Abstract:
Attempts to secure improved outcomes with persistent medically unexplained physical symptoms (MUS/PPS) are in their early stages of development and effective implementation in the United Kingdom remains inconsistent. There are scant systematically examined process and outcome studies in this field published in peer reviewed journals. Moreover, persistent sexual symptoms no longer appear in listed examples of MUS/PPS in the professional literature and consequently few studies can be found on 'what works' for improved outcomes with this group of sufferers.A systematic evaluation of an NHS Community Psychosexual Counselling Service delivered within an Integrated Contraception and Sexual Health Service for adults with persistent sexual dysfunction is summarised. The findings from the synthesis of evaluated research, professional guidance and a particular practice, at clinic cohort and embedded case levels, revealed common themes of engagement across the therapy divides when addressing “medically unexplained” physical symptoms. This supports future research in this area and calls for the inclusion of persistent sexual symptoms. The extracted themes from the therapy process may be used as tools to enhance skills of engagement with unresolved PPS in similar settings in which the individual seeks help to achieve improved outcomes.Keywords:

With the coming of age of individuals with autism spectrum disorder (ASD), it became clear that adolescence, and specifically psychosexual functioning, is particularly challenging for this population. The current thesis expands the knowledge that was acquired up until now on psychosexual functioning in adolescents with ASD by obtaining information from multiple informants in all domains of psychosexual functioning, comparing this with typically developing adolescents. Part 1 and 2 of this thesis showed that ASD traits are specifically linked to psychosexual difficulties, above and beyond other known influences (e.g. pubertal development). However, part 2 also shows that not all elements of psychosexual functioning are problematic in adolescents with ASD. Finally, part 3 showed that psychosexual knowledge of adolescents with ASD can improve with training, a finding substantiated by a more recent randomized controlled trial. Psychosexual functioning is without a doubt a topic relevant for individuals with ASD. It seems that particularly this group runs into difficulties in their psychosexual functioning during adolescence and support is needed to help them through the difficulties.

Psychosexual health of 308 children, aged 2-11 years, as well as that of their families, was studied. Deviations in psychosexual development were found in 34.6% of the children examined. The following types were detected: difficulties in formation of gender-determined behavior features--64.4%, precocious psychosexual development--13.7%, delayed psychosexual development--12.3%, obsessive masturbation--9.6%. Risk factors for deviant psychosexual development were found.

Psychosexual development is influenced by biological and psychosocial factors. Human beings show a great variability in psychosexual development both between and within gender-groups. However, there are relatively stable gender-related behaviors and self-perceptions, in which males and females differ distinctly. There is strong evidence that high concentrations of androgens lead to more male-typical behavior and that this also influences gender identity. Disorders of sex development (DSD) provide the opportunity to analyze the role of different factors on psychosexual development. We examined 166 children age 4 to 12 with DSD using instruments concerning gender role behavior, gender identity, and friendship. Results underline the hypothesis, that androgens play a decisive role in the masculinization of gender role behavior in children. There are also some relations between the experience of gender change and psychosexual outcomes which have to be discussed. Nevertheless, results indicated a high congruence between the children's gender identity and gender of rearing.

The author provides a literature review and developmental formulation, with the goal to assist clinicians working with medically ill adolescents with psychosexual issues. MEDLINE and PsychINFO database searches of English-language medical journal articles published between 1986 and 1997 for articles related to medical illness and psychosexual development in adolescence were done. The author found that little systematic research on the psychosexual implications of medical illnesses for adolescents has been undertaken, but existing studies suggest that psychosexual development is negatively affected by medical illness. A three-phase model of adolescent psychosexual development is presented, with specific psychosexual tasks associated with each phase. Impediments to progressing through adolescent psychosexual phases due to medical conditions are identified, and case examples are provided. The author concludes that clinicians working with adolescents with medical conditions should attend to the possibility of psychosexual impediments in these adolescents and use developmentally appropriate methods for assessing and treating these difficulties when they arise.

We studied psychosexual development in 28 adolescent males who had survived leukemia and compared it with that in 25 adolescent males who had survived other malignancies in childhood. A clinical semistructured psychiatric interview was used to form 8 criteria of psychosexual development. Sexual identity was also assessed using a psychological test. By every criterion, development was more disturbed in patients who had survived leukemia. Attitudes toward adult‐type genital sexuality, contact ability with the opposite sex, and the developmental level of sexuality were most significantly affected.
