With the increasing failure rae with time of cemented total hip arthroplasty there is a growing interest in uncemented total joint replacement. This review of 88 Isoelastic total hip arthroplasties, and 28 Isoelastic hemiarthroplasties suggests that this implant gives results comparable with cemented total hip arthroplasty at a similar time. The Isoelastic hip replacement system uses uncemented components that have been designed to closely approximate the biomechanical properties of the bone into which they are implanted. The surgical technique is demanding, and orthopaedic surgeons who anticipate using uncemented implants should receive thorough training before embarking on an implantation.

Background: The majority of people with hip arthritis get significant pain alleviation after undergoing total hip replacement.Objective: Evaluation of both early clinical as well as radiological outcomes of cementless total hip arthroplasty in young adults.Subjects and Methods: This was an interventional clinical trial that included 18 patients who were recruited from Orthopedic Department and had osteoarthritic hip who were treated with total hip arthroplasty (THA).Harris hip score (HHS) was utilized for clinical evaluation at 6 weeks, 3 months, and 6 months after surgery, and radiographs were evaluated for loosening signals at the end of 3 months and 6 months.Results: All HHS items improved significantly from 28.3 ± 15.9 to become 85.9 ± 6.7 with a p-value < 0.001 and the percent of improvement (70.4%) ranged from 43.1% to 89.2%.Postoperative HHS was significantly higher in patients without complications compared to those with complications, among males when compared to females, in cases under the age of 60 compared to those over the age of 60, and in cases with osteoarthritis compared to those who had avascular necrosis.Conclusion: Cementless total hip arthroplasty (THA), offers promising outcomes where it led to pain reduction, functional recovery, and enhanced quality of life.

人工股関節全置換術(THA)を受けた116症例について,機能回復状況から回復に影響する要因と経時的な障害モデルを把握する為,術前から術後24ヶ月までを日整会股関節判定基準(Hip Score)と股関節周囲筋力を測定した。分散分析で回復状況を分析し,重回帰分析で要因を抽出し,構造方程式モデリング(SEM)で障害モデルを推測した。Hip Scoreの変化は術前56.3±13.9点から術後24ヶ月で88.3±8.6点と有意に回復が認められ,重回帰分析では術後の機能回復に影響を与える要因として年齢,手術既往数,非術側筋力,合併症の有無が抽出された。SEMの結果は,Whyteの示した2つの障害モデルを含んでいた。

Introduction : Orthopaedic surgery results in significant blood loss. There are no studies that can aid the surgeon in the African region estimate the expected blood loss after total hip replacement. We conducted a study to quantify the blood loss following total hip arthroplasty and to determine the factors associated with this blood loss. Methods : We conducted a cohort study of primary total hip arthroplasties and estimated the mean blood loss using a predetermined formula. Results : The mean blood loss was estimated to be 4.3g/dl and was associated with higher pre operative haemoglobin concentration and increasing duration of surgery. Conclusion : Total hip arthroplasty is associated with high levels of blood loss. The reduction of surgical time and pre operative autologous blood donation may reduce this loss and potentially reduce the need for post operative allogeneic blood transfusion. Total hip arthroplasty is an operative procedure in which the diseased and destroyed hip joint is resected and replaced with a new bearing surface. Since its introduction into the surgical management of the arthritic hip in the early 1960s, total hip arthroplasty has helped alleviate the suffering of millions of patients throughout the world. Patients with coxarthrosis can now look to total hip arthroplasty as a reliable means of pain relief and improved function. While the operation has been successful over the years, problems have manifested themselves as the length of follow-up has increased. As total hip arthroplasty approaches its fourth decade of use, we outline these problems and the proposed solutions.
