    Abstract:
    症例は57才,女性.歯肉出血および右下肢の出血斑を主訴に来院.血小板数の著明な低下を認めITPの診断のもとに摘脾を行ったところ,脾臓に非乾酪性類上皮細胞肉芽腫を認めサルコイドーシスと診断された.本症例は,サルコイドーシスに特徴的な皮膚症状,眼症状および呼吸器症状を認めず,血小板減少による出血症状が唯一の症状であったが,その原因として抗血小板抗体による破壊が推察され,極めてまれと考えられた.

    Hemorrhagic diathesis is a collective term. It includes hemophilia, Verlhof's disease, Schnlein-Henoch's disease and scorbutus. Bleeding is something common that unites these diseases, different in their clinical manifestations and, undoubtedly, in their pathogenesis. It is true that due to the development of the teaching of vitamins, scorbutus has fallen out of the purview of hematologists and is now closely related to problems of nutrition. Nevertheless, every clinical case of bleeding, especially accompanied by bleeding gums and spontaneous bruising, always makes the clinician first of all exclude scorbutus and then focus his attention on other forms of hemorrhagic diathesis.

    A 19 year-old female, complaining of gigantic tumor on each breast and hemorrhagic diathesis, was admitted to our hospital. Laboratory data and clinical findings suggested the presence of DIC. Bilateral mastectomies reduced the bleeding tendency within a week. In conclusion, removal of the tumors was effective to avoid fatal hemorrhage.

    Summary A description is given of the coagulation disturbances in six patients with a haemorrhagic diathesis due to Stuart-Prower deficiency. Relatives of these patients, if heterozygous for this deficiency, showed no significant haemorrhagic diathesis; in the laboratory, their one-stage “prothrombin” times showed only a slight prolongation. Thromboplastin formation in these relatives was sufficient, but determination of the Stuart-Prower factor revealed lower values. The relatives in question were incapable of giving the same correction of the thromboplastin generation test in their bleeder relatives as normal serum does. The relation between Stuart-Prower deficiency and Christmas factor is discussed. One patient is described who combined heterozygotism for Stuart-Prower deficiency with alcaptonuria. The mode of inheritance of the Stuart-Prower deficiency is discussed.

    Bleeding diathesis as a cause of menorrhagia should not be overlooked at any age even in the perimenopausal woman. Three cases have been presented, wherein bleeding diathesis was the cause of menorrhagia. Two of these patients had acute myeloid leukaemia and the third had immunothrombocytopaenic purpura. A noteworthy point of this series is that one patient was in her teens, the second in the peak reproductive age group and the third in the perimenopausal age.
