The bureaucratic, matrix, and organic organizational models are analyzed to determine the relationship of organizational structure to successful integration of clinical nurse specialist and nurse manager role functions. The shared-governance structure is proposed to best facilitate the interaction required for mutual attainment of organizational goals. The ideas discussed are based on three assumptions: (1) both the clinical nurse specialist and the nurse manager are master's prepared; (2) the clinical nurse specialist and the nurse manager, in collaboration, are more effective in achieving organizational goals than either would be working independently; and (3) some organizational structures are more facilltative and supportive of the collaboration process than others.

Many nurse manager orientation programs are loosely structured and fail to provide new managers with the experiences and support that will assure them a smooth transition. The authors describe an orientation program for new nurse managers that combines 3 components-nurse manager competencies, precepting by the nurse manager's supervisor, and written and classroom resources that, together, assure that new managers are ready to face the challenges that await them.

First-line nurse managers play a critical management role because they greatly influence the success of healthcare organizations. Several studies have profiled the first-line nurse manager, but have focused on the characteristics and responsibilities of these individuals. This study delineated and identified specific behavioral competencies that are considered important for hospital-based nurse manager effectiveness. The results of the survey are relevant for the selection, preparation, and development of effective hospital-based nurse managers.
