Abstract:
No previous study clearly demonstrated the association between the absolute neutrophil count (ANC) nadir and the absolute monocyte count (AMC) nadir among patients undergoing chemotherapy.We retrospectively reviewed patients with an incurable lung cancer in two university hospital who were treated with platinum doublet or triplet. In this study, the nadir was defined as the lowest value during days 1-22.We observed 75 patients: 61 men (81%) and 14 women (19%), median age of 69 years. The number of days that elapsed between the first day of chemotherapy and the median ANC nadir of 12 days was longer than that for AMC nadir of 6 (Wilcoxon signed rank test: P < 0.001). The number of days that elapsed between the first day of chemotherapy and the AMC nadir was the only factor that had a significant correlation with that for ANC nadir (Spearman's rank correlation: r = 0.34, P = 0.003). Among 75 patients, 23 (31%) had an ANC at nadir <1000/μL. AMC at nadir of 100/μL predicted an ANC at nadir <1000/μL with a sensitivity of 83% and a specificity of 56%.Both neutrophils and monocytes are differentiated from a common progenitor, a granulocyte macrophage colony forming cell, which can provide good explanation for the association between ANC and AMC nadirs. We would like to recommend physicians to observe the AMC nadir to predict the timing and severity of the ANC nadir.Keywords:

Objective To evaluate the effect of treating lung cancer through bronchial artery medication(drug treatment).Method Total 16 patients with lung cancer treated by bronchial artery infusion(BAI),and the changes of the tumor diameter,alleviation of clinical symptoms and complications following infusion were observed.Results The diameter of tumor shrunk to be smaller after BAI,most of the clinical symptoms were also alleviated and no major complication occured.Conclusion The effect and the safety are good after treating lung cancer with BAI.

Objective To evaluate the clinical significance of peripheral blood telomerase activity in lung cancer after chemothenapy. Methods The telomerase activity in peripheral blood was measured by TRAP-ELISA in 49 patients with lung cancer before and after chemotherapy. Its relationship with response was analyzed. Results The change of telomerase activity before and after chemotherapy was dependent on the histology of lung cancer, especially in patients with small cell lung cancer. In 31 patients whose activity was decreased after chemotherapy, 21 cases had response. In contrast, 15 of 18 patients with increased activity after chemotherapy showed no response. Conclusion The change of telomerase activity after chemotherapy appears to be an indicator for response in lung cancer.

Objective To analyze to decrease surgical complications and to improve patients prognoses of treatment of lung cancer of aged.Methods 116 cases of aged lung cancer patients(more than 75 years old) treated by surgery were retrospectively analyzed.Results The first symptoms of aged lung cancer patients were usually cough,chest pain,hemoptysis and fever.Combined diseases before surgery and complications after surgery were high.Conclusion Surgical treatment in the case of strictly controlling indications can increase survival rate of aged lung cancer patients.To correctly deal with combined diseases before surgery and complications after surgery and to enhance after surgery monitoring are the keys to improve prognoses.

Objective: To investigate the expression of ERCC1 and the correlation with platinum-based chemotherapy sensitivity in advanced non-small cell lung cancer(NSCLC) as well as its clinical prognostic significance.Methods:The expression of ERCC1 was examined by immuno histoche-mical technique in 81 patients with advanced non-small cell lung cancer.All patientswere treated with platinum-based chemotherapy.Results:Inthese 81 cases,positive expression rates of ERCC1 was 38.5%(29/81),and there were no correlation with patient age,gender,scor,clinical stage and smoking,but its expression in pulmonary adenocarcinoma was higher than that in squamous carcinoma.The response rate of platinum-based chemotherapy in positive expression group was 20.69%,which in negative group was 55.77%,and there was significant difference between the two groups(P0.05).The medial survival time was 303 days in ERCC1 expression positive group,which was 564 days in ERCC1 expression negative group,survival analysis was carried by Kaplan-Meier analysis and there has significant difference(P0.05).Conclusion:The expression of ERCC1 correlate with paltinum-based chemothempy sensitivity and could predict the sensitivity in advanced non-small cell lung cancer.Positive expression of ERCC1 predicates poor prognosis for patients with advanced NSCLC.The result of this study may provide new sight for tailor chemotherapy(TC) and decision prognosis to individual NSCLC patients.

The influence of several factors for response to Chemotherapy and survival was analysed in 310 patients with inoperable lung cancer who were treated by single or combination chemotherapy. In small cell lung cancer, survival was strongly affected by the results of induction chemotherapy, and combination chemotherapy with three or four drugs was most effective. In non-small cell lung cancer, performance status and body weight loss on admission were important factors for response to chemotherapy and survival. Combinations did not improve the response rate in non-small cell lung cancer.

Objective To investigate lobectomy for lung cancer perioperative management and prevention of complications.Methods 40 patients with lung cancer underwent unilateral lobectomy, patients in their treatment, complications and pathological type and stage were analyzed and summarized.Results The preoperative or intraoperative surgical complications, prevention and treatment, unilateral lobectomy complications: pulmonary infection in 9 cases (22.5%), atelectasis / leak 2 cases accounted for 5.0%, smoking and failure1 case 2.5%, 2.5% of blood clots one case, the other three cases accounted for 7.5%.16 cases of postoperative complications accounted for 40.0%.Conclusion Lobectomy of lung cancer patients with preoperative or perioperative complications of surgery on the prevention and treatment, to make lung cancer patients survive the perioperative period, for treatment and restore health.

Background: Lung cancer is common and has poor survival outcome. Chemotherapy provides modest improvement. Many patients can not afford chemotherapy and it utility in advanced lung cancer in resource limited setting is debatable.Our goal was to assess the effect of chemotherapy on survival Method: 78 cases with advanced Non Small Cell Lung Cancer were followed up. All cases were offered platinum based chemotherapy. Patient characteristics, staging and performance status was noted. Chemotherapy arm was compared with the group not willing for chemotherapy for mortality and other parameters. Relevant statistical analysis was done. Kaplan-Meir survival estimates was generated. Results: 41 patients received chemotherapy and 37 patients did not receive chemotherapy. The 2 groups were well matched. Results of Kaplan-Meir analysis are shown. Conclusion: In advanced lung cancer patients a platinum based chemotherapy provided a significant survival advantage and should be advised to these patients even in a resource limited setting.
