Treatment of Gonorrhea in Auckland, New Zealand: Marked Variation in Guideline Compliance

To determine whether concurrent gonorrhea reactivates latent chlamydial infection, we studied 74 recurrent chlamydial infections and the effect of concurrent gonorrhea at the recurrent episode on the chlamydial serovar identified. Serotyping of 74 recurrent pairs of chlamydial isolates from patients attending a sexually transmitted diseases clinic indicated that 47.1% (16of 34) with gonorrhea at the time of recurrence harbored chlamydiae of the same serovar as at the initial infection, while only 22.5% (9 of 40) without gonorrhea had the same serovar (P = .03). The proportion of recurrences by the same serovar in the group without gonorrhea did not differ from the proportion predicted by a random exposure model (22.2% vs. 18.4%, P = .46), while the proportion in the gonorrhea group did (47.1% vs. 19.8%, P < .0001).The possibility of reinfection did not appear more likely in the group with gonorrhea than in the group without. These observations support the hypothesis that concurrent gonorrhea can reactivate latent chlamydial infection.

Gonorrhea is the leading cause of sexually transmitted infections (STIs) caused by Neisseria gonorrhoeae. Commercial sex workers are vulnerable to infection with STIs. The aim was to determine the prevalence of gonorrhea among commercial sex workers in Yogyakarta and the risk factors. The research design is cross-sectional. 43 commercial sex workers in Kota Yogyakarta had their cervical secretions taken for the diagnosis of gonorrhea. Demographic data and subject behavior were obtained through interviews. The results showed that the majority of respondents had a junior high school education, and 25.6% were infected with gonorrhea. The incidence of gonorrhea was not related to the behavior. Age is related to the incidence of gonorrhea. The prevalence of gonorrhea in Kota Yogyakarta is 25.6% and age is a risk factor for gonorrhea

The productivity of gonorrhea case-finding among persons epidemiologically associated with children infected with Neisseria gonorrhoeae was examined. During a 33-month period, 36 cases of gonococcal infection in children prompted the examination of 244 individuals who were identified as either sexual contacts, household associates, or nonhousehold associates. Overall, we found an infection rate of 18.4% among 244 persons associated with cases of gonococcal infection in children. Of 21 sexual contacts examined, ten (47.6%) had gonorrhea. Of 223 persons associated with the childhood cases of gonorrhea, 21 (16%) of household associates and 14 (15.2%) of nonhousehold associates were found to have gonorrhea. Associates aged greater than or equal to 30 years who were not named as sexual contacts had a risk of infection significantly lower than that of younger persons examined. These results emphasize that gonorrhea screening prompted by reports of cases in children should not be limited to those persons identified as sexual contacts or household members.

We describe a sexual network consisting of 1 nonbinary-gendered participant and 2 male and 4 female participants in Australia, 2018. Six of 7 participants had oropharyngeal gonorrhea in the absence of urogenital gonorrhea. This observation supports a new paradigm of gonorrhea transmission in which oropharyngeal gonorrhea can be transmitted through tongue kissing.

A prospective study was performed to examine the prevalence of pharyngeal gonorrhea in two urban female adolescent populations and to compare pharyngeal infection with a history of orogenital activity and concurrent genital gonorrhea. Group I was drawn from a children's hospital adolescent clinic and group II was drawn from a public health clinic for sexually transmitted diseases. None of the 240 adolescents in group I had a pharyngeal culture positive for Neisseria gonorrhoeae compared with 3.4% in group II. Only 2.5% of group I had genital gonorrhea, but 33% of group II had positive genital cultures. In only two of the 20 patients with pharyngeal gonococcal infection was the pharynx the only infected site. The addition of routine pharyngeal culturing for gonorrhea yielded only 1% additional gonorrhea cases. There was a significant relationship between concurrent genital and pharyngeal gonorrhea. These findings indicate that routine screening for pharyngeal gonorrhea is not productive in some adolescent populations. A more economic approach would be to use gonorrhea treatment that is effective against both genital and pharyngeal gonorrhea or to obtain pharyngeal cultures in those adolescents returning for test-of-cure cultures after antibiotic treatment for genital gonorrhea.

Neisseria gonorrhoeae infections are a major cause of pelvic inflammatory disease, infertility, and ectopic pregnancy in women and facilitate the transmission of human immunodeficiency virus (1). To characterize the epidemiology of gonorrhea in the United States, CDC examined national surveillance data on gonorrhea cases reported to CDC through state health departments in 1998 and surveyed selected states with increases and decreases in gonorrhea rates since 1996. This report summarizes the results of this analysis, which indicate that following a 13-year decline, the number of gonorrhea cases in 1998 increased by 9% compared with 1997. Although changes in gonorrhea screening and surveillance practices may have contributed to the higher reported rates, reports from states suggest that true increases in gonorrhea cases also occurred in some populations.

The research was conducted in 76 patients with gonorrhea aged 14 to 60 years. The diagnosis of gonorrhea was confirmed by clinical, epidemiological and laboratory methods. It has been studied what antibacterial drugs patients take to achieve good results. Based on the obtained results, the indicators were statistically processed. Which antibiotics are significantly more susceptible to gonorrhea have been studied and determined. In the acute, subacute, chronic type of gonorrhea on clinical course, the use of the antibiotic Cefakim + Azithromycin has shown reliable results in clinical course and laboratory analysis.

Objectives: To understand the prevalence trend and characteristics of gonorrhea epidemics in Xi'an,and to discover the prevalence pattern of gonorrhea with an aim to provide reference for future prevention.Methods: Analysis was made on the gonorrhea epidemic materials collected between 2005 and 2010 in Xi'an.Results: The epidemic peak appeared in 2005,and the prevalence rate kept steady during 2006~2010.No obvious concentration of occurrence was found in each month.The professions of the infected population mainly include workers,peasants,migrant workers,cadre staff,homemakers,the unemployed workers and students.The infected population are predominantly aged between 20 and 45,the sex ratio being 8∶12.Conclusion: The gonorrhea infected population are mainly young adults,with more males than females.Workers,peasants are mainly infected population,but the students take a greater proportion and their sexual health should be more concerned.
