnosis of sarcoidosis. The patients with pulmonary sarcoidosis were found to have 2 clinical and X-ray variants according to the location of X-ray changes. Four clinical histories of difficult cases of the differential diagnosis of pulmonary sarcoidosis are given.

Sarcoidosis is a systemic disease that can be suspected based on cutaneous findings and confirmed using diagnostic testing such as biopsy and laboratory or radiographic studies. We report the case of a 66-year-old black woman with ichthyosiform sarcoidosis (IS) on the lower extremities. Ichthyosiform sarcoidosis is a rare variant of cutaneous sarcoidosis that is frequently associated with further internal involvement. We review this uncommon presentation of sarcoidosis along with other cases reported in the literature to determine the organ systems most commonly affected.

The paper considers the results of histological and bacterioscopic examinations of biopsy specimens from the lung and mediastinal lymph nodes of 120 patients with an unclear diagnosis of tuberculosis and sarcoidosis and with problem of their differential diagnosis. One hundred and five of these patients were microscopically diagnosed as having either tuberculosis or sarcoidosis. Additional examination of differently stained sections and that using immunohistochemical M. tuberculosis detection were conducted in 15 cases, which could diagnose tuberculosis and sarcoidosis in 7 and 8 patients, respectively. Bacterioscopic examination, in terms of extreme variability the phenotypic properties of mycobacteria, is of decisive importance in the differential diagnosis of tuberculosis and sarcoidosis.
