    Application of Lung Spiral CT Scan in the Diagnosis of Intrapulmonary Metastases from Lung Cancer

    Abstract:
    Objective: To evaluate the appearance and the values of application of lung spiral CT in patients with intrapulmonary metastases from lung cancer.Methods: Analyzed retrospectively the imaging data for forty-five patients with diagnosis of intrapulmonary metastases from lung cancer. Results: Among 45 patients,hematogenous metastases were found in 23 case,lymphatic metastases in 8 case,bronchial spread in 5 cases;mixed metastases in 9 cases. Conclusion: The lung spiral CT scan have different appearance in intrapulmonary metastases and could accurate identify their metastatic paths,and could provide the scientific evidence when diagnosis and clinical intrapulmonary metastases from lung cancer.

    Globally, carcinoma of lung is the predominant cause of cancer deathamong both men and women. While hematogenous spread from primarylung cancer to multiple other organs is frequently reported, metastases ofmalignant tumors to diaphragm and spleen are rare. Nowadays, Positronemission tomography (PET) with 18 F- fluorodeoxyglucose (FDG) hasemerged in such a way that it has become an effective imaging technology for the evaluation of different carcinomas, particularly for cancerstaging and follow up after therapy. PET scan is able to provide metabolicinformation. In this case, we present an attractive unified PET/CT scanimage in a patient with lung carcinoma having unusual diaphragm andsplenic metastases. So, PET/CT imaging could strongly identified rareand unusual metastatic sites of cancer and added more appropriate stagingin patient with carcinoma of lung.

    肺癌脳転移の発生頻度は8.8%で大細胞癌>小細胞癌>腺癌>扁平上皮癌であった.初発症状は多彩で脳転移までの期間は腺癌が長く, 脳転移先行例も腺癌が多かった.脳CTから腺癌は単発, 充実性, isodensityが多く, 円形, 病巣周辺の高度浮腫といった特徴がみられた.治療は放治+化療例が多いが予后は開胸術+開頭術がよく, 生存例も多い.死因は, 脳転移を直接死因とする例が半数を越えていた.

    Due to local ventilation and perfusion differences in the pulmonary lobes, the microenvironmental influence on metastasis formation in the lung can be studied. We, therefore, investigated whether the anatomical distribution of first lung metastases follow a particular pattern.Thirty-three out of 273 patients with melanoma who underwent 18F-fluorodeoxyglucose positron emission tomography and computed tomography (18FDG-PET/CT) were identified as patients with detected primary pulmonary metastases. All solitary metastases were allocated to the appropriate lung segment.Segment L3 had the significantly highest number of metastases (n=11; p<0.001). Overall, both upper lung lobes manifested numerously more metastases in comparison to the lower lobes (26 metastases (70%) vs. 11 metastases (30%); p<0.001).Our results provide novel information supporting the hypothesis that pulmonary metastases occur prevalently in the upper lung segments.

    Objective:To discuss the MRI features and its diagnostic valve in brain metastases of lung cancer.Methods:MRI manifestations of 65 cases with brain metastases of lung cancer were retrospectively reviwed.Results:65 cases with lung cancer underwent the MRI examination of head,of which 42 cases were adenocarcinoma,14 cases were squamous cell cancer,8 cases were small cell lung cancer,and 1 case were alveolar carcinoma.Among all these cases,17 cases showed ring-like enhancement,28 cases nodular enhancement,14 cases mixed enhancement,3 cases patch-like enhancement,and 4 cases displayed linear meningeal enhanment.Conclusion:MRI with enhancement and special scan series,which has remarkable characteristics such as multiple ring-like enhancment,nodular enhancment lesion and obvious edema around the lesion,is very helpful to the detection of brain metastases of lung cancer.

    Initial studies suggested a low prevalence of nodal metastases in T1 lung cancer.However,several recent studies have reported a relatively high prevalence of mediastinal lymph node metastases.The extrathoracic metastases in T1 lung cancer differs widely(0%-24%).In this article,we discuss the thinsection CT and FDG-PET findings in T1 non-small cell lung cancer and also discuss the correlation of CT findings with prognosis and survival,the correlation between tumor size and survival,and extrathoracic metastasis of T1 lung cancer.
