Abstract:
To observe the efficacy and safety of felodipine in treating pulmonary heart disease with congestive heart failure (PHD-CHF). METHODS: Fifty patients (M 40, F 10; age 68 a± s 8 a) with PHD-CHF treated with felodipine 5 mg, po, qd for 7 d, in addition to conventional therapy (inhalation of oxygen and anti-infection therapy etc). Other 50 patients served as control (M 38, F 12; age 65 a ± 7 a) were treated with conventional therapy only. RESULTS: The total efficiency rate of felodipine group was 98% vs that of the control group 46% (P 0. 01 ). No serious adverse reactions were found in 2 groups. CONCLUSION: Felodipine in treating PHD-CHF was satisfactory.Keywords:

AIM: To observe the efficacy of diltiazem in the treatment of pulmonary heart disease with mild to moderate cardial insufficiency METHODS: Patients of cor pulmonale with mild to moderate cardiac insufficiency were divided into two groups. The conventional therapy was given to the control group of 39 patients (M 35, F 4; age 65 a± s 5 a), while the conventional therapy plus diltiazem was given to the treatment group of 38 patients (M 33, F 5; age 63 a±6 a) diltiazem 0.6 mg·kg -1 was infused intraveneously by micro pump for 2 h. The therapy couse of the two groups was 1 wk. RESULTS: The efficacy of the treatment group(efficacy rate:94 %) was better than that of the control one(efficacy rate: 80 % ). There was significant difference between two groups( P 0.05). CONCLUSION: diltriazem can be used in patients of pulmonary heart disease with cardiac in sufficiency. That is safe, effective, and with less adverse reactions.

Objective:To evaluate the therapeutic efficacy of valsartan accompanied with metoprolol in the patients with congestive heart failure(CHF).Methods: 96 patients with CHF were randomly divided into group A ( n =46) who had taken conventional treatment of digitalis and diuretics and group B ( n =50) who were given valsartan (40 to 160 mg·d -1 ) and metoprolol (12.5 to 100 mg·d -1 ) on the conventional therapy.The therapeutic period was 6 months. Results: After treatment,cardiac function (NYHA) improved; blood pressure,heart rate,cardiothoracic ratio,left ventricular diastolic ending diameter (LVDd) and left aurial diameter (LAd) all decreased; left ventricular ejectional fraction (LVEF) increased significantly( P 0.05 ) in group B comparing with those in group A. There were few side effects and patients' tolerance was perfect in Group B.Conclusion:The therapeutic efficacy of valsartan accompanied with metoprolol in the patients with congestive heart failure(CHF) is satisfactory. There are few side effects.

Objective To observe the effects of candesartan in treating patients with congestive heart failure(CHF).Method Forty CHF patients were divided into the treatment group(20 cases) and the control group(20 cases) randomly.Digitalis,diuretics and nitrates drugs were given to the control group.Besides these therapies,candesartan 4~16 mg/d orally for 8 weeks averaged was added to the treatment group.The changes of heart rate(HR),blood pressure(BP),the left ventricular ejection fraction(LVEF),the left ventricle end diastolic diameter(LVDD),the left ventricle fibers shortening(LVFS) and cardiac function were observed.Results The remarkable effectiveness rate and the total effectiveness rate of the treatment group were relative higher than the control group(P0.05).HR,BP and LVDD all decreased significantly(P0.05,P0.01),LVEF,LVFS and cardiac function increased(P0.05,P0.01).Irbesartan had fewer adverse effects and the patients′tolerance was perfect.Conclusion The effect of irbesartan in treating patients with congestive heart failure is satisfactory.It has fewer adverse effects.It appears to be a promising new therapy for patients with congestive heart failure.

Objective To observe the effect of carvedilor on congestive heart failure disease(CHF). Methods Ninety-two cases of patienets with chronic CHF were randomly divided into two groups: one group was received routine treatment (digitalis, diuretics, vasodilator, ACEI), and another group was received carvedilor (25 mg bid×90 d) as well as routine treatment. Results The treatment efficiency in carvedilor group and control group were respectively 91.3% and 65.2% with signifisent differnce between two groups (P0.01). No adverse reactions associated with carvedilor were observed. The improvements of HR, BP, LVEF, EDV, ESV after treatment in carvedilor group were significant (P0.01 or P0.05). Conclusion Carvedilor with routine therapy may be a safe way to treat CHF.

Objective:To study the effect of carvedilol on cardiac function in patients with chronic congestive heart failure(CHF).Methods:78 patients with CHF were randomly divided into treatment group(42 cases) and control group(36 cases).The control group used conventional treatment programs.The treatment group was treated with Carvedilol from low dose to target dose in addition to conventional therapy for 6 months.Results:Heart rate(HR)in Carvedilol group were significantly lower than those in control group(P0.01).The LV ejection fraction(LVEF), stroke volume(SV) and E/A were significantly higher than those in control group(P0.01).Conclusion:Carvedilol is effective and safety in treatment of CHF.

