From unsatisfactory results in fractures in the hand treated with K-wires or other osteosynthesis we felt the need to develop new material for osteosynthesis. Our goal was to simplify the operative procedure, to enable exact anatomical reposition to avoid or reduce splinting in order to prevent adhesions and secondary operations. Further more to be able to reduce and stabilise small fragments. After more than 4 years of experience with our material in more than 200 osteosynthesis we can conclude, that our goal has been accomplished to a great deal. We would like to present this procedure.

The lower jaw of 60 dogs was fractured and effects of wire and three variants of chemical osteosynthesis have been followed up concerning validity, course and rapidity of osteogenesis. A wire of 0.4 mm in the diameter was applied in wire osteosynthesis and autopolymerizing acryalte ("Simplex" Galenica) in chemical osteosynthesis with briding of the fracture line interfragmentarily or mixed with bone scrapings. The pathohistological examinations performed 7, 14, 21, 60 and 90 days after the intervention and osteosynthesis employment have shown that there is no difference in validity, course and rapidity of osteogenesis in fractures of the lower jaw under conditions of wire and pure chemical osteosynthesis. Since there is no histochemical obstacles and taking into account advantages of chemical over wire osteosynthesis it should be recommended in fractures of the lower jaw.

The results of surgical treatment of 235 patients with the fractures of trochanterian area have been analyzed. In 180 of them internal osteosynthesis was performed (angular fixator AO 124 and fixator P.Y.Bakicharov 32, DHS system 18, polifascicular osteosynthesis 6) and on 54 perosseous osteosynthesis. The study of surgical results showed that in the surgical treatment of the patients perosseous osteosynthesis is preferable as it allows to get optimal results and reduce the period of hospital treatment. The results of surgical treatment of 166 patients with the trochanter fractures of the femur have been analyzed at the Department of Traumatology and Orthopedy of Grodno State Medical University. 112 patients were treated by internal osteosynthesis and 54 – by perosseous osteosynthesis. Positive results have been obtained: in 92,8% ofcases after internal osteosynthesis and 100 % ofcases after perosseous osteosynthesis.

Progressive direction in the development of inner osteosynthesis is the preservation of blood circulation in fracture zone, use of minimum operative approach and closed reposition. Authors have summarized the known principles of biological osteosynthesis and gave their opinion on problem. The experience in use of lightly invasive technique of osteosynthesis and AO implants are analyzed basing on 335 patients with fractures of various localization. Possibilities of active postoperative rehabilitation were shown. Original techniques of lightly invasive transcutaneous osteosynthesis with primary and secondary strained pins in periarticular fractures are given. Special attention is paid to the necessity of drug correction of bone quality in the postoperative period.

The article reported the results of osteosynthesis in 65 patients with pathological fractures on the background of metastatic bone lesions. We have applied such methods of osteosynthesis: perosseous osteosynthesis with external fixation device in 33 (50.8 %) patients, reinforced osteosynthesis in 32 (49.2 %). Osteosynthesis has been applied in comprehensive treatment using polychemotherapy, radiation therapy, hormone therapy, immunotherapy and bisphosphonates. Due to the use of osteosynthesis techniques in complex treatment regimens, complications occurred in 2 (3.1 %) patients, recurrences of metastatic tumors in 3 (4.6 %) patients, new foci of metastatic lesions in bones and visceral organs during the treatment were detected in 35 ( 53.8 %) patients. The three-year overall survival rate of patients was 28.60 ± 1.32 %, five-year 14.40 ± 1.96 %. The use of osteosynthesis techniques in the comprehensive treatment in patients with metastatic lesions of long bones helps to restore the function and support ability of the limb, to reduce pain and therefore to improve the quality of life of these patients.

In the article are presented the results of surgical treatment of 176 patients with associated craniocerebral injury. To the primary osteosynthesis in the first post-hospital day have been subjected 75 patients and to the postponed osteosynthesis--101 patient. For osteosynthesis there have been applied the methods of stable-functional osteosynthesis by plates (57.3%), by means of the external fixation apparatus (19.8%), by rods (10.8%) and by other means. The experience of the primary osteosynthesis, including the experience of simultaneous operation on 2-3 segments with application of the atraumatic technique, indicates that the operation doesn't aggravate the severity of injury but ensures more quick rehabilitation of the patients with craniocerebral injury, guarantees the complete restoration of function during 1-2 post-operative months.
