Objective
To observe the clinical efficacy of silicone lacrimal drainage tube combined with retrograde stent tube placement for complex lacrimal duct obstruction.
Methods
Twenty-six eyes of 26 cases of complex lacrimal duct obstruction were observed, including 16 eyes with both lacrimal canaliculus and nasolacrimal duct obstruction, 7 eyes with lacrimal canaliculus stenosis and chronic dacryocystitis and 3 eyes with lacrimal canaliculus, common canaliculus and nasolacrimal duct obstruction. They underwent combined lacrimal duct operation. All patients were followed up for 3~12 months.Periodic irrigation of lacrimal passage after operation were conducted, and the tubes were extracted 3 months after surgery. Symptoms and lacrimal duct irrigation condition of patients was observed.
Results
All the 26 eyes were successfully operated without any complication during operation. Finally 21 eyes were cured (80.77%), 3 eyes improved (11.54%) and 2 eyes invalid (7.69%).
Conclusion
Combined antegrade and retrograde whole lacrimal duct intubation is reliable in treatment of complex obstruction of lacrimal ducts.

Congenital nasolacrimal duct obstruction is the blockage of the lacrimal drainage system. It occurs in approximately 5 to 20% of normal newborn infants. A history of tearing and mucous or mucopurulent discharge and recurrent pink eye in a young child should alert the physician to the presence of nasolacrimal duct obstruction. Usually this condition is diagnosed clinically, though there are some investigations for precise diagnosis. Treatment is supportive and non surgical in 90 to 95% of cases, only 5 to 10% patients need surgical intervention. Outcome is good with minimal complications.

Objective To investigate the curative effects of micro-trepan combined with silicone implantation treating obstruction of nasolacrimal duct under lacrimal duct endoscopy. Methods From February 2007 to November 2007,47 cases(62 eyes) with obstruction of nasolacrimal duct were selected. With micro-three channel lacrimal endoscopy, lacrimal mucosa and narrow site were dynamically observed;Micro-trepan was used to relief obstruction, and then the retrograde operation of silicone implantation was taken;Silicone duct was taken out 1 month to 3 months later;Lacrimal patency was observed under lacrimal endoscopy for 6 months following up.Results Fifty-nine eyes were cured(95.1%),2 eyes were improved(3.2%) and 1 eye was ineffective(1.6%) in all cases treated with micro-trepan combined with silicone implantation under lacrimal duct endoscopy. Conclusion Lacrimal duct endoscope can be used to observe tiny pathological changes in lacrimal duct.Micro-trepan can avoid the formation of false passage and damage to normal tissue under lacrimal duct endoscopy, which can directly operate at narrow or obstructed lacrimal duct, restore the physiological structure of nasolacrimal duct.Micro-trepan combined with sillcone implantation and observing under lacrimal duct endoscopy is an ideal method for treatment.
