Predictive Validity of the Functional Movement ScreenTM in a Population of Recreational Runners Training for a Half Marathon

Abstract:
PURPOSE: The Functional Movement Screen™ (FMS) has been touted as an easily administered 7-test battery that predicts risk of injury in non-specified athletic populations. A cut score of 14 on the 21 point instrument has been cited as a predictor of basic movement pattern dysfunction and related risk of injury. This study tested the predictive validity of the recommend cut score for the FMS in a sample of recreational runners training for a half-marathon. METHODS: Participants for this study were recruited from the 35,000 individuals registered to participate in the 2006 Indianapolis Mini-Marathon. Sixty individuals were evaluated using the FMS as described. The participants then completed a weekly online survey as a means of tracking training and injury status during their preparation and participation in the event. In each survey participants answered questions related to training variables, subjective pain ratings, and symptoms which may have changed training or prompted participants to seek medical attention. Data was collected for a four-month period leading up to the completion of the half-marathon. RESULTS: FMS scores ranged from 11 to 20 on the 21 point instrument. Forty- nine of the 60 participants (82%) completed the weekly questionnaires for the duration of the study. Of these 49 individuals, 12 reported symptoms consistent with overuse injuries, making the total incidence of injury 24.49%. Only 1 of the 12 injured individuals scored below the suggested cut score of 14, which generated 8.3% sensitivity and 94.5% specificity, respectively. A cut score of 20 generated the highest sensitivity (100%), identifying all injured participants but resulting in 27.0% specificity. The cut score with the highest specificity was 11 (97.2%), but it had a sensitivity of 0%. The cut score of 17 showed the highest combination for specificity (41.6%) and sensitivity (56.7%). Neither linear nor logistic regression analyses identified FMS scores as predictors of musculoskeletal injury. CONCLUSIONS: Based on the results of this study, the cut scores on the FMS did not positively predict musculoskeletal injury, indicating this instrument lacks predictive validity in this population of recreational runners. These findings suggest further study is needed to establish the predictive validity of the FMS in athletic populations.Keywords:

A test of interests is described which is open‐ended yet objectively scorable and which yields validatory evidence for 20 of the 22 relevant interests. There is some suggestion too that the test may be useful in estimating degree of stability and social adjustment. A number of experiments have been conducted, using as subjects university and other students, naval ratings, secretaries and graduates. The test appears to be suitable for such subjects and also for senior schoolchildren.

Physiotherapy for people with low back pain frequently includes assessment and modification of lumbo-pelvic movement. Interventions commonly aim to restore normal movement and thereby reduce pain and improve activity limitation. The objective of this systematic review was to investigate: (i) the effect of movement-based interventions on movement patterns (muscle activation, lumbo-pelvic kinematics or postural patterns) of people with low back pain (LBP), and (ii) the relationship between changes in movement patterns and subsequent changes in pain and activity limitation. MEDLINE, Cochrane Central, EMBASE, AMI, CINAHL, Scopus, AMED, ISI Web of Science were searched from inception until January 2012. Randomised controlled trials or controlled clinical trials of people with LBP were eligible for inclusion. The intervention must have been designed to influence (i) muscle activity patterns, (ii) lumbo-pelvic kinematic patterns or (iii) postural patterns, and included measurement of such deficits before and after treatment, to allow determination of the success of the intervention on the lumbo-pelvic movement. Twelve trials (25% of retrieved studies) met the inclusion criteria. Two reviewers independently identified, assessed and extracted data. The PEDro scale was used to assess method quality. Intervention effects were described using standardised differences between group means and 95% confidence intervals. The included trials showed inconsistent, mostly small to moderate intervention effects on targeted movement patterns. There was considerable heterogeneity in trial design, intervention type and outcome measures. A relationship between changes to movement patterns and improvements in pain or activity limitation was observed in one of six studies on muscle activation patterns, one of four studies that examined the flexion relaxation response pattern and in two of three studies that assessed lumbo-pelvic kinematics or postural characteristics. Movement-based interventions were infrequently effective for changing observable movement patterns. A relationship between changes in movement patterns and improvement in pain or activity limitation was also infrequently observed. No independent studies confirm any observed relationships. Challenges for future research include defining best methods for measuring (i) movement aberrations, (ii) improvements in movements, and (iii) the relationship between changes in how people move and associated changes in other health indicators such as activity limitation.

The effectiveness of "hands-on" physiotherapy for stroke is unclear. The objective here is to analyze the effectiveness of such interventions on movement-related International Classification of Functionality, Disability and Health (ICF) categories. A systematic review was undertaken of randomized controlled trials published since 1980, using the following criteria: stroke, humans, ≥ 18 years, outcomes related to ICF movement-related categories, physiotherapeutic handling techniques, control group as placebo or no intervention, including experiments where both groups have the same intervention and the experimental group has one extra intervention. Nine studies were included and a best evidence synthesis is presented. Recommendations with limited evidence favor slow-stroke back massage for shoulder pain, range-of-motion exercises for upper-limb and lower-limb structures and functions of muscles and joints, proprioceptive neuromuscular facilitation (PNF) for gait step, walking backwards with hip facilitation for gait parameters and performance, and conventional physiotherapy with facilitation techniques for gait parameters. Recommendations with indicative findings favor PNF with trunk rhythmic stabilizations for function and mobility of upper limbs. Recommendations with limited evidence show the non-efficacy of Bobath therapy for upper-limb function and activity and facilitation of the step on body weight support treadmill training for gait parameters and performance. In conclusion, some hands-on interventions have limited evidence in stroke rehabilitation.

This work presents normative data for Brazilian children on the Rey Auditory Verbal Learning Test (RAVLT), Verbal Fluency Test (VFT), and Stroop test (ST). The effects of age were evaluated. The patterns of results were compared with the literature, and validity was assessed using cluster analysis.

Background It is assumed that the lumbar movement pattern observed during a clinical test is representative of the movement pattern used during a functional activity. Very little is known about how the lumbar movement pattern during a clinical test is associated with the lumbar movement pattern during a functional activity and how the lumbar movement pattern is associated with functional limitation. Objective The purpose was to examine the lumbar movement pattern during a clinical test and a functional activity test in people with and people without low back pain (LBP), and the relationship of lumbar motion to LBP‐related functional limitation. Design Observational study. Participants 16 back‐healthy adults and 32 people with chronic LBP. Methods Participants performed a standardized clinical test of forward bending and a functional activity test of picking up an object. Main Outcome Measurements Maximal lumbar excursion and lumbar excursion at 0% to 50% and 50% to 100% of movement time were examined. Results Significant associations were present between the two movement tests for both back‐healthy people and people with LBP (r = 0.47‐0.73). In people with LBP the amount of lumbar motion in the 0% to 50% of movement time interval for both tests was significantly associated with functional limitation (r = 0.43‐0.62). Conclusion Lumbar movement patterns were similar between the two tests, and lumbar motion early in the movement of a functional test was related to self‐report of functional limitation.
