Increased T-Cell Activation and Th1 Cytokine Concentrations Prior to the Diagnosis of B-Cell Lymphoma in HIV Infected Patients

To review the clinical characteristics and treatment outcomes of primary intestinal non-Hodgkin's lymphomas (PINHL) and analyze the differences between T-cell and B-cell lymphomas.The characteristics of PINHL patients treated in our hospital between January 2003 and December 2010 were reviewed for their clinical manifestations, diagnosis, endoscopic findings, treatments and outcomes.Eighty-five cases of PINHL meeting the Dawson's criteria were identified. The median age of the patients at the time of diagnosis was 52 years and the male: female ratio was 3.05:1; 58 cases (68.2%) had B-lineage and 27 cases (31.8%) had T-cell lineage lymphomas. Compared to those with B-cell lymphoma, patients with T-cell lymphomas showed a younger age of disease onset (32 vs 56 years, P<0.01) and presented with a greater incidence of such symptoms as fever, hematochezia, diarrhea and night sweats (P<0.05); T-cell lymphoma showed more multifocal and ulcerative/ulcero-infiltrative lesions under endoscope with a longer diagnosis time (4 vs 2 months, P<0.01) and a greater likeliness of misdiagnosis (16/27 vs 12/58, P<0.01) and poor prognosis. Extranodal NK/T-cell lymphoma was the most common type of T-cell lymphomas.In our cases, T-cell lymphoma appeared to be more common than B-cell lymphoma with a younger onset age, more difficult diagnosis, a greater likeliness of misdiagnosis, poorer prognosis and more extranodal NK/T-cell lymphoma.

Objective To study the relationship of Epstein Barr virus (EBV) and T cell lymphoma.Methods Sixty cases of T cell lymphomas were examined for the presence of EBV using in situ hybridization for EBV encoded RNA (EBERs).Results EBERs were detected in tumor cells in 37(69.8%) of 53 cases with peripheral T cell lymphoma,but in none of seven cases of precusor T lymphoblastic lymphoma.The total detected EBERs were 37(61.6%) in 60 cases of T cell lymphomas.By Revised European Amercan Lymphoma(REAL) classification,EBERs were detected in 2/2 angioimmuno blastic T cell lymphoma,17/18 angiocentric lymphoma, 4/6 anaplastic large cell lymphoma and 14/27 peripheral T cell lymphoma,unspecified (51.9%).The frequency of EBERs among the extranodal peripheral T cell lymphoma was higher than the nodal ( P 0.01),there was no significent correlation with the sex,age and clinical stage.Conclusion This study indicated that high incidence of EBV was observed in peripheral T cell lymphoma,with predilection for angiocentric lymphoma and extranodal presentation.

objective:To analyze the clinical manifestation,diagnosis,mis-diagnostic reasons and treatment method of orbital NK/T cell lymphoma.Methods:The literature was reviewed and 2 patients with orbital NK/T cell lymphoma were collected.Then we analyzed the diagnosis,differential diagnosis and therapy of orbital N K/T cell lymphoma.Results:Orbital NK/T cell lymphoma was one kind of relatively rare,new reported extra-nodal lymphoma,which was usually mis-diagnosed as conjunctivitis,idiopathic orbital inflammation,periorbital area inflammation.The orbital NK/T cell lymphoma and nasal NK/T cell lymphoma shared similar stopathology,immunopathology.Conclusions:The orbital NK/T cell lymphoma shows distinctive cliniopathologic,immunophenotypic and characteristic CT or MRI manifestations.The patients are diagnosed difficultly in the early stage,presented highly aggressive clinic course and a very high lethality.

Objective To compare the detection rate of Epstein-Barr virus latent membrane protein 1 (LMP1) 30 base pair deletion in extranodal nasal type NK/T-cell lymphoma with that in chronic inflammation of nasopharynx and tonsillitis; and to analyze the prognostic significance of LMP1 deletion in extranodal nasal type NK/T-cell lymphoma.Methods Polymerase chain reaction was used to detect the deletion of LMP1 in 55 cases of extranodal nasal type NK/T-cell lymphoma and 19 cases of chronic inflammation of nasopharynx and tonsillitis.Follow-up information of 1 to 58-month duration was available in 33 patients.Results In all the 55 extranodal nasal type NK/T-cell lymphoma cases studied, 9 cases contained the wide-type or predominantly wide-type LMP1.On the other hand, 46 cases contained the deleted or predominantly deleted LMP1.In the non-lymphoma control group, 16 cases contained the deleted or predominantly deleted LMP1.However, no statistically significant difference was found in the detection rate of 30 base pair deleted LMP1 between extranodal nasal type NK/T-cell lymphoma and control group (P0.05).The prognosis of deleted or predominantly deleted LMP1 in extranodal nasal type NK/T-cell lymphoma was worse.Conclusion Though 30 base pair deletion of Epstein-Barr virus LMP1 may not be an important pathogenetic step in extranodal nasal type NK/T-cell lymphoma, it may play some role in tumor progression.

Objective To study the pathological features of T and NK cell lymphomas and the relative frequency of each type of these lymphomas.Methods 152 cases of T and NK cell lymphomas selected from archival tissues of lymphoid neoplasms diagnosed during the period of 1985~2005 were studied based on morphology,immunohistochemistry and in situ hybridization for Epstein-Barr virus(EBER) and correlated clinical findings,and reclassified according to new WHO classification(2001).Results The mean age of all the patients was 43 years old,ranged from 4 to 80 years old.The ratio of male to female was 3 to 1.There were 83 cases(54.6%) of extranodal lymphomas and 69 cases(45.4%) of nodal lymphomas.Ten types were identified in 152 cases of T and NK cell lymphoma.The most frequent type of the lymphoma was extranodal nasal NK/T cell lymphoma(52 cases,34.2%), followed by unspecified peripheral T cell lymphoma(31 cases,20.4%),precursor T lymphoblastic lymphoma(25 cases,16.4%) and anaplastic large cell lymphoma(21 cases,13.8%).Epstein-Barr virus was detected in 51 of 52 cases(98%) of extralnodal nasal NK/T cell lymphoma.Conclusions T and NK cell lymphomas show great morphological diversity.And each type has certain pathological features,which are the base of further classification.The most common type is extranodal nasal NK/T cell lymphoma.Frequency of T and NK cell lymphomas showed in this study in China is different from that in European and American countries,suggesting there may be different etiological factors.
