Abstract:
To evaluate peripheral neuropathy in patients with vibration syndrome, an examination was conducted of sensory nerve conduction velocity (SCV) in the digital segment of the median nerve in the middle finger and vibration perception threshold (VPT) at 125 Hz on the same middle fingertip. In addition, possible correlations were investigated between the two measurements. SCVs in the digital segment were measured by stimulating at the wrist electrically and recording from two pairs of electrodes in the finger. Fractionated SCVs were also measured in the palm-to-finger, wrist-to-palm, and elbow-to-wrist segments. The subjects were 52 patients with vibration syndrome and 40 healthy controls of similar age. SCVs in the digital segment and the wrist-to-palm segment were significantly slower in the patients than in the controls, and VPTs were higher in the patients. The strongest correlation of VPTs with SCVs among nerve segments measured was shown in the digital segment. With an increase in VPTs, SCVs in the digital segment tended to be slower, and slowed digital SCVs were encountered more frequently: 13% in VPTs below 5.0 dB and 56% in VPTs above 17.5 dB. Slowed digital SCVs were found in 43% of the patients and increased VPTs were encountered in 92%.Keywords:

Objective] The aim was to study the effect of the manganese intoxication and exposure on the conduction velocity of peripheral nerves. [Methods] In 40 observed objects and patients of case group,and 48 persons of control group.the conduction velocity of peripheral nerves was examined by electromyography. [Results] The results showed that there is a significant difference in the peripheral conduction velocity in which the sensory conduction velocity is much slower and the distal latency is longer in case group ( P 0 01). [Conclusion] Manganese was harmful to not only central nerve system but also peripheral nerve system. The peripheral nerves conduction velocity can be used as a reference parameter for diagnosis of the chronic manganese intoxication.

The methods used to measure the conduction velocity of peripheral motor nerves in arms and legs and of the distal part of sensory fibres in the hand are demonstrated. Normal values in infancy, childhood and adolescence are given and discussed in relation to the maturation of the peripheral nervous system. This maturation is further elucidated by a study of the conduction velocity of peripheral motor nerves in different groups of mentally retarded children. The normal values in different ages also serve as a reference with which to compare a value found in a child with a possibly impaired function of the peripheral nerves. Conduction velocity of peripheral nerves is normal in diseases affecting the anterior horn cells or the muscles. It is abnormally low in diseases primarily localized in the peripheral nerves. It is thus a diagnostic tool in establishing a diagnosis of polyneuropathy, a condition, which may be particularly hard to diagnose clinically in children because of the difficulties in performing a good sensory examination in young child. The clinical conditions, in which measuring of conduction velocity of peripheral nerves may be of great help, are discussed. The technique of performing electromyography is briefly reviewed. The findings in normal children of different ages are discussed in relation to the maturation of the neuromuscular system. Abnormalities typical of different diseases of the peripheral neurones and the muscles are described. Finally, the clinical conditions are discussed in which electromyography may be of diagnostic help.

To observe the effect of buflomedil on clinical symptom and nerve conduction velocity (NCV) of diabetic peripheral neuropathy patients.58 cases of diabetic patents with peripheral neuropathy were divided into diabetes group treated with buflomedil (DB) 100 mg.d-1 and diabetes control group (DP) treated with PGE1 200 micrograms.d-1. NCV was measured and peripheral nerve symtom was observed in two groups before treatment and after 2 weeks.NCV and peripheral nerve symptom were obviously improved both in DB group and DP group (P < 0.01). The effective rate were respectively 80.0% and 89.3% in DB group and DP group. The diversity was no significant in DB group and DP group (P > 0.05).buflomedil is an effective and safe drug for treating diabetic peripheral neuropathy patients.

After a neurorrhaphy, there will be a distal disconnection between the cortex and skin receptors, along with interruption of sensibility information. This report demonstrates the efficacy of a new sensory re-education program for achieving optimal sensation in a relatively short time. Between 1999 and 2001, in the authors' Hand Rehabilitation Department, 11 patients with previous neurorrhaphy were subjected to a program of early “compensatory sensory re-education.” Lesions were caused by clean cut. There were 13 primary digital nerve procedures, 12 at the distal palmar MP level, and one at the radial dorsal branch of the index (just after emerging from the common digital nerve). The technique of compensatory sensory re-education was based on a previous, but modified, sensory re-education method.

Clicking at the wrist, in association with movements of the fingers, has been commonly but erroneously called "trigger wrist". In true trigger wrist, triggering occurs only in relation to movements of the wrist. The authors report a case of true trigger wrist in a 23-year-old professional tennis player. Triggering was caused by a "synovial mass" around partially destroyed tendons of the ECRB, ECRL, and EPL at the level of the extensor retinaculum.

Abstract Digital nerve conduction velocities in the median and ulnar nerves were measured in order to obtain a more accurate indication of peripheral nerve impairment induced by hand‐arm vibration. The subjects were 26 patients with vibration syndrome and 28 healthy controls, both groups of about the same age. Two pairs of surface electrodes were attached to the finger and one pair was positioned on the palm, while electrical stimulation was applied to the nerve at the wrist and elbow. Then sensory nerve conduction velocities were antidromically measured within the finger, and also from the wrist to the palm and from the elbow to the wrist. Slowed conduction within the finger was mostly found among the patients. The slowing was also encountered from the wrist to the palm across the carpal tunnel. Abnormalities were observed in the digital nerve segment in about 50–70% of the patients, and in the wrist‐to‐palm segment in about 10–25% of them. The present study indicates that digital nerve impairment predominantly occurs in the vibration‐exposed patients, and that measurement of digital nerve conduction velocity can be a useful objective means to detect peripheral neuropathy in vibration syndrome.

A technique is described for stimulating and recording from nerves in the finger using surface electrodes. A decrease in amplitude and velocity was found with increasing age. In control subjects the digital potential was approximately one and a half times larger than the potential recorded at the wrist. In patients with carpal tunnel syndrome there was some reduction in amplitude and velocity of the digital potential, but the changes were more marked at the wrist. In diabetic patients more uniform changes were found in the two segments. The technique was particularly useful in enabling conduction velocity to be calculated in the digital nerves when no potential could be recorded at the wrist.