Objective To investigate the therapeutic effect of metoprolol on congestive heart failure (CHF).Methods 120 paitents with CHF were randomly divided into treatment group and control group.The patients in control group received routine treatment including angiotension converting enzyme inhibitors (ACEI),diuretic and digitora,ect.The treatment group was added with metoprolol 6.25 mg,twice a day,on the basis of the routine treatment,and increasing doseage gradually,with the maximum of 50 mg,twice a day.The changes of heart rate,blood pressure,cardiac function and ultracardiography were observed before and after treatment.Results The effective rate and total effective rate in treatment group were 76.9% and 90.0%,which were 55.0% and 77.5% in control group respectively.There was a significant difference between two groups (P0.05).After treatment there were significant differences in heart rate,improvement of cardiac function and UCG parameters (E/A,LVEF,LVSD,LVDD)between two groups (P0.01 or P0.05).Conclusion Metoprolol can increase the therapeutic effect on CHF.

Objective: To investigate the influence of perindopril combined with metoprlol on the cardiac function, heart rate, heart chest ratio and left ventricular ejection fraction (LVEF) in patients with congestive heart failure (CHF). Methods: Sixty patients with CHF were divided into two groups. The perindopril and metoprlol treatments were on the basis of conserved therapy in therapeutic group. Control group was treat with conserved therapy and perindopril. Both groups were treated for six weeks. Results: In the therapeutic group, the heart rate slowed, the heart chest ratio, blood pressure decrased and the LVEF increased after treatment (P0. 05). However in the control group, the above stated parameters were not significant change after treatment. Above stated parameters were significant difference between two groups (P0. 05). Conclusion: Perindopril combined with metoprlol can decrease heart rate, blood pressure and heart chest ratio, and raise LVEF in patients with CHF.

Objectives:To explore the efficacy and safety of treating chronic congestive heart failure(CHF) with carvedilol.Methods:A total of 180 patients with chronic CHF were randomly divided into study group and control group,90 in each.Carvedilol was gradually added to conventional therapy from initial dose to the target or maximum dose tolerated in the study group,while placebo was given in the same pattern as carvedilol in the control group.The course was 24 weeks.The heart rate,blood pressure,echocardiography were measured and heart function was classified with the New York Heart Association(NYHA) classification before and after treatment,as well as adverse effects were recorded.Results:Total efficacy of the study and control groups were 96.67% and 82.22%,respectively.After the treatment,the patients' heart rate and systolic blood pressure were decreased,exercise tolerance index was increased,left ventricular ejection fraction was improved,left ventricular diastolic dimension and left ventricular systolic dimension were decreased,and the expression of noradrenalin,plasma renin activity and angiotensin Ⅱ were decreased in the study group,as compared with the control group with a significant difference(P0.05),and no obvious adverse reactions occurred.Conclusion:Carvedilol based on conventional therapy may be effective to patients with chronic CHF of NYHA grades Ⅱ or Ⅲ,with fewer adverse reactions.

Objective To observe the clinical effect of carvedilo1 in treatment of chronic congestive heart failure and its toxic and side reaction.Methods 70 cases of chrome congestive heart failure caused by various factors were randomly and equally divided into two groups.The treatment group was given cardiac glycoside,diuretics,vasodilators and an gi0-tensin-eonveting enzyme inhibitors,and the observation group was treated with carvedilol on the basis of the above mentioned drugs.The initial dosage of carvedilol was 2.5 mg,bid,and duration of treatment was over 3 months.Results In the observation group the excellent effective rate and the overall effective rate were 48.6% and 91.4%,respectively,an d higher than that in control group(28.6% and 68.6%,respectively).The statistical difference was significant between the two groups(P O.05)in the overall effective rate.The LVEF and LVEDD improved in the observation group compared with that before treatment and the difference was significant statistically.Conclusion Carvedilo1 has dramatic effect of treating chronic congestive heart failure,and the toxic and side reaction is mild.So it is worth spreading application in the usual treatment of chronic congestive heart failure.

The 236 patients with congestive heart failure were divided into 2 groups: (A) 139 patients(M 102, F 37; age44 ± 10 a)were treated with digoxin 0.125-0.25 mg qd or bid and hydrochloro-thiazide 50-100 mg qd po; and(B) 97 patients(M 73, F 24; age 47± s 10 a)were given isosorbide dinitrate 10 mg, 3-6 times/d in addition. The courses of treatment were ≥ 3 wk in both groups. The results showed better cardiac functions and lower heart failure scores in group B than group A.
