The data on the surgical and combined treatment of 35 cases of lung cancer associated with pleural effusion detected during surgery have been evaluated. Mean 5-year survival was 68.5%, pleural effusion--70%.

OBJECTIVES:To evaluate the value of measurement on apoptosis (APO) in malignant peritoneal effusion cells, and to study the relationship between APO and the therapeutic effect on intracavitary chemotherapy.METHODS:The flow cytometry’s Annexin V/PI method was used in measuring the APO level of peripheral blood cells and malignant peritoneal effusion cells before and after intracavitary chemotherapy.RESULTS:The APO rates of peripheral blood cells [(4 92±3 07)%] and peritoneal effusion cells [(1 81±1 25)%] in malignant tumor patients were both higher than the APO rate of peripheral blood cells [(1 70±1 18)%] in normal control group. However, there was no statistically significant difference between malignant peritoneal effusion [(1 81±1 25)%]in untreated patients and peripheral blood in normal control group in APO.The APO rate of malignant peritoneal effusion cells after treatment was higher than that before treatment. There was a more significant difference between them in the patients before treatment [(1 81± 1 25)%] and after treatment [(8 29±6 45)%] in effective or evidently effective group, P 0 01,as well as between them in effective or evidently effective group and ineffective group[(2 29±2 03)%], P 0 05.CONCLUSIONS:The dynamic analysis of the APO level of malignant peritoneal effusion cells before and after intracavitary chemotherapy not only helps to select effective drugs but also to evaluate the therapeutic effect and prognosis.

The present study was performed to clarify the relation of cause and effect between pleural effusion and hypoplastic lung in the human fetus. In six cases with a duration of less than two weeks between the initial diagnosis of pleural effusion and delivery, lung weights were within the mean weight +/- 1.5 S.D. In fourteen cases with a duration of more than two weeks between diagnosis and delivery, seven cases had lungs with weights of less than -2.0 S.D. and twelve had lungs of less than -1.5 S.D. in weight. The incidence of lung weights less than -1.5 S.D. or -2.0 S.D. was significantly higher in cases with a diagnosis-delivery duration of more than two weeks, compared to those with a duration of less than two weeks. These results suggest that the development of hypoplastic lung in fetuses with pleural effusion may depend on the length of the presence of pleural effusion.

Objective To investigate the diagnostic value of determination of pleural effusion or ascitic fluid, serum survivin contents for differentiating benign from malignant pleural effusion or ascites fluid and analyze the correlation between serum and pleural effusion or ascitic fluid.Methods22 patients with pleural effusion or ascites were detected for survivin level in serum, pleural effusion or ascites fluid.ELISA was taken to detect the survivin in serum, pleural effusion or ascites fluid.ResultsSurvivin of serum, pleural effusion or ascites fluid is elevated significantly in patients with tumors than those with benign dieases(P0.05).Survivin of serum is consistently increased with negative correlation with survivin level in pleural effusion or ascites fluid indicated by correlation analysis(P0.01).ConclusionSurvivin may be a valuable signal in the differentiating benign from malignant pleural effusion or ascites fluid.Determinate survivin of serum, pleural effusion or ascites fluid may be useful in the distinguishing of effusion or ascites.

Objective To discuss the diagnostic value of peritoneal effusion detection by emergency-ultrasound in patients with closed abdominal injury. Method From August 2006 to June 2009,212 patients with closed abdominal injury were studied to evaluate peritoneal effusion detection by emergency ultrasound. Results of 212 patients, peritoneal effusion frequency rate was 78.8%( 167/212), meanwhile, abdominal paracentesis confirmation ratio was only 46.2%(98/212). In the follow-up, 13 patients with injuried hollow viscera and 1 patient with rupture of kidney showed peritoneal effusion. The volume of abdominal fluid was increasing in 17 patients, which needed to be managed by surgery. The accuracy rates were respectively 78.3%( 112/143) and 36.1%(13/36) in the solid organs and the hollow organs. Conclusion During the course of diagnosis and treatment in closed abdominal injury, peritoneal effusion monitoring by ultrasound should be used routinely, which can help to decrease the rate of misdiagnosis and avoid delayed treatment. The authors report a new case of rheumatoid pleurisy proved by the clinical contest and the biological samplings of pleural fluid: a pleurisy with cholesterol pleural effusion. Six observations of rheumatoid pleurisy with cholesterol pleural effusion were found in the literature. This new case confirms this biological peculiarity of rheumatoid pleurisies and the authors insist on the existence of acute pleurisies with cholesterol pleural effusion.

A recurrent unilateral pleural effusion developed without obvious cause in two patients with cirrhosis of the liver. By the demonstration of the rapid passage of a radiolabelled colloid from abdomen to thorax, these effusions were proved to be secondary to clinically undetectable peritoneal effusions. A diaphragmatic tear, which had occurred during a previous splenectomy and which was apparent only at autopsy, was the cause of peritoneopleural communication in one patient. Previous surgery could also have been responsible for the pleural effusion in the other patient.
