Abstract:
Many authors agree that preliminary traction prior to closed or open reduction for congenital dislocation of the hip is helpful. Different ways of traction have been used and each of them has its own advantages and disadvantages. One of the problems in the very young child is the difficulty in maintaining a suitable traction that is biomechanically effective. We found that using a rocker bed made the traction more "user friendly" for the child, the parent and the doctors.Cite

Purpose: Supracondylar humerus fractures are among the most common surgically treated pediatric fractures. Displaced supracondylar fractures are generally treated with open or closed reduction and percutaneous pinning. Multiple attempts at reduction may increase the risk of neuropraxia, elbow stiffness, myositis ossificans, and anesthetic complication. We reviewed the complications developing after treatment with wire traction for femoral neck fractures in thirty elderly female patients ranging in age from 69 to 90 years. Eight cases of wire movement, six cases of local infection, and nine cases of psychological symptoms were recorded during traction. Regarding the mobility of knee joint and quadriceps muscle power at four weeks postoperatively, no significant difference was found between traction through the distal part of the femur and the tibial tuberosity.

肩関節脱臼骨折の中でも稀である上腕骨2 part外科頚骨折を伴った脱臼骨折に対し,K-wireを使って骨頭を経皮的に整復した後に,Fox tail nailを使って骨折部を展開することなく骨接合を行った症例を報告する.【症例】68歳 女性,2007年10月,自宅廊下で足を滑らせて転倒し受傷した.救急搬入後に静脈麻酔下での徒手整復を試みるも整復できなかったため,手術場で全身麻酔下にK-wireを経皮的に刺入して骨頭を整復した.受傷後5日目に外科頚骨折に対しFox tail nailを使った骨接合術を行った.術後1年,骨癒合,関節適合性は良好で特に障害なく日常生活を行っている.肩関節脱臼骨折において脱臼した骨頭が経皮的に整復できれば,その後の骨折に対する治療法の選択肢が広がり,より低侵襲で適切な治療を行うことができる.経皮的整復は脱臼骨折の型に構わず試みてよいのではないかと考えている.

We believe that spontaneous reduction of fracture-dislocation of the cervical spine may occur. We found two groups of patients to have spontaneous reduction based on resultant operative findings. Group 1: Initial radiograph on admission showed normal alignment but operative findings showed a pattern of fracture dislocation. Group 2: Radiograph at previous hospital showed fracture dislocation but initial radiograph on admission showed normal alignment. Group 1 had 9 patients and Group 2 had 3 patients. The results indicated that we must consider that spontaneous reduction of a fracture dislocation of the cervical spine can occuve in patients with acute injury to the cervical spine.
