Abstract:
Anterior cervical spine fusion and stabilization is a well-recognized procedure for a number of cervical spine disorders. Unfortunately, the complex anatomy of the cervical spine means that these procedures are not without complications. Pharyngo-oesophageal perforation is a rare but potentially life-threatening complication of cervical spine surgery and may present intra-operatively, in the immediate post-operative period or many years later. We present the case of a gentleman with ankylosing spondylitis who presented with a pharyngeal perforation and fistula five years after cervical spine surgery.Keywords:

Unusual destructive lesions of the cervical and lumbar vertebrae are described in two patients with ankylosing spondylitis. Biopsy of a lesion from one of the patients showed it to be inflammatory in nature. Possible mechanisms for vertebral centrum involvement in ankylosing spondylitis, centering on the role of enthesopathies are discussed.

Abstract
This is the first of a three part series describing the clinical issues surrounding the radiographic assessment of the cervical spine. Defined in this literature review is the working definition of cervical stability. Described are the “normal” anatomical relationships between the cervical vertebrae for both the adult and the child, as portrayed by lateral radiographs. Also presented is a review of available documentation regarding the normal segmental function of the vertebrae in the upper and in the lower cervical spine. The next two parts in this series will deal with the definitions and radiographic evaluation of cervical hypermobility and instability respectively.

Ankylosing spondylitis is not an uncommon disease worldwide, yet is relatively rare in Bahrain. There is a typical pattern of joint involvement in cases of ankylosing spondylitis, but the presentation of discitis is rare. We present a case of a patient presenting with backache and was diagnosed to have discitis. The diagnosis of ankylosing spondylitis was made only after he was found to be Human Leukocyte Antigen-B27 positive. This is the first case report of ankylosing spondylitis presenting as discitis in Bahrain.

The results of surgical treatment of 39 patients with tuberculous spondylitis, complicated with bronchial fistula, are analysed. The fistula was liquidated in 35 patients. Late results followed up from 3 to 18 years were studied in 26 patients. Bronchial fistula is yet present in 4 patients, who were discharged with its presence.

Two cases of pyogenic spondylitis of the cervical spine are reported. One case was operated on for destructive cervical spine and another for treating the patient's neurological symptoms. Magnetic resonance imaging was useful for making an early diagnosis of this lesion and for evaluating the effects of treatment.

Reports of the pyogenic spondylitis of the upper cervical spine are rare. Furthermore, treatment of pyogenic spondylitis of the thoracic and lumbar spine is generally conservertive, but treatment of the cervical spine requires frequent operation.We surgically the treated pyogenic spondylitis of the upper cervical spine with headache, neck pain, and neurological symptoms. Operation was found to improve the patient's symptoms.

The aim of this study was to compare the remaining motion of an immobilized cervical spine using an innovative cervical collar as well as two traditional cervical collars. The study was performed on eight fresh human cadavers. The cervical spine was immobilized with one innovative (Lubo Airway Collar) and two traditional cervical collars (Stifneck and Perfit ACE). The flexion and lateral bending of the cervical spine were measured using a wireless motion tracker (Xsens). With the Weinman Lubo Airway Collar attached, the mean remaining flexion was 20.0 ± 9.0°. The mean remaining flexion was lowest with the Laerdal Stifneck (13.1 ± 6.6°) or Ambu Perfit ACE (10.8 ± 5.8°) applied. Compared to that of the innovative Weinmann Lubo Airway Collar, the remaining cervical spine flexion was significantly decreased with the Ambu Perfit ACE. There was no significant difference in lateral bending between the three examined collars. The most effective immobilization of the cervical spine was achieved when traditional cervical collars were implemented. However, all tested cervical collars showed remaining motion of the cervical spine. Thus, alternative immobilization techniques should be considered.
