We describe a "split cannula method" for accurate placement of a deep brain stimulation electrode. This method eliminates the risk of displacement of the electrode when withdrawing the outer guide cannula.A separate short guide cannula is covered over the distal part of a long cannula for microrecording or macrostimulation. After confirmation of the stimulation point, the short cannula is fixed with an accessory-holding device and the long cannula is pulled out. The electrode for deep brain stimulation is inserted through the short cannula. Because this cannula is much shorter than the electrode, securing the stimulation electrode at any withdrawal stage of the outer guide cannula is always possible.With this simple method, we experienced no displacement of the stimulation point during stereotactic insertion of a deep brain stimulation electrode. This method can be easily adapted to any stereotactic frame.

[Objective]To understand the characteristics and causes of accidental injury of the elderly in community,in order to provide bases for preventing the occurrence of accidental injuries.[Methods]Retrospectively summarized the reports about 384 elderly cases of accidental injury in community.[Results]Homes and roads were the main sites for injury accidents;falling down and falling in accidental injuries were the main reasons for the elderly;accidental injury easily leaded to fractures of the elderly.[Conclusion]Physical characteristics of the elderly easily lead to accidental injury;improving the environment and facilities,preventing falls and preventing osteoporosis are the focuses in preventing accidental injuries;it should establish and improve accident prevention network.

When ascitic fluid is withdrawn from the peritoneal cavity through the cannula ordinarily used for this purpose, the end of the cannula frequently strikes loops of intestine or the omentum before the desired amount of fluid has been removed. This not only interferes with further withdrawal of fluid, but causes pain and anxiety in the patient. The end of the cannula can be freed from the intestine or omentum by manipulation, and additional amounts of fluid can be withdrawn. Such additional amounts, however, are usually small, and, after manipulation has been attempted several times, further effort at complete drainage has to be abandoned. The result is that a considerable amount of ascitic fluid may be left in the peritoneal cavity. An artery may be entered on the dorsum of the hand with the mistaken belief that it is a vein. Intraarterial injection of drugs is one of the dreadful consequences of accidental intraarterial cannulation. In this case of a 3-month-old infant, we emphasize the fact that careful observation can prevent unintentional intraarterial drug injection via an 'assumed intravenous' cannula and prevent the associated morbidity. When there is a suspicion of an intraarterial placement of venous cannula, it is of paramount significance to confirm before the injection of medications. Aberrant arterial anatomy should be kept in mind, particularly in children on the dorsum of the hand, where placement of an intravenous cannula is usually considered safe.

The chapter gives a concise overview of the patient and parent characteristics, assessment of such patients and differential diagnoses to consider in the process of managing children in these situations. I present a new puzzle that concerns Aristotle’s accidents. This puzzle arises when applying a basic requirement of accidentality to the variety of cases Aristotle provides. In short, Aristotle seems to offer, now the thought that a is accidental to b, and now that b is accidental to a; but if accidentality is asymmetric, as it seems to be, then a’s being accidental to b implies that b is not accidental to a. One might offer a schooled Aristotelian solution, allowing that while a is in a sense accidental to b, b is accidental to a in a quite different sense. But, as I will argue, this solution does not work, for there are cases in which a and b are accidental to each other in the same sense. Ultimately, the solution to the new puzzle relies not on distinguishing between senses of ‘accident,’ but rather on unearthing a new feature of accidentality: accidentality is contextual, in a sense to be defined in the paper.
